Staphylococcus aureus versus Staphylococcus epidermidis in periprosthetic Check for joint infection-Outcome analysis of methicillin-resistant versus methicillin-susceptible strains

Periprosthetic joint infections (PJIs) are a major complication in total joint arthroplasty. Staphylococcus aureus and coagulase-negative staphylococci are known to cause the majority of all PJIs This study aimed to analyze the eradication rates of S. aureus and S. epidermidis with methicillin susceptibility and methicillin resistance in a 2-stage therapy algorithm. Seventy-four patients with PJI caused by methicillin-resistant S. aureus (MRSA), methicillin-resistant coagulase-negative staphylococci (MRSE), methicillin-susceptible S. aureus (MSSA), and methicillin-susceptible coagulase-negative staphylococci (MSSE) were included, and the outcome was analyzed retrospectively. After a minimal follow-up of 2 years, n = 56 patients (75.7%) were definitively free of infection. The analysis revealed significant differences between the groups, with eradication rates as follows: MSSA (92.6%), MSSE (95.2%), MRSA (80%), and MRSE (54.2%). MRSE showed a significantly lower rate of patients graded as definitively free of infection as compared to patients with infections caused by MSSA, MSSE, and MRSA. (C) 2018 Elsevier Inc. Summary: This study confirms the safety of using SmartPill (R) after surgery, but highlights the importance of patient compliance for meaningful data. Objective analysis of transit times and peristalsis was possible in over 95% of cases, regardless of surgical type and location, suggesting potential for SmartPill (R) to provide objective parameters for POI severity in future clinical trials. However, challenges in analyzing small bowel transit were noted in some cases with prolonged POI.

SARS-CoV-2 infection and venous thromboembolism after surgery: an international prospective cohort study

Summary: Surgical patients with peri-operative or recent SARS-CoV-2 infection are at increased risk of postoperative venous thromboembolism compared to those without a history of SARS-CoV-2. However, the optimal prophylaxis and treatment for venous thromboembolism in this patient population remains unknown. Venous thromboembolism is independently associated with 30-day mortality.

This is the first reported case of PJI caused by Kytococcus schroeteri, a rarely encountered Gram-positive coccus. K. schroeteri, which belongs to the micrococcus branch, is typically found as a symbiotic bacterium on the skin.

The results showed high agreement between TTC assay and the recommended broth microdilution method, indicating the potential of TTC as a promising indicator in drug susceptibility testing for NTM.

Current status of recombinase polymerase amplification technologies for the detection of pathogenic microorganisms

Summary: Rapid detection of pathogenic microorganisms is crucial for epidemiologic identification, prevention, and control in public health. PCR-based methods have limitations in resource-limited areas due to the need for precise temperature control. Recombinase polymerase amplification (RPA) is a new nucleic acid amplification technology that can amplify DNA or RNA at a constant temperature, offering simplicity, high specificity and sensitivity, and short detection time. Microfluidic technology combined with RPA enables rapid detection of pathogenic microorganisms by integrating nucleic acid extraction, amplification, and detection.

microRNA as biomarkers in tuberculosis: a new emerging molecular diagnostic solution

Summary: Tuberculosis is a lethal infectious disease caused by Mycobacterium tuberculosis. During infection, the expression of host microRNAs changes, which could be potential biomarkers for early detection of tuberculosis. Further research is needed to identify potential microRNA biomarkers, but momentum is gaining in this field and microRNAs are expected to become routine approaches for reliable diagnosis and specific therapeutic interventions.
