Summary: This study investigated the efficacy of longer administration of famciclovir in reducing pain and preventing postherpetic neuralgia. Two groups of patients were treated with famciclovir for either one or two weeks, and their pain intensity and occurrence of postherpetic neuralgia were evaluated at different time points. The results showed no significant differences between the one-week and two-week treatment protocols in reducing pain intensity and frequency of postherpetic neuralgia.

Summary: This case report presents two patients with damaged sensory nerves originating from visceral neurons of the lateral horn of the spinal cord. They experienced intractable, severe lower back and abdominal pain without rash or herpes. After excluding organic lesions, they were diagnosed with herpetic visceral neuralgia without rash. Treatment for HZ neuralgia was effective in both patients. The article highlights the importance of recognizing herpes zoster neuralgia without rash to prevent misdiagnosis.

Summary: Pain is a common clinical symptom caused by nerve injury, tissue damage, or diseases. It affects daily physical function and quality of life, resulting in physiological and psychological torture. The underlying molecular mechanisms and signaling pathways of pain are not fully understood, making pain management challenging. Autophagy, an intracellular process, plays a vital role in maintaining neural function and has been linked to neuropathic pain. Traditional Chinese medicine monomers have also been found to involve autophagy in pain relief, suggesting autophagy as a potential target for pain management.

Summary: The molecular mechanisms of refractory pain in postherpetic neuralgia (PHN) patients remain unclear. This study identified 19 differentially expressed miRNAs in PHN skin and confirmed their expression levels in other PHN patients. Among them, miR-16-5p and let-7a-5p were also downregulated in RTX mice, consistent with PHN patients. Furthermore, intraplantar injection of agomir-16-5p alleviated mechanical hyperalgesia and thermal hypoalgesia in RTX mice, possibly by downregulating the expression of its target gene Akt3 in the skin.
