Summary: This retrospective study aimed to identify the pre- and intraoperative risk factors associated with complications in patients undergoing restorative proc-tocolectomy with ileal pouch-anal anastomosis (IPAA) for ulcerative colitis (UC). The results showed that the coexistence of extraintestinal clinical manifestations and pre-operative topical rectal stump therapy were significantly associated with pouchitis. Endoscopic pouchitis was related to preoperative topical rectal therapy, while anastomotic leak was significantly related to pouch failure. Surgical indication for malignancy increased the risk for early complications, and young age was associated with the occurrence of outlet stoma obstruction in patients with recent IPAA surgery.

Summary: Histologic variables of acute and chronic inflammation were found to be associated with future development of chronic pouchitis, need for biologic or small molecule treatment for chronic pouchitis, and hospitalization in patients with ileal pouch-anal anastomosis. The study suggests that histologic findings may have predictive value for clinical outcomes and the utilization of healthcare resources in these patients. Incorporation of key histologic variables into pouchoscopy pathology reports and closer monitoring of patients with significant histologic findings may be necessary.
