Summary: This study aimed to identify the optimal threshold of preoperative serum albumin concentration for risk-stratification of adverse post-operative outcomes in a large surgical population. The findings showed that a serum albumin threshold of 3.4 g/dL was able to predict adverse surgical outcomes effectively. Patients with hypoalbuminemia had increased odds of death within 30 days post-surgery, particularly among those with disseminated cancer.

Summary: The use of hypnotic and sedative medication for sleep improvement is common, but long-term use is associated with increased risk of adverse events and mortality. This study aimed to determine the incidence and factors associated with new persistent use of hypnotic and sedative medication after surgical procedures. The results showed that although a small proportion of surgical patients initiate the use of these medications, a substantial proportion develop persistent use, which is associated with adverse outcomes.

Summary: This study compared the long-term outcomes and clinicopathologic characteristics between patients treated with breast-conserving surgery (BCS) plus whole-breast irradiation (WBI) and mastectomy for early-stage breast cancer (BC). The results showed that the cumulative incidence of axillary lymph node recurrence was lower in the BCS + WBI group, while the incidence of contralateral BC was higher. Among patients with HER2 subtype BC, BCS + WBI was associated with a higher risk of ipsilateral breast tumor recurrence (IBTR), compared with mastectomy. However, there was no statistically significant difference in survival between the groups.

Summary: This study analyzed a large series of neuroendocrine carcinomas (NECs) to validate previously identified prognostic factors and determine if additional parameters could be isolated. The most relevant predictors for prognosis were Ki-67 index, morphology, stage, and site. However, morphology was not a significant factor when Ki-67 was greater than or equal to 55%.

Summary: Assessing the behavior of pancreatic neuroendocrine tumors (PNETs) remains a daily challenge for physicians. The definition of aggressiveness is crucial in determining the appropriate management strategy, ranging from observation to surgery. EUS has emerged as a valuable tool in diagnosing PNETs and has experienced rapid advancements in recent years, particularly in the assessment of aggressiveness using contrast-enhanced techniques. This review highlights the latest experiences and opportunities in evaluating PNET behavior with contact-enhanced EUS and contrast-enhanced harmonic EUS, with a focus on future applications and possibilities.

Summary: The aim of this study was to define the learning curve for robot-assisted thoracoscopic surgery lobectomy by analyzing the experience of a single surgeon. Data on surgical performance and cardiovascular and respiratory outcomes were collected from January 2021 to June 2022. Cumulative sum control charts were used to analyze the learning curve. The results showed that the transition beyond the learning phase occurred at different case numbers for various parameters. The study concluded that robotic lobectomy has a safe and feasible learning curve with adequate training, and confidence and competence can be achieved after approximately 20-30 procedures without compromising surgical efficiency and oncological radicality.

The lack of external validation hinders the progress toward clinical use.

Summary: The relationship between chronic intestinal disease and pancreatic disorders, such as inflammatory bowel disease (IBD), celiac disease (CelD), and pancreatic cancer, remains unclear. Possible factors contributing to this association include drug use, changes in microcirculation, disruption of gut permeability/motility, bacterial translocation, and activation of gut-associated lymphoid tissue. Other systemic conditions, like IgG4-related disease or vasculitides, may also affect both the pancreas and the intestinal tract. This review provides an overview of the current understanding of this complex relationship.

Summary: The aim of this study was to explore the correlation between incidence and mortality of pancreatic cancer and healthcare indicators in 36 European countries. The results showed that both incidence and mortality were increasing, and they were strongly positively correlated. Higher health expenditure, availability of beds and medical technology, as well as the number of practicing doctors were related to higher incidence. On the other hand, lower mortality was associated with indicators such as outpatient curative care expenditure, the number of pet scanners, and radiation therapy equipment. The study highlights the impact of healthcare environment on pancreatic cancer incidence and mortality.

Summary: This review summarizes the role of 24-hour urinary 5-hydroxyindolacetic acid (24u5HIAA) in the diagnosis and follow-up of neuroendocrine tumors-related carcinoid syndrome (NETs-CS). 24u5HIAA is an established biomarker with high sensitivity and specificity for diagnosing NETs-CS. It is also useful in the follow-up of patients, as a shorter double time and elevated levels are associated with an increased risk of disease progression and mortality.
