Summary: The aim of this study was to identify lymphatic vessels (LVs) in the mucosa of in-situ colorectal cancer (CRC) and detect tumor burden in regional lymph nodes (LNs). LVs were found in the mucosa of all tumors (39/39; 100%) using D240 immunostains. LNs were analyzed by both H&E and a RT-PCR-based molecular method. Low amounts of tumor burden were detected in 11/39 (28%) cases using the molecular assay. The quantification of tumor burden in LNs can help with clinical management.

Summary: The prognostic potential of repeated faecal haemoglobin (F-Hb) concentration measurements in faecal immunochemical test (FIT)-based screening for colorectal cancer (CRC) was examined. The study found that F-Hb concentrations in rounds 1 and 2 were the strongest predictors. In external validation, the model retained similar discrimination accuracy. This suggests that individuals can be accurately identified as low or high risk based on age, sex, and prior F-Hb concentrations, and risk stratification should be considered based on this information.

Summary: This study aimed to explore the optimal positive lymph node cutoff value for predicting survival outcomes in patients with iCCA and establish a prognostic nomogram. The results showed that patients with 1-3 positive lymph nodes had significantly better overall survival compared to those with more than 4 positive lymph nodes. This study provides new evidence for more precise post-operative management of iCCA patients.

A diagnostic algorithm combining FNAC, FNA-Tg, and ultrasound features was proposed for diagnosing metastatic lymph nodes of differentiated thyroid carcinomas.

Summary: This study evaluated the incidence and risk factors of occult metastasis to superficial level VI in patients with papillary thyroid carcinoma. The results showed that lymph nodes were found in approximately 48% of the harvested specimens of superficial level VI. However, the occurrence rate of occult metastasis to superficial level VI was relatively low, suggesting that prophylactic dissection of superficial level VI may not be necessary for primary papillary thyroid carcinoma.

Summary: This qualitative study investigated the decision-making process of surgeons regarding protective ileostomy creation after rectal cancer surgery. The results showed that 76% of surgeons supported the creation of ileostomy. When making the decision, 88% of surgeons considered risk factors. Emotions influenced the decision-making process in 22% of cases, and personal experiences influenced it in 49% of cases. The most frequently considered risk factors were the distance between the anus and the tumor (96%), neoadjuvant chemoradiotherapy (88%), positive intraoperative leak test (65%), blood loss (37%), and immunosuppression therapy (35%).

Summary: This retrospective multicentre cohort study aimed to identify risk factors for a permanent stoma (PS) in patients with left-sided obstructive colon cancer (LSOCC) and develop pre- and postoperative prediction models. The results showed that 37% of LSOCC patients had a PS, and 53% of those with a stoma directly after resection had a PS due to non-reversal. Both baseline characteristics and treatment strategies determine the risk of a PS in LSOCC patients. The developed predictive models will help physicians understand the role of individual variables in the risk of a PS and inform patients about the probability of a PS.

Summary: This systematic review and meta-analyses demonstrate a strong association between ctDNA and recurrent disease in non-metastatic rectal cancer. ctDNA analysis can stratify patients into high and low risk groups for recurrence, especially when detected after neoadjuvant treatment and surgery. Future research should focus on the feasibility of ctDNA-guided treatment and follow-up strategies in rectal cancer.

Summary: In a randomized phase II trial, the study compared the patient-reported outcomes (PROs) of patients with colorectal peritoneal metastases (CPMs) who received perioperative systemic therapy or cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (CRS-HIPEC) alone. The results showed that there were comparable PROs between the two groups at 3 and 6 months postoperatively, indicating acceptable tolerance of perioperative systemic therapy in this setting.

Summary: The Italian Research Group for Gastric Cancer developed a prospective database to evaluate the impact of a pragmatic attitude on the management of stage IV gastric cancer patients. The study found that different metastatic sites did not affect survival rates, but multiple metastatic sites were associated with worse survival. Patients who could undergo curative resection had better survival rates. A more accurate diagnostic workup and staging had a favorable impact on survival.

Summary: The aim of this study was to evaluate the functional outcomes of Florence intracorporeal neobladder (FloRIN) configuration technique performed with a stentless procedure. The results showed that the stentless procedure was associated with shorter console time and lower estimated blood loss compared to the stent group. There were no significant differences in terms of perioperative features and mid-term functional outcomes between the two groups.

Summary: This study evaluated the outcome of tonsillar cancer managed with neoadjuvant chemotherapy followed by surgery. The results showed that neoadjuvant chemotherapy reduced tumor volume and pathological adverse features, significantly decreasing the need for adjuvant therapy. A greater reduction in tumor volume predicted a complete pathologic response. There was no significant difference in survival rates between the groups.
