Higher frequencies of BCRP+ cardiac resident cells in ischaemic human myocardium

Aims Several cardiac resident progenitor cell types have been reported for the adult mammalian heart. Here we characterize their frequencies and distribution pattern in non-ischaemic human myocardial tissue and after ischaemic events. Methods and results We obtained 55 biopsy samples from human atria and ventricles and used immunohistological analysis to investigate two cardiac cell types, characterized by the expression of breast cancer resistance protein (BCRP)/ABCG2 [for side population (SP) cells] or c-kit. Highest frequencies of BCRP+ cells were detected in the ischaemic right atria with a median of 5.40% (range: 2.48-11.1%) vs. 4.40% (1.79-7.75%) in the non-ischaemic right atria (P 0.47). Significantly higher amounts were identified in ischaemic compared with non-ischaemic ventricles, viz. 5.44% (3.24-9.30%) vs. 0.74% (0-5.23%) (P 0.016). Few numbers of BCRP+ cells co-expressed the cardiac markers titin, sarcomeric alpha-actinin, or Nkx2.5; no co-expression of BCRP and progenitor cell marker Sca-1 or pluripotency markers Oct-3/4, SSEA-3, and SSEA-4 was detected. C-kit(+) cells displayed higher frequencies in ischaemic (ratio: 1: 25 000 +/- 2500 of cell counts) vs. non-ischaemic myocardium (1: 105 000 +/- 43 000). Breast cancer resistance protein(+)/c-kit(+) cells were not identified. Following in vitro differentiation, BCRP+ cells isolated from human heart biopsy samples (n = 6) showed expression of cardiac troponin T and a-myosin heavy-chain, but no full differentiation into functional beating cardiomyocytes was observed. Conclusion We were able to demonstrate that BCRP+/CD312 cells are more abundant in the heart than their c-kit(+) counterparts. In the non-ischaemic hearts, they are preferentially located in the atria. Following ischaemia, their numbers are elevated significantly. Our data might provide a valuable snapshot at potential progenitor cells after acute ischaemia in vivo, and mapping of these easily accessible cells may influence future cell therapeutic strategies.

Is total aortic arch replacement with the frozen elephant trunk procedure reasonable in elderly patients?

Summary: Total aortic arch replacement using the frozen elephant trunk (FET) technique carries significantly higher perioperative morbidity and mortality risks in septuagenarians compared to younger patients. Long-term survival is notably compromised in older patients. Thorough patient selection and optimization of perioperative management are recommended to improve outcomes.

Surgery for infective endocarditis following low-intermediate risk transcatheter aortic valve replacement-a multicentre experience

Summary: This study retrospectively analyzed 69 patients who underwent cardiac surgery due to TIE at 9 cardiac surgical departments across Germany. The main causative organisms for TIE were Enterococcus faecalis, coagulase-negative Staphylococcus spp., Methicillin-sensitive Staphylococcus aureus, and viridians group streptococci. The survival rates at 6 months and 1 year were found to be 68% and 53%, respectively.

The results show that the hemodynamic parameters of decellularized long aortic homografts are similar to those of healthy controls and patients who underwent valve-sparing aortic root replacement 2.5 years postoperatively.

Frozen elephant trunk in acute aortic type a dissection: risk analysis of concomitant root replacement

Summary: Full root replacement does not increase the perioperative risk in patients who undergo frozen elephant trunk for acute aortic dissection. Careful patient selection is important for such complex procedures. Continuous myocardial perfusion can help reducing the risk for intraoperative complications during such complex operations.

Targeting the Pentose Phosphate Pathway for SARS-CoV-2 Therapy

Summary: SARS-CoV-2 infection induces a metabolic shift towards glycolysis in host cells, with the glycolysis inhibitor 2DG being developed for COVID-19 treatment. Enhanced levels of transketolase (TKT) in infected cells suggest a role for the non-oxidative PPP, supported by the TKT inhibitor BOT inhibiting SARS-CoV-2 replication and synergizing with 2DG. Metabolic drugs like BOT and 2DG may improve COVID-19 therapy outcomes by targeting both viral replication and immunopathology.

Is aortic valve-sparing root reimplantation (David-I) justified in cardiac redo surgery?

Summary: Despite more concomitant total arch replacements in the redo group, early mortality was comparable in both groups. AVSRR can be performed in redo cardiac surgery without compromising the early postoperative outcome. Careful patient assessment and selection are crucial when considering the David procedure for patients with a history of previous cardiac surgery.

Performance and feasibility of three different approaches for computer based semi-automated analysis of ventricular arrhythmias in telemetric long-term ECG in cynomolgus monkeys

Summary: Computer-based analysis of long-term ECG monitoring in animal models is a time-consuming process due to the need for manual supervision for accurate arrhythmia detection. This study investigated the performance and feasibility of three ECG interval analysis approaches in detecting ventricular arrhythmias. Results showed that the combined attribute and pattern recognition analysis approach had higher accuracy in classifying detected events and accurately depicting arrhythmia burden. Additionally, this approach significantly reduced working time compared to manual supervision. Therefore, it is a valuable method for cost-effective and time-efficient analysis of large preclinical ECG datasets.

Summary: Abnormalities during trophoblast development can lead to pregnancy-related complications, but the underlying mechanisms are established during the first trimester. Human pluripotent stem cell (hPSC) technology provides a renewable source of cells for studying these diseases. By optimizing the treatment protocol, we were able to generate trophoblast cells enriched for HLA-G(pos) EVT-like cells, which will aid in understanding placenta-related disorders and investigation of immune tolerance towards the fetus.
