Lead positioning for cardiac resynchronization therapy: techniques and priorities

Although cardiac resynchronization therapy (CRT) has demonstrated to be an effective treatment for heart failure patients, up to 30-40% of the patients do not show a favourable response. Implantation of the left ventricular (LV) pacing lead is one of the determinants of CRT response. This procedure includes several challenging technical issues and strongly depends on the highly variable anatomy of the coronary sinus and tributaries. In addition, the final position of the LV pacing lead may target the latest activated areas of the left ventricle in order to obtain effective resynchronization. Furthermore, the presence of transmural myocardial scar at the region targeted by the LV lead may also determine the response to CRT. This review discusses all the issues related to LV lead implantation and the role of multimodality imaging to anticipate the implantation strategy. Finally, alternative LV pacing sites and their effect on clinical outcome and LV performance will be discussed.

Summary: The aim of this study was to determine the prevalence and prognostic implications of significant mitral regurgitation (MR) in patients with bicuspid aortic valve (BAV). The study found that significant MR is uncommon in patients with BAV, and it is only associated with adverse prognosis in patients with moderate to severe aortic regurgitation.

Summary: This study examines the relationship between presentation, care, and outcomes of non-ST-segment elevation myocardial infarction (NSTEMI) and country income classification. The findings suggest that NSTEMI patients in low-income countries are younger, more likely to have diabetes and be current smokers, but have fewer comorbidities. Although the rate of invasive coronary angiography is higher in low-income countries, the overall quality of care is not as high. Low-income countries have higher rates of acute heart failure, stroke/transient ischaemic attack, in-hospital mortality, and 30-day mortality compared to high-income countries.

Summary: Right ventricular (RV) size and function assessed by multimodality imaging are associated with outcomes in various cardiovascular diseases. Understanding RV anatomy and physiology is crucial for comprehending the strengths and weaknesses of current imaging methods and providing context for these measurements. The adaptation of the RV to different types and severities of stress, especially over time, is specific to the cardiovascular disease process. Multimodality imaging parameters, which determine outcomes, reflect the ability to image the initial and longitudinal RV response to stress. This paper reviews the standard and novel imaging methods for assessing RV function and discusses the impact of these parameters on outcomes in specific disease states.

Summary: This study aimed to explore the ability of right ventricular global longitudinal strain (RV GLS) to predict in-hospital mortality in patients with first acute myocardial infarction (AMI). The study found that RV GLS was the only RV performance index that differed significantly between different patients, and it outperformed conventional RV function indexes in predicting in-hospital mortality.

Summary: The management of patients with severe aortic stenosis (AS) may differ according to patient age. This analysis aimed to describe patterns of aortic valve replacement (AVR) use in European countries stratified by age. The study found considerable variation in the rates of AVR use and the adoption of transcatheter aortic valve implantation (TAVI) versus surgical aortic valve replacement (sAVR) between countries, with TAVI use increasing in recent years, particularly for older patients.
