Chronic pancreatitis: Report from a multicenter Italian survey (PANCROINFAISP) on 893 patients

Background. No data on chronic pancreatitis in Italy are available yet. Aim. To evaluate demographic, clinical, diagnostic and therapeutic aspects in patients suffering from chronic pancreatitis. Patients and methods. Eligible patients were prospectively enrolled from 2000 to 2005. Information concerning demographic data, lifestyle risk factors, family and clinical history, associated factors (alcohol, autoimmunity, cystic dystrophy of the duodenal Wall. Obstruction, genetic mutations) concomitant diseases, diagnostic findings, and pharmacological, endoscopic and surgical therapy were gathered. Results. 893 patients (74% males, mean age 53.7 +/- 15.2 years) were evaluated. 519/859 patients (60%) were drinkers and 555/840 (66%) were smokers. A strong positive correlation between drinking and cigarette consumption (R=0.53; p<0.0001) Was found. Heavy alcohol consumption (>80g of alcohol/day for more than 5 years) was considered the most important risk factor in 300 patients (34%). obstruction in 238 (27%), alcohol and obstruction in 82 (9%), autoimmunity in 34 (4%). cystic dystrophy of the duodenal wall/groove pancreatitis in 55 (6%), gene mutations in 36 (4%), and none (idiopathic) in 148 (17%). Bile stones were diagnosed in 287 patients (33%) and cholecystectomy was performed in 329 patients (38%). Pancreatic calcifications were diagnosed in 547/879 patients (62%). Pancreatic surgery was performed in 273 patients (31%). Endoscopic sphincterotomy was performed in 371 patients (42%). Exocrine and endocrine insufficiency were found, respectively, in 373/834 (45%) and 275,1885 patients (31%). Conclusions. An unexpected low frequency of alcohol abuse and new emerging associated risk factors for chronic pancreatitis were observed in Italy. (C) 2008 Editrice Gastroenterologica Italiana S.r.l. A propensity score-matched analysis of breast-conserving surgery plus whole-breast irradiation versus mastectomy in breast cancer

Summary: This study compared the long-term outcomes and clinicopathologic characteristics between patients treated with breast-conserving surgery (BCS) plus whole-breast irradiation (WBI) and mastectomy for early-stage breast cancer (BC). The results showed that the cumulative incidence of axillary lymph node recurrence was lower in the BCS + WBI group, while the incidence of contralateral BC was higher. Among patients with HER2 subtype BC, BCS + WBI was associated with a higher risk of ipsilateral breast tumor recurrence (IBTR), compared with mastectomy. However, there was no statistically significant difference in survival between the groups.

Summary: This study analyzed a large series of neuroendocrine carcinomas (NECs) to validate previously identified prognostic factors and determine if additional parameters could be isolated. The most relevant predictors for prognosis were Ki-67 index, morphology, stage, and site. However, morphology was not a significant factor when Ki-67 was greater than or equal to 55%.

Prevalence of Germline BRCA1/2 Variants in Ashkenazi and Non-Ashkenazi Prostate Cancer Populations: A Systematic Review and Meta-Analysis

Summary: This systematic review and meta-analysis found that germline BRCA1 variants are more prevalent in Ashkenazi Jewish men with prostate cancer, while BRCA2 variants show a similar distribution between Ashkenazi and non-Ashkenazi populations. This suggests that genetic screening for prostate cancer in Ashkenazi men should not be limited to the BRCA2 gene.

EUS has emerged as a valuable tool in diagnosing PNETs and has experienced rapid advancements in recent years, particularly in the assessment of aggressiveness using contrast-enhanced techniques. This review highlights the latest experiences and opportunities in evaluating PNET behavior with contact-enhanced EUS and contrast-enhanced harmonic EUS, with a focus on future applications and possibilities.

Learning Curve of Robotic Lobectomy for the Treatment of Lung Cancer: How Does It Impact on the Autonomic Nervous System of the Surgeon?

Summary: The aim of this study was to define the learning curve for robot-assisted thoracoscopic surgery lobectomy by analyzing the experience of a single surgeon. Data on surgical performance and cardiovascular and respiratory outcomes were collected from January 2021 to June 2022. Cumulative sum control charts were used to analyze the learning curve. The results showed that the transition beyond the learning phase occurred at different case numbers for various parameters. The study concluded that robotic lobectomy has a safe and feasible learning curve with adequate training, and confidence and competence can be achieved after approximately 20-30 procedures without compromising surgical efficiency and oncological radicality.

Regret affects the choice between neoadjuvant therapy and upfront surgery for potentially resectable pancreatic cancer

Summary: A regret-based decision model was applied to evaluate attitudes toward neoadjuvant therapy versus upfront surgery for potentially resectable pancreatic adenocarcinoma. Results showed that regret of omission and commission influenced clinicians' therapeutic decisions, and surgeons or specialists working in surgical centers were less likely to recommend neoadjuvant therapy.

Clinical Prediction Models for Recurrence in Patients with Resectable Grade 1 and 2 Sporadic Non-Functional Pancreatic Neuroendocrine Tumors: A Systematic Review

Summary: The risk prediction for tumor recurrence after surgery for non-functional pancreatic neuroendocrine tumors is a major unmet clinical need. Multiple prediction models have been developed, but none are currently incorporated into international guidelines. This systematic review found 13 original models, of which 3 were validated outside the patient group in which they were developed. The lack of external validation hinders the progress toward clinical use.

Summary: By comparing the autonomic nervous system and psychological responses of surgeons during robotic and open surgeries, this study aimed to evaluate and quantify the stress experienced by surgeons during their surgical activities. The results showed that the robotic approach caused less stress and ensured greater comfort for surgeons, although it also resulted in higher levels of anxiety.

Summary: The relationship between chronic intestinal disease and pancreatic disorders, such as inflammatory bowel disease (IBD), celiac disease (CelD), and pancreatic cancer, remains unclear. Possible factors contributing to this association include drug use, changes in microcirculation, disruption of gut permeability/motility, bacterial translocation, and activation of gut-associated lymphoid tissue. Other systemic conditions, like IgG4-related disease or vasculitides, may also affect both the pancreas and the intestinal tract. This review provides an overview of the current understanding of this complex relationship.

European' health care indicators and pancreatic cancer incidence and mortality: A mediation analysis of Eurostat data and Global Burden of Disease Study 2019

Summary: The aim of this study was to explore the correlation between incidence and mortality of pancreatic cancer and healthcare indicators in 36 European countries. The results showed that both incidence and mortality were increasing, and they were strongly positively correlated. Higher health expenditure, availability of beds and medical technology, as well as the number of practicing doctors were related to higher incidence. On the other hand, lower mortality was associated with indicators such as outpatient curative care expenditure, the number of pet scanners, and radiation therapy equipment. The study highlights the impact of healthcare environment on pancreatic cancer incidence and mortality.

Summary: This review summarizes the role of 24-hour urinary 5-hydroxyindolacetic acid (24u5HIAA) in the diagnosis and follow-up of neuroendocrine tumors-related carcinoid syndrome (NETs-CS). 24u5HIAA is an established biomarker with high sensitivity and specificity for diagnosing NETs-CS. It is also useful in the follow-up of patients, as a shorter double time and elevated levels are associated with an increased risk of disease progression and mortality.
