Summary: New criteria for difficult cannulation in trainee-involved procedures were proposed based on cannulation time, attempts, or inadvertent pancreatic duct cannulation exceeding the 75% percentile. The proportion of difficult cannulation using these criteria was 35.5% after propensity score matching.

Summary: This study investigated the etiologic spectrum of benign duodenal stenosis (BDS) and its response to endoscopic balloon dilation (EBD). The results showed that EBD was effective for treating BDS regardless of its etiology. Males were more likely to have distal duodenal involvement, and the usage of PPI was associated with clinical success.

Summary: The experimental results indicate that thermal damage in EBD persists until the completion of stenosis, requiring careful operation in the initial stage. The extension effect was only observed at the laceration site and later returned to a status similar to surrounding locations. Additional intervention is needed to prevent restenosis.

Summary: An intelligent difficulty scoring and assistance system (DSAS) based on deep learning was developed to assist endoscopists in scoring the technical difficulty of CBD stone extraction and guiding the choice of therapeutic approach and appropriate accessories during ERCP. The performance of the DSAS was superior to nonexpert endoscopists and more consistent with expert endoscopists. DSAS assessment scores >= 2 were correlated with lower stone clearance rates and more frequent EPLBD.

Summary: This study evaluated the feasibility of a novel short non-slip banded balloon for sphincteroplasty. The results showed that the non-slip balloon had a significantly higher technical success rate and lower slippage rate compared to the conventional balloon, without any immediate adverse events.

Summary: The study revealed that transpancreatic sphincterotomy is effective in increasing the success rate of biliary cannulation, while early needle-knife techniques and transpancreatic sphincterotomy perform well in reducing the incidence of post-ERCP pancreatitis. These interventions should be considered for patients with difficult cannulation.

Summary: The study revealed that a history of invasive endoscopic sphincterotomy (EST) significantly influences the biliary microbiota, leading to a marked shift in the composition of biliary bacteria. The bacterial genus Pyramidobacter was specifically associated with a history of EST.

Summary: A simplified scoring system was developed to predict the difficulty of CBD stone removal in clinical practice, which can assist endoscopists in making treatment decisions. Retrospective cohort data was collected and analyzed using multivariable logistic regression. The scoring system based on risk factors showed good prediction performance and was internally validated.

Summary: This study found that prophylactic cholecystectomy is more effective compared to deferring cholecystectomy for patients post-endoscopic biliary sphincterotomy for common bile duct stones. Further studies may identify specific subsets of patients who may not require cholecystectomy, such as those with large balloon biliary dilation.

Summary: PA-EMR is an effective technique that may be considered as a rescue technique for difficult biliary cannulation in expert hands. The success rate of bile duct cannulation with the PA-EMR technique was very high among the 14 patients, with only one case of mild bleeding reported.

Summary: This study aimed to assess the long-term outcomes of EPLBD (12-15 mm) with or without limited sphincterotomy for removal of common bile duct (CBD) stones. The results showed that EPLBD and ES-LBD had similar outcomes in terms of stone removal and complications, confirming the effectiveness and safety of EPLBD as an alternative method for extracting large CBD stones.

Summary: This study aimed to investigate the necessity of endoscopic sphincterotomy (ES) in the prevention of post-endoscopic retrograde cholangiopancreatography pancreatitis (PEP) after endoscopic biliary stenting for malignant biliary obstruction. The results showed no significant difference in the incidence of PEP between patients with or without ES.

Summary: This study aimed to evaluate the effect of performing endoscopic sphincterotomy (ES) before biliary stent/tube placement on the occurrence of post-endoscopic retrograde cholangiopancreatography pancreatitis (PEP). The results showed that performing ES before biliary stent/tube placement could reduce the occurrence of PEP in patients with biliary stricture.
