Staphylococcus aureus versus Staphylococcus epidermidis in periprosthetic Check for joint infection-Outcome analysis of methicillin-resistant versus methicillin-susceptible strains

Periprosthetic joint infections (PJIs) are a major complication in total joint arthroplasty. Staphylococcus aureus and coagulase-negative staphylococci are known to cause the majority of all PJIs This study aimed to analyze the eradication rates of S. aureus and S. epidermidis with methicillin susceptibility and methicillin resistance in a 2-stage therapy algorithm. Seventy-four patients with PJI caused by methicillin-resistant S. aureus (MRSA), methicillin-resistant coagulase-negative staphylococci (MRSE), methicillin-susceptible S. aureus (MSSA), and methicillin-susceptible coagulase-negative staphylococci (MSSE) were included, and the outcome was analyzed retrospectively. After a minimal follow-up of 2 years, n = 56 patients (75.7%) were definitively free of infection. The analysis revealed significant differences between the groups, with eradication rates as follows: MSSA (92.6%), MSSE (95.2%), MRSA (80%), and MRSE (54.2%). MRSE showed a significantly lower rate of patients graded as definitively free of infection as compared to patients with infections caused by MSSA, MSSE, and MRSA. (C) 2018 Elsevier Inc. Single, Recurrent, Synchronous, and Metachronous Periprosthetic Joint Infections in Patients With Multiple Hip and Knee Arthroplasties

Summary: This study assessed the prevalence of recurrent, synchronous, and metachronous periprosthetic joint infection (PJI) in patients who had multiple arthroplasties and identified risk factors for a subsequent PJI. The results showed that over 20% of patients with multiple arthroplasties and a single PJI will develop a subsequent PJI, with 12% recurring in the initial arthroplasty and nearly 10% occurring in another arthroplasty. Tobacco use, bacteremia, and Staphylococcus aureus isolation at the time of the initial PJI were identified as particular risk factors.

Successful eradication rate following one-stage septic knee and hip exchange in selected pre-operative culture-negative periprosthetic joint infections

The study included 933 primary THA patients, with no statistically significant differences in age, sex, or operated side between the group that underwent chlorhexidine bathing and the control group. While PJI cases were more common in the control group, the difference was not statistically significant.

The Outcomes of Hip and Knee Fungal Periprosthetic Joint Infections: A Retrospective Cohort Study

Most patients with fungal PJI presented with open wounds or a history of sinus disease.

Risk factor analysis for above-knee amputation in patients with periprosthetic joint infection of the knee: a case-control study

Summary: Periprosthetic joint infection (PJI) following knee arthroplasty is a severe complication that may require above-knee amputation (AKA) for control, although the risk factors for this endpoint are not well known. This retrospective case-control study found that severe comorbidities, alcohol abuse, and preoperative anemia were more frequently associated with AKA among PJI patients. Preoperative anemia was present in all AKA patients and in a majority of limb salvage (LS) patients, highlighting it as a potential risk factor for AKA in the setting of PJI.

Profiling the Immune Response to Periprosthetic Joint Infection and Non-Infectious Arthroplasty Failure

Summary: Arthroplasty failure, a major complication of joint replacement surgery, can be caused by periprosthetic joint infection (PJI) or non-infectious etiologies. Differentiating between PJI and non-infectious arthroplasty failure (NIAF) is crucial for proper medical and surgical treatment. Various methods, such as culture, nucleic acid amplification tests, and metagenomic techniques, have been successful in distinguishing between the two. Host-based approaches, including biomarkers and immune response profiling, hold promise in overcoming limitations of microbial-based detection methods and shedding light on the causes of arthroplasty failure.

The results showed that synovial white blood cell count and synovial polymorphonuclear percentage had good discriminatory ability in the group with antibiotic use, indicating that the use of antibiotics should not exclude the utility of these markers in the diagnosis of late PJI.

SARS-CoV-2 infection and venous thromboembolism after surgery: an international prospective cohort study

Summary: This study evaluated risk factors for failure of custom-made acetabular implants in revision total hip arthroplasty (RTHA). The study found that previous periprosthetic joint infection, additional revision of femoral components, rheumatoid disease, elevated preoperative CRP, and low preoperative hemoglobin were statistically significant risk factors associated with treatment failure.

Summary: Gram-positive cocci, including Staphylococcus aureus, Staphylococcus epidermidis, and other coagulase-negative staphylococci, are common causes of periprosthetic joint infections (PJI). This is the first reported case of PJI caused by Kytococcus schroeteri, a rarely encountered Gram-positive coccus. K. schroeteri, which belongs to the micrococcus branch, is typically found as a symbiotic bacterium on the skin.

Summary: Preoperative SARS-CoV-2 vaccination can support safer elective surgery. The number needed to vaccinate (NNV) to prevent COVID-19-related deaths is more favorable in surgical patients compared to the general population. Prioritizing preoperative vaccination of patients needing elective surgery globally could prevent additional COVID-19-related deaths in 1 year.

Diagnosis of Helicobacter pylori infection of the gastric biopsy by rapid urease test, histopathology and Raman spectroscopy

Summary: This study aimed to compare the diagnostic potential of Raman spectroscopy with rapid urease test and histopathology in diagnosing H. pylori infection. The results showed that Raman spectroscopy had higher sensitivity, accuracy, and specificity compared to the rapid urease test and histopathology. This study demonstrates the applicability of Raman spectroscopy as an innovative detection tool for molecular diagnosis of H. pylori infection in gastritis.

DBS can be a simple and convenient alternative to plasma or serum for HBsAg detection, but site-specific validation of the assay is necessary for anti-HCV detection.

The pathology biopsy represents the gold standard for diagnosis: a case report

Summary: This case report describes a 70-year-old male patient with previously unknown immunodeficiency. Multiple pulmonary nodular shadows were observed on radiography. Fungal infection was detected in various samples, and the patient was ultimately diagnosed with disseminated Talaromyces marneffei infection and B-cell non-Hodgkin's lymphoma. The main significance of this study is to emphasize the importance for clinicians to obtain comprehensive specimens from patients presenting with multiple masses in order to ensure accurate clinical diagnosis.

Summary: This study proposed and evaluated a drug susceptibility testing method using 2,3,5-triphenyl tetrazolium chloride (TTC) as a colorimetric indicator for the clinical relevant non-tuberculous mycobacteria (Mycobacterium abscessus complex (MABC), M. avium complex (MAC), and M. kansasii). The results showed high agreement between TTC assay and the recommended broth microdilution method, indicating the potential of TTC as a promising indicator in drug susceptibility testing for NTM.

Current status of recombinase polymerase amplification technologies for the detection of pathogenic microorganisms

Summary: Rapid detection of pathogenic microorganisms is crucial for epidemiologic identification, prevention, and control in public health. PCR-based methods have limitations in resource-limited areas due to the need for precise temperature control. Recombinase polymerase amplification (RPA) is a new nucleic acid amplification technology that can amplify DNA or RNA at a constant temperature, offering simplicity, high specificity and sensitivity, and short detection time. Microfluidic technology combined with RPA enables rapid detection of pathogenic microorganisms by integrating nucleic acid extraction, amplification, and detection.

microRNA as biomarkers in tuberculosis: a new emerging molecular diagnostic solution

Summary: Tuberculosis is a lethal infectious disease caused by Mycobacterium tuberculosis. During infection, the expression of host microRNAs changes, which could be potential biomarkers for early detection of tuberculosis. Further research is needed to identify potential microRNA biomarkers, but momentum is gaining in this field and microRNAs are expected to become routine approaches for reliable diagnosis and specific therapeutic interventions.
