Summary: Anastomotic leakage (AL) is a common complication after colorectal surgery and the underlying mechanism is still not well understood. This study aimed to investigate the association between gut microbiota and colorectal AL development. Through analyzing tissue-associated microbiota using 16S rRNA sequencing, lower microbial diversity was observed in the AL group compared to non-leak anastomosis (NLA) group. The facultative anaerobic bacterium Gemella palaticanis showed high abundance and may be a potential marker for AL.

Summary: The dilation status of the bile duct before hepaticojejunostomy for major bile duct injury does not appear to affect short- or long-term outcomes. Patients with undilated bile ducts had more severe injury and a lower incidence of preoperative cholangitis compared to patients with dilated bile ducts. The study suggests that both surgical and radiological external biliary drainages can reduce the risk of cholangitis before repair without increasing the risk of long-term anastomotic stricture.

Summary: Postoperative biliary fistula after pancreatoduodenectomy, including clinically insignificant, transient biliary fistula, is associated with an increased risk of a late biliary anastomotic stricture requiring stenting. The severity of the fistula is correlated with the risk of stricture, with patients experiencing grade A and grade B postoperative biliary fistula having a higher risk of developing stricture.
