Sentinel lymph node biopsy without axillary lymph node dissection may not be safe for patients with cytology-proven axillary metastasis.

Summary: This study evaluated the negative predictive value (NPV) of F-18-FDG dedicated lymph node positron emission tomography (LymphPET) in breast cancer patients. The results showed that LymphPET has a high NPV (>90%) for detecting macrometastasis of lymph nodes. Therefore, LymphPET can be used to screen patients and potentially avoid unnecessary sentinel lymph node biopsy (SLNB).

Summary: This study evaluated the rate of non-sentinel lymph node (NSLN) positivity and factors influencing this in patients with a positive sentinel lymph node (SLN) following neoadjuvant chemotherapy (NAC). The results showed a high rate of nodal positivity on completion axillary lymph node dissection (cALND) in the setting of positive SLN after NAC, supporting the current standard of routine cALND. In patients with cN+ disease, NSLN positivity varied based on tumor biology, multicentricity/multifocality, number of positive SLNs, and SLN metastasis size.

Summary: The purpose of this study was to determine the correlation between the biological subtype of breast cancer and the risk of metastasis to a sentinel lymph node. In the analyzed group of 1018 women with breast cancer, 57% were classified as Luminal A subtype. A positive sentinel lymph node was found in 26.5% of the women.

Summary: This study compared the detection rate of indocyanine green (ICG)-fluorescent imaging and Tc-99m-nanocoilloid for sentinel lymph node (SLN)-mapping in breast cancer patients. The results showed that ICG-fluorescence had a higher detection rate than Tc-99m-nanocoilloid, and both methods had equal detection rates for pathological lymph nodes. The study suggests that ICG-fluorescence may be a safe and effective alternative for SLNB in early-stage breast cancer patients.

Summary: Sentinel lymph node biopsy (SLNB) appears to be a safe and effective technique for breast cancer during pregnancy, according to this systematic review. A total of 63 articles were included, with 47 strongly in favor of SLNB in pregnancy-related breast cancer (PABC), 4 partially in favor, 10 strongly against, and 2 partially against. Sub-categorization based on study type showed that the majority of studies in favor had higher levels of evidence. In total, 382 women with PABC underwent SLNB, and full data were reported for 237 cases. The overall live birth rate was 95.8%, while the overall neonatal complication rate was 3.4%. No cases of maternal side effects or anaphylactic reaction, maternal death, stillbirth, or neonatal death were reported (0%).

Summary: In recent years, there has been a trend towards less aggressive surgical management of axillary lymph nodes in breast cancer. This study evaluated the effectiveness of evaluating axillary sentinel lymph nodes and found that serial sectioning at 2-mm intervals reduced false negatives due to macrometastatic disease. Discordant cases were mostly false negatives detected on the first permanent section level due to sampling errors.

Summary: The Italian Research Group for Gastric Cancer developed a prospective database to evaluate the impact of a pragmatic attitude on the management of stage IV gastric cancer patients. The study found that different metastatic sites did not affect survival rates, but multiple metastatic sites were associated with worse survival. Patients who could undergo curative resection had better survival rates. A more accurate diagnostic workup and staging had a favorable impact on survival.

Summary: The aim of this study was to evaluate the functional outcomes of Florence intracorporeal neobladder (FloRIN) configuration technique performed with a stentless procedure. The results showed that the stentless procedure was associated with shorter console time and lower estimated blood loss compared to the stent group. There were no significant differences in terms of perioperative features and mid-term functional outcomes between the two groups.

Summary: This study evaluated the outcome of tonsillar cancer managed with neoadjuvant chemotherapy followed by surgery. The results showed that neoadjuvant chemotherapy reduced tumor volume and pathological adverse features, significantly decreasing the need for adjuvant therapy. A greater reduction in tumor volume predicted a complete pathologic response. There was no significant difference in survival rates between the groups.
