The blind assessment of these videos reported a high rate of unsafe or inappropriate technique for rectal mobilization and transection, with fair interobserver agreement among reviewers. Peer review of videos could facilitate training and accreditation in this complex procedure.

Summary: The study assessed factors associated with maintaining a functional anastomosis in patients with anastomotic leakage (AL). After analyzing consecutive patients with AL after colorectal or coloanal anastomosis, it was found that 60.9% of patients had a functional anastomosis without a stoma after one year of surgery. Diabetes, neoadjuvant chemoradiotherapy, and Grade B or C leak significantly increased the risk of anastomotic failure at one year. The type of initial treatment and different types of anastomosis also influenced the risk of anastomotic failure.

Summary: Total abdominal proctocolectomy with ileal pouch-anal anastomosis (IPAA) is a common treatment for ulcerative colitis (UC), but it can lead to complications such as pouchitis and pouch disorders. Pouchitis can usually be treated with antibiotics, but chronic antibiotic refractory pouchitis (CARP) may require biologic therapies. Crohn's-like disease of the pouch (CLDP) is a less common complication that can be treated similarly to CARP. Standardized diagnostic criteria for inflammatory pouch disorders can help guide future therapeutic options. Structural pouch disorders are often related to surgical complications, and their management may involve endoscopic interventions and surgical procedures.

Summary: For the past 40 years, the ileal pouch-anal anastomosis has been used for gastrointestinal reconstruction in patients with certain conditions. However, this procedure comes with complications such as inflammation, functional problems, and other issues that are worsened by factors like diet and medication. As a result, radiologic evaluation is often needed, including pouchography and small bowel imaging. This review discusses the anatomy and physiology of the ileal pouch as a baseline for measuring pouch dysfunction.

Summary: Total proctocolectomy and ileal pouch-anal anastomosis (IPAA) surgery is the standard treatment for severe refractory ulcerative colitis (UC) patients who do not respond to medical treatment. However, this surgery can lead to complications such as anastomotic leaks, abscesses, and rare complications like pouch volvulus. We present a case of a 57-year-old female who developed recurrent pouch volvulus 15 years after undergoing IPAA surgery. After multiple endoscopic decompressions and pouch fixation surgeries, a loop ileostomy was performed. The patient is currently alive and well with a permanent ileostomy.

Summary: This study aims to identify predictive factors associated with anastomotic fistula after colon cancer surgical resection and describe the impact of this complication on mortality and postoperative length of stay. The results showed that diabetes, smoking, hypoalbuminemia, preoperative anemia, location of anastomotic fistula, perioperative transfusion, and longer duration of surgery were predictors of the development of this complication.
