Summary: This study evaluates the feasibility of using pooling patient specimens with the FilmArray Respiratory Panel 2.1 to increase testing capacity and maintain detection of SARS-CoV-2 and influenza virus. The results show that pooling samples has a high detection rate and positive agreement, and the sensitivity of the pooling assay meets the detection requirements for nucleic acid amplification testing.

Summary: This study evaluated the diagnostic performance of the Biofire(©) FilmArray pneumonia (PN)-panel against standard culture in broncho-alveolar lavage (BAL) samples. The results showed that the PN-panel had high sensitivity and specificity, but low positive predictive value. The detection rate of bacterial pathogens was higher in the PN-panel compared to culture. Further evaluation is needed to assess the impact of the PN-panel on clinical management.

Summary: Human bocavirus (HBoV) is an important respiratory pathogen, especially in children, but it is often found in co-detection with other respiratory viruses, which makes the diagnostic approach challenging. We compared multiplex PCR and quantitative PCR for HBoV with multiplex tandem PCR (MT-PCR) in 55 cases of co-detection of HBoV and other respiratory viruses. In addition, we investigated whether there is a connection between the severity of the disease, measured by the localization of the infection, and amount of virus detected in the respiratory secretions. No statistically significant difference was found, but children with large amount of HBoV and other respiratory virus had a longer stay in hospital.

However, the design and validation of multiplex rRT-PCR are more complex, and comprehensive research is lacking. This review provides guidance for designing and optimizing multiplex rRT-PCR detection panels and assesses the clinical utility of multiplex rRT-PCR in viral respiratory tract infection diagnosis.

Summary: This study aimed to assess the circulation and molecular evolution of influenza viruses (IVs), particularly A(H3N2), during the 2021-2022 season in Italy. Analysis of respiratory specimens collected from January 1, 2022 to May 31, 2022 showed that all confirmed IVs were type A, with a majority being H3N2. Phylogenetic analysis revealed genetic clusters and mutations in the HA gene sequences, suggesting the presence of drifted strains. Summary: We report the isolation of a strain of Pantoea genus carrying a VIM carbapenemase, which is the first of its kind. The strain was obtained from a rectal swab of a 10-day-old newborn in a neonatal intensive care unit (NICU). Whole-genome sequencing revealed that the strain was identified as Pantoea brenneri. The strain harbored the carbapenemase gene bla(VIM-1). Immediate contact measures and isolation of the newborn prevented the dissemination of VIM-producing P. brenneri and the plasmid carrying the VIM-1 gene to other newborns.

Summary: Torque teno virus (TTV) can be detected in over 90% of healthy individuals. It can be detected in various samples such as blood, serum, breast milk, cerebrospinal fluid, and saliva, as well as in different tissues such as the vagina and hepatocytes. TTV DNAemia is stable in healthy infected individuals, while TTV viral load is inversely correlated with T lymphocyte numbers and functions in immunocompromised populations. The role of TTV in cancer pathogenesis and response to anticancer treatments is still not well-known.

Summary: Through a genome-wide rare variant burden association analysis, it was found that there is an association between at-risk variants in the TLR7 gene and rare loss-of-function variants in TLR3-dependent type I interferon immunity genes. These findings suggest that rare variants in TLR3- and TLR7-dependent type I interferon immunity genes may underlie life-threatening COVID-19 in patients under 60 years old.

Summary: The study evaluates the booster effect of an S1 subunit vaccine in aged mice previously primed with adenoviral vaccines. It demonstrates that a booster dose of the rS1Beta subunit vaccine, given to mice, induces strong and long-lived S1-specific immune responses and significantly increases neutralizing antibodies against Wuhan, Beta, and Delta strains, including cross-reactive antibodies against Omicron variants. The findings suggest that the rS1Beta subunit vaccine can offer cross-neutralization against broad variants, providing important implications for controlling breakthrough SARS-CoV-2 variants in elderly individuals primed with adenovirus-based vaccines.

The findings revealed differences in bacterial distribution between different response groups.

Summary: This study aimed to explore the efficacy of autologous EBV-specific CTL transplantation in the treatment of recurrent/metastatic EBV-NPC patients after chemotherapy. The results showed that patients who received CTL transplantation had a 5-year overall survival rate of 44% and a median overall survival of 60 months. This indicates that autologous CTL transplantation can effectively control the disease.

Summary: We prospectively evaluated the long-term immunogenicity of the third dose of the SARS-CoV-2 BNT162b2 mRNA vaccine in 55 patients undergoing cancer treatment. Our findings indicate that one year after receiving the third dose, more than 67% of patients showed an immune response against the virus, with less than 20% of patients showing no response to certain variants.

Summary: This study aimed to compare the diagnostic potential of Raman spectroscopy with rapid urease test and histopathology in diagnosing H. pylori infection. The results showed that Raman spectroscopy had higher sensitivity, accuracy, and specificity compared to the rapid urease test and histopathology. This study demonstrates the applicability of Raman spectroscopy as an innovative detection tool for molecular diagnosis of H. pylori infection in gastritis.

Summary: This case report describes a 70-year-old male patient with previously unknown immunodeficiency. Multiple pulmonary nodular shadows were observed on radiography. Fungal infection was detected in various samples, and the patient was ultimately diagnosed with disseminated Talaromyces marneffei infection and B-cell non-Hodgkin's lymphoma. The main significance of this study is to emphasize the importance for clinicians to obtain comprehensive specimens from patients presenting with multiple masses in order to ensure accurate clinical diagnosis.

Summary: This study proposed and evaluated a drug susceptibility testing method using 2,3,5-triphenyl tetrazolium chloride (TTC) as a colorimetric indicator for the clinical relevant non-tuberculous mycobacteria (Mycobacterium abscessus complex (MABC), M. avium complex (MAC), and M. kansasii). The results showed high agreement between TTC assay and the recommended broth microdilution method, indicating the potential of TTC as a promising indicator in drug susceptibility testing for NTM.

Summary: Rapid detection of pathogenic microorganisms is crucial for epidemiologic identification, prevention, and control in public health. PCR-based methods have limitations in resource-limited areas due to the need for precise temperature control. Recombinase polymerase amplification (RPA) is a new nucleic acid amplification technology that can amplify DNA or RNA at a constant temperature, offering simplicity, high specificity and sensitivity, and short detection time. Microfluidic technology combined with RPA enables rapid detection of pathogenic microorganisms by integrating nucleic acid extraction, amplification, and detection.

Summary: Tuberculosis is a lethal infectious disease caused by Mycobacterium tuberculosis. During infection, the expression of host microRNAs changes, which could be potential biomarkers for early detection of tuberculosis. Further research is needed to identify potential microRNA biomarkers, but momentum is gaining in this field and microRNAs are expected to become routine approaches for reliable diagnosis and specific therapeutic interventions.
