Vascularized fibula flap reconstruction of the mandible in bisphosphonate-related osteonecrosis

Aims: To point out the feasibility of microsurgical reconstruction of the mandible in patients with bisphopsphonate-related osteonecrosis (BRONJ). Methods: Seven patients with extensive mandibular osteonecrosis underwent subtotal mandibulectomy and immediate reconstruction with a free fibula flap. They were six women and one man aged 49-72 years. The mean size of the bone and oral mucosa defects were 18.5 and 22.5 cm(2) respectively. Results: The mean time of surgical intervention was 12 h. All flaps survived and the postoperative course was uneventful. Oral feeding was resumed 14 days after surgery in all cases. The donor legs healed without complications. The pathology report confirmed the diagnosis of BRONJ in all patients. Normal bone was detected at the resection margins in six out of seven patients. Patients were followed-up at intervals of 3 months. After a median follow-up time of 23 months, no clinical and radiographic evidence of recurrent BRONJ were detected in six patients. One patient with osteomyelitis at the resection margins had signs of recurrent BRONJ 6 months after surgery. The overall curative rate of the population was 86%. Conclusions: Despite the limited number of patients studied so far, our data show that mandible reconstruction with the fibula flap is feasible and does not influence the natural course of the primary disease in BRONJ-resected patients. Three-dimensional CAD/CAM reconstruction of the iliac bone following DCIA composite flap harvest

Summary: This article presents a new technique for restoring iliac bone integrity using a customized titanium device designed by CAD/CAM in patients undergoing DCIA flap harvest. Post-surgery follow-up showed minimal complications and satisfactory long-term outcomes at the donor site, with all patients expressing satisfaction with the aesthetic result. The study recommends the use of this device in patients at high risk of herniation. Further research is needed to confirm the long-term stability of the device.

Safety of boneless reconstruction of the mandible with a CAD/CAM designed titanium device: The replica cohort study

Summary: In this study, the safety and efficacy of REPLICA, a CAD/CAM-designed patient-specific titanium mandible, were evaluated in patients with mandibular defects not suitable for traditional reconstruction techniques. The primary outcome of absence of complications and improvement in oral function was achieved by the majority of patients, and quality of life data at 6 months showed positive results. Further research is needed to explore the full potential of REPLICA in different patient populations.

Summary: Ameloblastoma is a rare odontogenic tumour with aggressive local behavior, often associated with BRAF V600 mutations. This study found that BRAF V600 mutated ameloblastomas are more common in younger patients, located in the mandible, and presented with unicystic variant. Local recurrence occurs in 30% of patients with ameloblastoma, and further studies are needed to understand the outcomes of this rare disease.

Summary: The study found that compared to aggressive surgery alone, combined conservative surgery showed a statistically significant difference in the 6-month improvement rate. However, there was no significant difference in the 6-month total resolution rate between the two groups. Additionally, combining conservative surgery with various adjuvant non-invasive procedures was found to achieve partial or complete healing at all stages, with improved results and amelioration of many variables.

Neoadjuvant chemotherapy followed by surgery for HPV-associated tonsillar cancer: Does imaging reflect the pathological response?

Summary: This study evaluated the outcome of tonsillar cancer managed with neoadjuvant chemotherapy followed by surgery. A greater reduction in tumor volume predicted a complete pathologic response. There was no significant difference in survival rates between the groups.
