Summary: This study investigated the incidence, nature, severity, and recovery of early dysphagia in patients following surgical resection of oral and/or oropharyngeal squamous cell carcinoma with a mandibular lingual release approach (MLRA). The results showed that the majority of patients experienced severe dysphagia after surgery, requiring extended hospital admission and prolonged enteral feeding. Some patients still had dysphagia postdischarge.

Summary: This exploratory study suggests that in the treatment of oropharyngeal cancer, TORS may have functional advantages over chemoradiation when adjuvant radiotherapy to the primary site can be avoided, showing improvements in feeding tube duration, penetration/aspiration, and airway protection. Further evaluation is warranted to determine the potential benefits of selecting TORS for patients where adjuvant therapy can be omitted or confined to the neck.

It is recommended that patients with these risk factors undergo comprehensive preoperative assessment, and criteria for preoperative swallowing evaluation should be clarified for identifying suitable candidates for TOS in the future.

Summary: Chemotherapy, in addition to radiotherapy and surgery, is associated with improved overall survival in patients with oral cavity and oropharyngeal cancers. Induction chemotherapy may prolong survival by 8 to 20% and adjuvant concomitant chemoradiotherapy may prolong survival by up to 16%. In patients with unresectable tumours, concomitant or alternating chemoradiotherapy may prolong survival by 10 to 22%. There is insufficient evidence as to which agent or regimen is most effective and the additional toxicity associated with chemotherapy given in addition to radiotherapy and/or surgery cannot be quantified.

Summary: This study aimed to verify whether an ultrasonographic evaluation of swallowing movement facilitates the detection of neurogenic oropharyngeal dysphagia, revealing significant differences in laryngeal duration, laryngeal displacement, and the hyoid bone-laryngeal motion ratio between healthy individuals and stroke patients with dysphagia. Laryngeal duration and the HL motion ratios were identified as factors affecting dysphagia, with high sensitivity and specificity in differentiating between the two groups.

Summary: The study evaluated the feasibility, reliability, and validity of the Swallowing Proficiency for Eating And Drinking (SPEAD) test in patients with dysphagia after treatment for head and neck cancer (HNC) and healthy participants. Results confirmed good to excellent test-retest, intra-rater, and inter-rater reliability of ingestion duration, as well as supporting construct validity of the test through correlations with objective and subjective swallowing outcomes. Initial findings suggest that the SPEAD test can be a useful tool for objectively evaluating and monitoring swallowing capacity in HNC patients.

Summary: The study aimed to evaluate swallowing, mouth opening and speech function of patients with stage III-IV oropharyngeal carcinoma (OPC) during the first year after radiation-based treatment (RT(+)) with the introduction of a dedicated preventive rehabilitation program. Results show that functional outcomes deteriorated until 6 months post-treatment, improved by 12 months but did not return to baseline levels, with a significant proportion of patients still experiencing dysphagia, trismus and speech problems. Despite successful implementation, there is room for improvement in the current rehabilitation program, with pretreatment sarcopenia identified as a potential target for rehabilitation strategies.

Summary: This multicenter RCT investigated the effect of three different service-delivery modes on patients' adherence to prophylactic swallowing exercises during radiotherapy. The study found that adherence decreased over time in all groups, but the therapist-supported group achieved the highest adherence rates. Increasing face-to-face contact with a speech-language therapist can overcome the problem of low adherence in this population.
