Epidemiology and outcome of bacteraemia in neutropenic patients in a single institution from 1991-2012

This study was part of a bloodstream infection surveillance programme that prospectively collected data on consecutive patients with bacteraemia in our institution from 1991 to 2012. We included 2092 bacteraemias in neutropenic patients. Shock and mortality accounted for 299 and 349 cases, respectively (14% and 17%). The main microorganisms isolated were coagulase-negative staphylococci (CoNS, 634, 30%), Escherichia coli (468, 22%) and Pseudomonas aeruginosa (235, 11%). During 2006-2012, there were 155 (27%) E. coli isolates; of these, 73% were fluoroquinolone resistant and 26% cefotaxime resistant. The independent risk factors for mortality were shock on presentation, rapidly fatal prognosis of underlying disease, corticosteroid use, and polymicrobial bacteraemia. Factors associated with lower mortality were the isolation of CoNS [odds ratio (OR) 0.38, 95% confidence interval (CI) 0.20-0.73, P= 0.004] and empirical therapy with amikacin (OR 0.50, 95% CI 0.29-0.88, P= 0.016). The progressive increase of Gram-negative microorganisms resistant to antibiotics influences the choice of empirical treatment in febrile neutropenia and in our experience, the addition of amikacin could be beneficial for such patients.

Summary: This study evaluated the diagnostic accuracy of the newly proposed definition of periprosthetic joint infection (PJI) by the European Bone and Joint Infection Society (EBJIS), comparing it with previous definitions and assessing its accuracy in preoperative diagnosis. The results showed that the EBJIS definition was the most sensitive and accurate in ruling out infection preoperatively.

The Clinical Outcome of Early Periprosthetic Joint Infections Caused by Staphylococcus epidermidis and Managed by Surgical Debridement in an Era of Increasing Resistance

Summary: This study aimed to assess the antibiotic resistance patterns of Staphylococcus epidermidis in early periprosthetic joint infections (PJI) and correlate with clinical outcomes. Results showed that methicillin and ciprofloxacin resistance were associated with higher clinical failure rates. Reduced susceptibility to vancomycin was also related to a higher failure rate. Resistance to fluoroquinolones was identified as an independent risk factor for clinical failure. Alternative prophylactic antibiotics and optimized treatment strategies are needed to improve outcomes.

Invasive candidiasis: current clinical challenges and unmet needs in adult populations

Summary: Invasive candidiasis (IC) is a serious infection caused by Candida species, and it is the most common fungal disease in hospitals in high-income countries. Despite improvements in healthcare and treatment, mortality rates in IC have not significantly improved. This review aims to summarize the main challenges in managing adult patients with IC and highlight specific forms of the infection.

Real-life epidemiology and current outcomes of hospitalized adults with invasive fungal infections

Summary: We conducted a retrospective observational study to describe the epidemiology of invasive fungal infections (IFIs) and outcomes in hospitalized patients. A total of 367 IFIs were diagnosed, with candidemia and invasive aspergillosis being the most common types. Corticosteroid use and prior viral infection were identified as the main risk factors. The overall mortality rate at 12 weeks was 32.2%.

Pseudomonas aeruginosa Bloodstream Infection, Resistance, and Mortality: Do Solid Organ Transplant Recipients Do Better or Worse?

Summary: This study aimed to investigate the antimicrobial resistance of Pseudomonas aeruginosa (P. aeruginosa) in solid organ transplant (SOT) recipients. The results showed that hematologic malignancy and previous carbapenem therapy were associated with an increase in carbapenem-resistant P. aeruginosa bloodstream infections (BSIs). Age, urinary catheter, severity of infection, high-risk source, and bacteremia caused by carbapenem-resistant strains were associated with increased mortality. Correct empirical therapy was found to be a protective factor. The resistance of P. aeruginosa to carbapenems showed an increasing trend, with over 30% of isolates being resistant in the last period. SOT was not associated with a higher risk of carbapenem-resistant BSIs or higher mortality.

Despite the presence of multidrug-resistant bacteria, ceftazidime-avibactam achieved a treatment success rate of 77.3%. The findings of this study provide real-world evidence for the use of ceftazidime-avibactam.

Summary: We aim to report our early experience in using cefiderocol as a rescue therapy for patients infected with difficult-to-treat pathogens. Our case study suggests that cefiderocol is an effective and safe treatment option for patients with extensively drug-resistant bacteria, although the risk of resistance emergence exists.

Summary: In this article, the principles and limitations of local antibiotic delivery are reviewed, with a focus on the potential association between antibiotic-loaded bone cement (ALBC) and antimicrobial resistance. The efficacy of ALBC for prosthetic joint infection (PJI) prophylaxis and treatment is discussed, and concerns regarding the emergence of resistance are addressed. The current evidence does not support an association between the use of ALBC and increased antimicrobial resistance.
