Newer Biological Agents in Rheumatoid Arthritis Impact on Health-Related Quality of Life and Productivity

Health-related quality of life (HR-QOL) in patients with rheumatoid arthritis (RA) is significantly impaired as a result of pain, deficits in physical function and fatigue associated with this disease. Decrements in HR-QOL are also associated with an increased probability of no longer working, absence from work due to RA-associated sickness, and reduced productivity while at work or in the home, all of which have consequences for the patient as well as society. HR-QOL and productivity are thus important components in the assessment of outcomes in RA, and assessment of HR-QOL is now recommended in clinical trials that assess the efficacy of new treatments for RA. Measures to assess HR-QOL include the Medical Outcomes Study Short Form 36 (SF-36), EuroQol (EQ-5D) and the Health Utilities Index - Mark 3 (HUI3); these measures not only provide an indication of the clinical (i.e. statistical) efficacy of a treatment, but also provide information on whether this efficacy is truly 'meaningful' from a patient's perspective. These measures have been utilized in clinical trials of biological agents in patients with RA, including tumour necrosis factor inhibitors (etanercept, infliximab, adalimumab, certolizumab pegol and golimumab), the co-stimulatory inhibitor molecule abatacept, the B-cell depletion agent rituximab and the interleukin-6 receptor antagonist tocilizumab, and have demonstrated that these agents can significantly improve HR-QOL. Assessment of work productivity in patients with RA and the impact of treatment is a practical way to measure disability from RA from individual and societal perspectives. As RA affects women three times more frequently than men, there is also a critical need for productivity assessment within the home as well as participation in family/social/leisure activities. Data from recent trials of biological agents demonstrate that these agents can reverse disease-related decrements in productivity and limitations in participation in family. social and leisure activities in patients with active RA. However, despite this recognition, several challenges in the assessment of productivity remain, including standardization of instruments. Development of additional instruments to assess HR-QOL and productivity that are easier to use in daily practice may further improve our ability to monitor the effectiveness of therapies.

Summary: This study examines the incidence, time trends, and outcomes of alcohol use disorder (AUD) hospitalizations in individuals with musculoskeletal diseases. The results show an increase in AUD hospitalizations in all five musculoskeletal diseases studied. Providers and patients should be aware of the risks and impact of alcohol use and interventions are needed to reduce the healthcare burden associated with AUD hospitalizations in musculoskeletal diseases.

Summary: Racial differences in XO activity and mtDNA DAMPs were observed in Black and White adults with resistant hypertension. Black resistant hypertension patients had higher XO activity, worse diastolic dysfunction, and greater mtDNA DAMPs than White patients. Urinary sodium excretion was correlated with left ventricular function in Black patients. These findings highlight the importance of further investigating the role of XO and mtDNA DAMPs in cardiac remodeling and heart failure in Black adults with resistant hypertension.

Change in serum urate level with urate-lowering therapy initiation associates in the immediate term with patient-reported outcomes in people with gout

Summary: Higher serum urate levels are associated with poorer health-related quality of life, pain, and disability scores in gout patients. Recent fluctuations in serum urate levels are also associated with poorer outcomes, primarily driven by reductions in serum urate. Clinical emphasis should be placed on slow reduction of serum urate levels and the routine use of effective anti-inflammatory medications during urate-lowering therapy initiation or escalation, as this may help prevent short-term poor outcomes.

Summary: This study provides estimates of HIV prevalence at local scales in sub-Saharan Africa, specifically for adults aged 15-59 years. The results show significant variation in HIV prevalence across localities, ages, and sexes, which has not been acknowledged in previous analyses. The findings also demonstrate changing trends in HIV prevalence over time. These estimates are crucial for targeted interventions in the fight against HIV in sub-Saharan Africa.

Rituximab is associated with worse COVID-19 outcomes in patients with rheumatoid arthritis: A retrospective, nationally sampled cohort study from the US National COVID Cohort Collaborative (N3C)

Summary: This study assessed the association between rituximab (RTX) use and COVID-19 outcomes in rheumatoid arthritis (RA) patients. The results showed that compared to conventional synthetic disease-modifying anti-rheumatic drugs (csDMARDs), RTX use was associated with increased odds of COVID-19-related hospitalization, ICU admission, and invasive ventilation. These findings can provide guidance for decision-makers, healthcare providers, and patients during the COVID-19 pandemic.

The Emerging Safety Profile of JAK Inhibitors in Rheumatic Diseases

Summary: Janus kinase inhibitor (JAKi) medications are highly effective oral drugs for treating rheumatic diseases. They have similar safety concerns to biologic therapies but also have distinct differences. Recent data comparing JAKi with tumor necrosis factor inhibitors (TNFi) led to new warnings about cardiovascular and cancer risks by the FDA. This review summarizes the current published data on the safety of JAKi in rheumatic diseases, including risk differences between agents and indications.

Disease modification in gout: a qualitative study of gout expert rheumatologists

Summary: This study aims to explore the views of healthcare providers on disease modification in gout and derive a provisional set of domains for disease modification. The results indicate that reducing flares and lowering serum urate are considered the most important aspects of disease modification in gout. Other domains include improving quality of life, restoring function, reducing pain, reducing tophi burden, and joint preservation or resolution of joint damage.
