Sinus rhythm restoration by catheter ablation in patients with long-lasting atrial fibrillation and congestive heart failure: impact of the left ventricular ejection fraction improvement on the implantable cardioverter defibrillator insertion indication

Aims In the setting of congestive heart failure (CHF), atrial fibrillation (AF) ablation can improve clinical status and the left ventricular ejection fraction (LVEF) value. However, the impact of AF ablation on the implantable cardioverter defibrillator (ICD) indication has never been specifically addressed. Methods and results Study subject were six CHF (mean age 61.1 +/- 6.9 years, mean LVEF 25.8 +/- 7.3%) patients refractory to conventional medical treatment with long-lasting AF unresponsive to external cardioversion. Five patients had an idiopathic dilated cardiomyopathy (DCM) and one had an ischaemic cardiomyopathy (ICM). Their New York Heart Association (NYHA) class was III-IV. Two patients had renal insufficiency. No patient had left ventricular delay. All patients underwent AF ablation. LVEF and NYHA class were dramatically improved in the five DCM patients. New York Heart Association class, but not the LVEF, was improved in the ICM patient. A redo ablative procedure was undertaken in four of five DCM patients and in the ICM patient due to arrhythmia recurrence. Left ventricular ejection fraction and NYHA were improved again in the DCM patients (56 +/- 4.4%, I-II, respectively) and led to ICD indication preclusion. The LVEF remained low in the ICM patient (30%) and led to ICD insertion. Sinus rhythm has been stable during the 18.1 +/- 5.7 months follow-up period. Conclusion Atrial fibrillation ablation in CHF patients can improve both the clinical status of patients and their LVEF, especially among those affected by DCM. The LVEF improvement has the potential to preclude the indication for a primary prevention ICD insertion.

Mexiletine for recurrent ventricular tachycardia in adult patients with structural heart disease and implantable cardioverter defibrillator: an EHRA systematic review

Summary: This study systematically reviewed the effectiveness and safety of oral mexiletine in adult patients with structural heart disease and implantable cardioverter defibrillators for recurrent ventricular arrhythmia. The results showed that mexiletine reduced the number of ICD therapies, especially in patients where catheter ablation was not successful or contraindicated. However, in patients with ischemic cardiomyopathy, mexiletine seemed to be less effective than catheter ablation. The main side effects of mexiletine were related to gastrointestinal and neurological systems.

Long-term outcomes of left atrial appendage isolation using cryoballoon in persistent atrial fibrillation

Summary: The long-term safety and efficacy of left atrial appendage isolation (LAAi) in addition to pulmonary vein isolation (PVI) using cryoballoon (CB) in persistent atrial fibrillation (AF) were investigated in this study. The results showed that the LAAi + PVI strategy using CB had acceptable long-term outcomes in patients with persistent AF. However, systemic thrombo-embolic events were an important concern throughout the follow-up, especially in cases of non-adherence to anticoagulants.

Major gaps in the information provided to patients before implantation of cardioverter defibrillators: a prospective patient European evaluation

Summary: This study aims to evaluate the information transmission to patients prior to ICD implantation and its impact on their quality of life. The findings show that although 71.5% of patients felt optimally informed at the time of implantation, many were not provided with information about ICD-related complications, driving restrictions, and end-of-life ICD deactivation. Women were less involved in the decision-making process and reported to be less often optimally informed before ICD implantation compared to men.

Rhythm monitoring, success definition, recurrence, and anticoagulation after atrial fibrillation ablation: results from an EHRA survey

Summary: This study conducted a survey to show how clinicians currently approach the knowledge gap in dealing with recurrences after a failed ablation for atrial fibrillation (AF). The majority of respondents perform routine monitoring for AF recurrences after ablation and prefer a long-term monitoring strategy. The combination of absence of symptoms and recorded AF is considered as the definition of success by most respondents.

Summary: This study compared the procedural efficacy and safety, as well as the 12-month AF recurrence, between the PolarX and AFA-Pro cryoballoon systems. It was found that the PolarX cryoballoon reaches lower temperatures than the AFA-Pro, but it is associated with a higher rate of transient phrenic nerve palsy. No significant differences were observed in terms of 1-year freedom from AF recurrence between the two systems.

Regional disparity on patient characteristics and perceptions after implantable cardioverter-defibrillator implantation: results from an EHRA patient survey

Summary: The aim of this study was to identify potential regional disparities in characteristics of ICD recipients, patient perceptions and perspectives after implantation and level of information provided to patients. The study found that patients in Western Europe reported higher satisfaction after ICD placement, while patients in Central/Eastern Europe felt more optimally informed at the time of device implantation. Therefore, novel strategies are needed to address regional differences in patients' quality of life and provision of information.

Impact of peri-procedural management of direct oral anticoagulants on pocket haematoma after cardiac electronic device implantation: the StimAOD multicentre prospective study

Summary: The aim of this study is to investigate the impact of direct oral anticoagulant (DOAC) management on the incidence of pocket haematoma in patients undergoing pacemaker or implantable cardioverter-defibrillator implantation. The results of a large multicenter prospective observational study showed that the incidence of clinically relevant haematoma was low in patients with DOAC interruption, indicating that DOAC management can effectively reduce the risk of pocket haematoma.
