Summary: This study investigates bone remodelling responses to mandibulectomy by developing an algorithm that incorporates patient-specific longitudinal data. The study aims to simulate bone remodelling activity in the conjunction region with a fibula free flap reconstruction and compare the long-term outcomes of different fixation plate designs. The results show a decrease in overall bone density over time, except in the docking site where bone apposition occurs after a decrease in bone density. Different screw positioning does not significantly influence bone remodelling outcomes.

Summary: This study evaluated the changes in condylar position and morphology after mandibular reconstruction with condyle preservation. The results showed significant changes in condylar position on the ipsilateral side after reconstruction surgery, but no significant changes on the contralateral side. However, there was no statistically significant association between clinical characteristics and changes in condylar position and morphology.

Summary: The study evaluated the long-term facial symmetry after mandible reconstruction using the vascularized fibula free flap in growing patients. The results showed that this technique could effectively maintain facial symmetry without the need for secondary procedures to enhance symmetry in the late postoperative period.

Summary: The study revealed that patients who have undergone fibula free flap harvest experience challenges related to ankle support and balance at the donor site, as well as functional difficulties impacting their quality of life. Multidisciplinary approaches for targeted rehabilitation after fibula free flap harvest should be explored to improve patients' quality of life.

Summary: This study evaluated outcomes of vascularized bone flap reconstruction for oncologic bony extremity defects and compared union rates based on different insetting methods. A retrospective review of patients who received fibula flap reconstruction after oncologic resection was conducted. The overall union rate was 91.7%, with a mean time to full weightbearing of 16 months for lower extremity reconstructions.

Summary: This study compared the application of auxiliary techniques in mandibular reconstruction. The use of adjustable resection/reconstruction aids and CAD/CAM assistance showed no significant differences in terms of feasibility, accuracy, and operative parameters. However, the adjustable aids had advantages of less preoperative planning time and lower costs per case, which may be more favorable for mandibular angle-to-angle reconstruction.

Summary: This study aimed to assess the impact and cost-effectiveness of virtual surgical planning on fibula free flap mandibular reconstruction. The results show that while there was no significant difference in peri- and postoperative data between planned and non-planned surgeries, planned surgeries had a higher rate of centred occlusion. Further studies are needed to evaluate the long-term benefits of virtual surgical planning.

Summary: This study aimed to evaluate the incidence of complications following mandibular reconstruction and analyze contributing factors. Skin perforation was most often detected in the reconstruction plate subgroup, with low BMI and total radiation dosage being the most relevant risk factors. Microvascular bone flaps were found to have fewer skin perforations overall, and BMI and expected radiation dosage should be considered when choosing a reconstructive technique.

Summary: This study evaluated the primary fixation stability of CAD/CAM fixators and compared the performance of different types of fixators under biomechanical conditions. The results showed that CAD/CAM miniplates had higher mechanical strains, while CAD/CAM reconstruction plates resulted in higher strains. However, the stresses in the fixation systems were below the material yield stress, indicating that material failure would not be expected.

Summary: This article presents a new technique for restoring iliac bone integrity using a customized titanium device designed by CAD/CAM in patients undergoing DCIA flap harvest. Post-surgery follow-up showed minimal complications and satisfactory long-term outcomes at the donor site, with all patients expressing satisfaction with the aesthetic result. The study recommends the use of this device in patients at high risk of herniation. Further research is needed to confirm the long-term stability of the device.

Summary: Ameloblastoma is a rare odontogenic tumour with aggressive local behavior, often associated with BRAF V600 mutations. This study found that BRAF V600 mutated ameloblastomas are more common in younger patients, located in the mandible, and presented with unicystic variant. Local recurrence occurs in 30% of patients with ameloblastoma, and further studies are needed to understand the outcomes of this rare disease.

Summary: The study found that compared to aggressive surgery alone, combined conservative surgery showed a statistically significant difference in the 6-month improvement rate. However, there was no significant difference in the 6-month total resolution rate between the two groups. Additionally, combining conservative surgery with various adjuvant non-invasive procedures was found to achieve partial or complete healing at all stages, with improved results and amelioration of many variables.

Summary: Antiresorptive drugs play a crucial role in the treatment of bone metastases in solid cancer patients. However, the choice of drugs and treatment options pose challenges in decision making. The underestimation of medication-related osteonecrosis of the jaw (MRONJ) risk and the need for long-term risk stratification based on actuarial data are important considerations. Surgical therapy also plays a significant role in improving the quality of life and survival of patients.

Summary: The Italian Research Group for Gastric Cancer developed a prospective database to evaluate the impact of a pragmatic attitude on the management of stage IV gastric cancer patients. The study found that different metastatic sites did not affect survival rates, but multiple metastatic sites were associated with worse survival. Patients who could undergo curative resection had better survival rates. A more accurate diagnostic workup and staging had a favorable impact on survival.
