The architecture of the left lateral atrial wall: a particular anatomic region with implications for ablation of atrial fibrillation

Aims We examined the left lateral ridge (LLR) between the orifices of the left pulmonary veins and the left atrial appendage for a better understanding of its structural composition relevant to ablations for atrial fibrillation (AF). Methods and results The LLR and its surrounding areas were studied in 40 heart specimens by dissection and histological sections. The LLR is a fold of the atrial wall with a mean width that was narrower superiorly than inferiorly (P < 0.001). Its myocardial thickness at the antero-superior level was thicker than at the postero-inferior level (2.8 +/- 1.1 vs. 1.7 +/- 0.8 mm, P < 0.001). Transmurally from subepicardium to subendocardium, the LLR comprises myofibres from the leftward extension of Bachmann's bundle together with the inferior branches of the septopulmonary bundle and the septoatrial bundle. The vein or ligament of Marshall is located on the epicardial aspect of the LLR. The Marshall structures and autonomic nervous system are in close proximity (< 3 mm) to the endocardial surface at the superior level of the ridge in 70% of specimens. Conclusion The variability in width and thickness of the LLR, its proximity to Marshall structures and autonomic nerves, and myofibre arrangement may be significant in the fibrillatory process and spread of AF activity.

Summary: This study aimed to describe the clinical characteristics of carriers of LMNA genetic variants in a Spanish cardiac-laminopathies cohort and assess risk criteria. The results showed that LVEF <45% and NSVT were associated with major arrhythmic events, while sex or type of genetic variant were not. Female carriers of missense variants with NSVT or LVEF <45% should not be considered a low-risk group. Individualized risk stratification is important for carriers of LMNA missense variants, as prognosis varies among individuals.

The study also found that mortality is higher in immunosuppressed patients, and a combination of cardiac surgery and azole therapy improves the outcome.

Summary: Genotype and left ventricular scar on cardiac magnetic resonance (CMR) are risk markers for adverse outcomes in non-ischaemic dilated cardiomyopathy (DCM). This study investigated the combined influence of genotype and late gadolinium enhancement (LGE) on prognosis in a large cohort of DCM patients. The results showed that classification of patients according to genotype and LGE improved the prediction of malignant ventricular arrhythmias (MVA) and end-stage heart failure (ESHF). Scar assessment with CMR and genotyping should be considered in selecting patients for primary prevention implantable cardioverter-defibrillator placement.

Lack of evidence for longitudinal dissociation of the atrioventricular conduction axis

Summary: We used morphological methods to study the longitudinal dissociation of specialized cardiomyocytes in the non-branching portion of the atrioventricular conduction axis. Evidence for concealed fasciculo-ventricular connections as revealed by His bundle pacing

Summary: This study found that histologically specialised pathways connecting the branching component of the atrioventricular conduction axis with the crest of the muscular ventricular septum exist in about three-fourths of the histological datasets. By observing 34 patients undergoing His bundle pacing, it was found that the QRS complex closely resembled known conduction through fasciculo-ventricular pathways in 32 patients, with only two patients not showing a delta wave at any pacing voltages. Therefore, the conclusion was drawn that His bundle pacing can reveal fasciculo-ventricular pathways concealed during sinus rhythm.
