Summary: The study found that there was no significant difference in functional recovery between patients receiving extended early intervention or regular care after 2 years of early psychosis intervention. Longer periods of symptom remission and high school completion were positively associated with functional recovery, but one-third of patients were still unemployed or not in school at year 5. The lack of additional progress even in extended early intervention suggests the need for specific interventions targeting functional roles beyond the initial 2-year period.

Summary: Psychotic symptoms, such as hallucinations, delusions, and disorganized thinking and behaviors, are characteristic of schizophrenia but can occur in other psychiatric and medical conditions. Many children and adolescents experience psychotic-like symptoms, which may be linked to other psychopathology and past experiences. However, most youth reporting such symptoms do not have or develop schizophrenia or another psychotic disorder. Accurate assessment is crucial as different presentations have different diagnostic and treatment implications. This review focuses on the diagnosis and treatment of early onset schizophrenia, as well as the development of community-based first-episode psychosis programs and the importance of early intervention and coordinated care.

Summary: This study used a robust, democratic process to determine the top priorities for research into early intervention in psychosis. The consensus was reached on 10 priority questions, which largely focused on access and engagement, information needs, and the influence of service-user priorities and beliefs on treatment choices and effectiveness. The study found that the priorities of service users, carers and healthcare professionals were aligned with researchers' and funders' priorities in some areas but misaligned in others.

The findings showed that portfolio use did not effectively support self-regulated learning in this context, primarily due to conflicts arising from the dual purpose of portfolios for both learning and assessment.

Summary: Despite the availability of continuing medical education (CME) programmes on evidence-based diabetes care, there is still a lack of evidence-based best practice in actual GP practice. This study examined the uptake of online diabetes CME and its impact on GPs' attitudes and knowledge of Type 2 diabetes management. The findings highlight the need for mandatory CME to bridge the gap between evidence-based practice and actual GP practice.

Summary: There is evidence of disparities in the provision of psychosis care among different ethnicities. This study investigated the variations in the receipt of Cognitive Behavioural Therapy for psychosis and family intervention among ethnic groups in Early Intervention in Psychosis teams in England. The results showed pervasive inequalities in receiving these interventions for almost all minoritized ethnic groups, highlighting the need for further research.

Summary: This meta-analysis found that antipsychotics and medications used to treat ADHD were significantly better than placebo in improving emotional dysregulation and irritability in individuals with ASD. Specifically, risperidone and aripiprazole showed efficacy in short-term treatment. However, other drug classes did not show evidence of efficacy. Lower efficacy was associated with comorbid epilepsy.

Summary: Psychosocial interventions play an important role in schizophrenia and early psychosis. However, the literature on this topic is heterogeneous and contradictory. This umbrella review summarizes the findings from multiple meta-analyses comparing the effectiveness of different psychosocial interventions with standard treatment or other active interventions. The results suggest that early intervention services and cognitive behavioral therapy have significant effects on symptom reduction and functional improvement in early psychosis. For schizophrenia patients, cognitive behavioral therapy, psychoeducation, and any family interventions show positive effects on symptom reduction, relapse prevention, and functional improvement.

Summary: Precision psychiatry is an approach that aims to stratify individuals with psychotic disorders according to different clinical outcomes and tailor treatment to their individual needs. Currently, it is difficult to predict interindividual differences in outcomes of psychotic disorders solely on the basis of clinical assessment. Therefore, current research focuses on building models that integrate clinical information with a range of biological measures to predict outcomes.

Summary: Gender inequality has been linked to higher mental health risks and lower academic achievement for women globally. Differences in brain structure between men and women may be partially explained by unequal exposure to harsher conditions in gender-unequal countries, leading to worse outcomes for women. A meta-analysis of MRI scans from 139 samples across 29 countries revealed that women in gender-equal countries had no differences or even thicker cortical regions, while those in countries with greater gender inequality had thinner cortices. These findings highlight the potential negative impact of gender inequality on women's brains and the need for policies based on neuroscience for gender equality.

The findings suggest that IPS implementation within a CHR clinical team is feasible and may be a beneficial intervention for young people at CHR. Longer follow-up is needed to assess its impact on educational outcomes.
