Purpose: In patients with multiple sclerosis (MS), epileptic seizures occur more frequently than in the general population. The aim of this study was to analyze clinical characteristics of epilepsy in patients with MS, potential correlation between the semiology of seizures, EEG and magnetic resonance imaging (MRI) findings in these patients, as well as to examine the response to anticonvulsant therapy. Methods: In a series of 268 consecutive patients with definite MS hospitalized at the Institute of Neurology, Belgrade, we identified 20 (7.5%) patients with seizures or epilepsy. All patients with seizures or epilepsy were submitted to standard EEG and brain MRI with gadopentetate dimeglumine. Results: In four patients, epilepsy occurred 1-5 years before other clinical manifestations of MS. Eight patients had seizures only during MS relapses (provoked seizures). In two of them, seizures were the only manifestations of relapse. In 12 patients, seizures occurred regardless of the phase of MS (chronic epilepsy). In the majority of patients, seizures were partial with secondary generalization. Five patients experienced episodes of status epilepticus, and they all had dementia. Abnormal EEG pattern was found in 11 patients. Brain MRI disclosed cortical-subcortical lesions in nine patients and focal cortical atrophy in one, whereas in the remaining patients, findings were inconclusive. Probable EEG-MRI-seizure type correlation existed in 10 patients. Conclusions. Our data suggest that epilepsy may represent an initial symptom of MS and a single clinical manifestation of a relapse, and further support the assumption of the existing correlation between the presence of cortical-subcortical lesions and epileptic seizures or epilepsy in patients with MS.

Clinical predictivity of thalamic sub-regional connectivity in clinically isolated syndrome: a 7-year study

The prevalence of comorbidities in patients with multiple sclerosis: population-based registry data

Summary: The study found that the most common comorbidities in MS patients in Belgrade, Serbia were psychiatric and cardiovascular diseases, with depression and hypertension being the most prevalent single comorbidities. Type 2 diabetes was significantly more prevalent in patients with primary progressive MS compared to other MS phenotypes. The study also identified a positive correlation between number of comorbidities and progression index, as well as a higher risk of developing comorbidities in patients treated with disease-modifying therapies.

Impact of comorbidities on the disability progression in multiple sclerosis

Summary: The study found that hypertension significantly accelerated reaching irreversible disability levels (EDSS 4.0, 6.0, and 7.0), while the presence of any cardiovascular disease comorbidities and type 2 diabetes significantly accelerated reaching EDSS 4.0 and EDSS 6.0. In a multivariable model, higher progression index (PI) was identified as an independent predictor of cardiovascular disease occurrence in multiple sclerosis (MS) patients. In the case of type 2 diabetes, PI and MS phenotype were statistically significant in multivariable Cox regression analysis.

Application of deep-learning to the seronegative side of the NMO spectrum

Summary: This study applied a deep-learning algorithm to analyze brain MRIs of patients with NMOSD and NMOSD-like manifestations, finding that aquaporin-4-seronegative patients shared similar structural features with aquaporin-4-seropositive NMOSD patients. The algorithm successfully differentiated aquaporin-4-seropositive NMOSD and MS patients with a high accuracy, and suggested that aquaporin-4-seronegative patients should be included in the NMOSD category. Anti-MOG patients were found to have intermediate brain features between NMOSD and MS.

Sudomotor dysfunction in people with neuromyelitis optica spectrum disorders

Summary: This study aimed to determine the extent of sudomotor dysfunction in patients with neuromyelitis optica spectrum disorder (pwNMOSD) and compare it with patients with relapsing-remitting multiple sclerosis (pwRRMS). The results showed that sudomotor dysfunction is common in pwNMOSD, and symptoms are more frequent compared to pwRRMS. Age and diagnosis of neuromyelitis optica spectrum disorder were significant predictors for symptomatic sudomotor dysfunction.

Summary: In patients with NMOSD, there is an increased accumulation of brain water as indicated by the increased T2 relaxation time. This alteration could potentially be a useful marker for identifying patients with active disease.

Summary: This study assessed the reliability and validity of the Serbian version of the Brief International Cognitive Assessment for Multiple Sclerosis (BICAMS). The results showed a high prevalence of cognitive impairment in multiple sclerosis patients, with significant correlations between BICAMS scores and age, education, disease severity, and duration.
