Summary: Pancreatic neuroendocrine neoplasms (PanNENs) have the highest rate of increase in incidence. Some cases are caused by inherited tumor syndromes, while sporadic PanNENs are associated with mutations in multiple genes. Furthermore, pancreatic neuroendocrine carcinomas share genetic alterations with ductal adenocarcinomas, and are also influenced by microenvironment and microRNA.

Summary: There is a growing interest in the role of sex and gender in cancer patients, but the impact of sex differences in oncological systemic therapies, especially in uncommon neoplasms like neuroendocrine tumors (NET), is still unknown. In this study, the authors analyzed the toxicities reported in five clinical trials with multikinase inhibitors (MKI) in gastroenteropancreatic (GEP) NET, and found differential toxicities by sex.

Summary: Neuroendocrine tumours (NETs) are rare tumours arising in the digestive system, mainly the small bowel or pancreas. This study aimed to describe the presence of sarcopenia at the time of diagnosis of stage IV NET in the digestive system and correlate it with survival. Sarcopenia was present in 69% of patients, and its presence in pancreatic NETs was associated with poorer survival.

The authors discuss 10 troublesome questions in everyday clinical practice based on their extensive experience. Multidisciplinary decision-making is still necessary for the treatment of NF-Pan-NEN.

This study aimed to describe the spectrum of NENs in a tertiary center, reclassify G3 tumors into G3 NET and NEC, and compare their survival rates. The study found that G3 NETs had higher survival rates than NECs.

Summary: For patients with advanced gastroenteropancreatic neuroendocrine carcinoma (GEP-NEC) treated with cisplatin/etoposide chemotherapy, a reduction of 45% in tumor size may be a more reliable predictor for treatment response compared to the RECIST 1.1 criteria. This threshold can help differentiate responders with improved progression-free survival from nonresponders.

Summary: CALM-NET was a phase IV exploratory study conducted in the UK to evaluate if the presence of circulating tumour cells (CTCs) at baseline could predict symptomatic response in patients with midgut neuroendocrine tumours (NETs) treated with lanreotide autogel (LAN). The study found that patients without baseline CTCs may be more likely to achieve a symptomatic response following LAN treatment, and there was a correlation between plasma 5-HIAA and urinary 5-HIAA during LAN treatment.

Summary: Cross-sectional studies have shown that a significant number of people with GEP NET are malnourished. This study aimed to examine the impact of GEP NET on nutritional status and quality of life over a six-month period. The results showed that a considerable number of patients experienced weight and fat-free mass loss, indicating altered body composition. Additionally, a significant portion of patients reported dietary changes, but only a small percentage sought advice from a dietitian. Valid screening measures are needed to identify and address weight loss and nutrition issues in GEP NET patients.
