Summary: Small intestinal bacterial overgrowth (SIBO) is characterized by an increase in the number and/or alteration in the type of bacteria in the upper gastrointestinal tract, causing various bowel symptoms. SIBO can be diagnosed using microbial culture or breath tests after carbohydrate ingestion. However, these methods have limitations and risks. Urinary excretion tests using bile acid conjugates have shown potential as diagnostic substrates for SIBO. PABA-UDCA disulfate offers a simple and rapid method for evaluating SIBO. Treatments for SIBO include antibiotics, probiotics, therapeutic diets, herbal medicines, and fecal microbiota transplantation.

Summary: The systematic review and meta-analysis indicated that methane-positive SIBO is more prevalent in IBS, particularly in IBS-C, compared to controls, while it is less common in patients with IBD. In patients with IBD, methane-positive SIBO was significantly lower in Crohn's disease compared to ulcerative colitis. More studies are needed to establish a causal relationship due to the low quality of evidence and clinical heterogeneity in the current studies.

Summary: This study found poor agreement between lactulose breath test (LBT) and duodenal aspiration (DA) in the evaluation of small intestinal bacterial overgrowth (SIBO). LBT may be more favorable than DA due to being safer, cheaper, and less likely to yield a contaminated result. Patients with diabetes mellitus or proton pump inhibitor (PPI) use were more likely to have a positive result in DA, while those with a history of small bowel resection were more likely to have a positive result in LBT.

Summary: Small-intestinal duplication is a rare congenital anomaly, with single occurrences being more common than multiple ones. Most malformations are found in the ileocecal region. The main surgical treatment involves complete resection of the malformations and adjacent intestinal ducts. However, preserving the ileocecal junction is challenging, and multiple intestinal repairs increase the risk of postoperative intestinal fistula. Here, we report a case of ileocecal preservation surgery for multiple small intestinal duplications near the ileocecal area, which resulted in successful recovery and follow-up.

Summary: This study aimed to evaluate the ability of Liver Imaging Reporting and Data System (LI-RADS) and radiomic features in predicting progression-free survival (PFS) in patients with nodular hepatocellular carcinoma (HCC) treated with radiofrequency ablation. The results showed that multifocality, the continuity of an enhancing capsule, and a higher radiomic signature were associated with poorer PFS in early-stage HCC.

Summary: This study conducted a targeted literature search to investigate how noninferiority pragmatic trials address issues related to margins, adherence, and analytic population. The results showed that most trials provided justification for the noninferiority margins, monitored treatment adherence to some extent, and conducted various types of analyses. However, some trials excluded a portion of participants from the primary analysis. It is suggested that new guidelines should be developed as the current guidance does not fit noninferiority pragmatic trials.
