Summary: An 82-year-old man underwent outpatient nuclear medicine gastric-emptying scintigraphy for dysphagia and regurgitation. The scintigraphy showed delayed gastric emptying but subsequent CT scan and upper gastrointestinal fluoroscopy revealed normal stomach size and function, with an enormously dilated esophagus indicating achalasia. It is important to pay attention to the location and shape of the visualized stomach and recognize that a significantly dilated esophagus can mimic an elongated stomach during scintigraphy.

Summary: The study found that robotic pancreaticouodenectomy (RPD) had a lower incidence of delayed gastric emptying (DGE) compared to open pancreaticouodenectomy (OPD). Nausea/vomiting, jaundice, malignancy, and lymph node involvement were identified as significant risk factors for DGE. Intraoperative blood loss and postoperative complications were not significantly associated with DGE, but the hospital stay was longer in patients with DGE.

Summary: The study showed that patients with type 2 diabetes undergoing Roux-en-Y gastric bypass (RYGB) surgery experienced significant reductions in body weight, fat mass, fasting glucose and insulin levels, and circulating lipids 2 weeks post-operation. Oral feeding enhanced insulin and incretin secretion after RYGB, while G-tube feeding completely abolished this enhancement, suggesting that foregut exclusion plays a role in improving glucose metabolism after RYGB, but may not be the sole explanation.

Summary: This study evaluated the relationship between plasma glucose, GLP-1, and insulin secretion and gastric emptying (GE) in patients with type 2 diabetes (T2D). The results showed a positive correlation between plasma glucose levels at 30, 60, and 120 minutes and GE, but not at 180 minutes. Multiple regression analysis revealed a weak correlation between plasma glucose at 120 minutes and GE. However, there was no relationship between plasma GLP-1 and GE, and GE did not significantly affect insulin secretion. Further evaluation of this relationship is necessary.

Summary: This study aimed to investigate the effect of co-ingestion of dietary protein with, or before, carbohydrate on postprandial hyperglycemia in older men with type 2 diabetes (T2D). The results showed that simultaneous intake of glucose and whey protein significantly reduced blood glucose concentrations and had a synergistic effect on insulin concentrations in both men with and without T2D. Additionally, gastric emptying was faster in men with T2D. Therefore, whey protein supplementation may be an effective strategy for the prevention and management of T2D in older individuals.
