Carotid-Femoral Pulse Wave Velocity Is Associated With Cerebral White Matter Lesions in Type 2 Diabetes

OBJECTIVE-Patients with type 2 diabetes have a high incidence of cardiovascular events including stroke. Increased arterial stiffness (AS) predicts cardiovascular events in the general population. Cerebral white matter lesions (WMLs) are associated with an increased risk of stroke. It is unknown whether AS in patients with type 2 diabetes is associated with WMLs. RESEARCH DESIGN AND METHODS-We examined 89 patients recently diagnosed with type 2 diabetes (<5 years) and 89 sex- and age-matched controls. AS was assessed with carotid-femoral pulse wave velocity (PWV). WMLs were identified using magnetic resonance imaging and graded qualitatively with the Breteler scale (no/slight changes = 0, moderate changes = 1, severe changes = 2) and semiquantitatively. RESULTS-The diabetic population had excellent glycemic control (HbA(1c), 6.5% [6.2-6.8]; median [interquartile range {IQR}]) and had, compared with the controls, lower office blood pressure (BP) (127 +/- 12/79 +/- 8 vs. 132 +/- 14/84 +/- 10 mmHg) and total cholesterol (4.3 [3.9- 4.71vs. 5.6 [5.1-6.4]; mmol/L; median {IQR]), (P < 0.01 for all). Despite this, PWV was higher in the patients with diabetes compared with controls (9.3 +/- 2.0 vs. 8.0 +/- 1.6 m/s; P < 0.0001). PWV was associated with Breteler score (OR 1.36 [95% CI 1.17-1.581; P < 0.001) and WML volume (OR 1.32 [95% CI 1.16-1.511; P < 0.001) per 1 m/s increase in PWV. These associations remained significant when adjusted for age, sex, diabetes, 24-h mean arterial BP, BMI, heart rate, and use of antihypertensives and statins (Breteler score: OR 1.28 [95% Cl 1.03-1.601; P < 0.05 and WML volume: OR 1.30 [95% Cl 1.06-1.58]; P <005). CONCLUSIONS-PWV was higher among patients with well-controlled type 2 diabetes compared with controls and was independently associated with WMLs. PWV may represent a clinically relevant parameter in the evaluation of cerebrovascular disease risk in type 2 diabetes. Diabetes Care 36:722-728, 2013

The analysis showed that patients in severe stages of CLI who received a second opinion from a dedicated multidisciplinary team had high limb salvage and survival rates after major amputation.

The concept of the dominant value margin is introduced as a metric for the effectiveness of divide-and-conquer strategies. However, the outcome of a market where suppliers with larger dominant value margins can sell to all customers does not necessarily result in social efficiency.

Summary: This study investigated the effects of Semaglutide and Empagliflozin, alone or in combination, on markers of retinal function. The results showed that these treatments did not alter retinal oxygenation, retinal vascular autoregulation, or retinal vessel diameter. However, Semaglutide slightly increased central retinal thickness, although the clinical significance of this finding is uncertain.

Rebound Hypoglycemia and Hyperglycemia in Type 1 Diabetes

Summary: The aim of this study was to investigate rebound hypoglycemic and hyperglycemic events and their relation to other glycemic metrics. Data from 159 persons with type 1 diabetes were analyzed and it was found that there was a strong correlation between rebound hypoglycemia and rebound hyperglycemia, suggesting an individual behavioral characteristic toward intensive correction of glucose excursions.

Best Practice Guide for Cryoballoon Ablation in Atrial Fibrillation: The Compilation Experience of More than 1000 Procedures

Summary: Nowadays, cryoballoon (CB) has become an established alternative to radio frequency (RF) ablation for pulmonary vein isolation (PVI), with the ability to isolate PVs in a single application. In a study on over 1000 consecutive patients, our center optimized the CB PVI procedure. It is expected that future guidelines will recommend CB as the first-line PVI for patients with paroxysmal AF and a class I indication. In a long-term follow-up of the EARLY-AF trial, CB showed a lower incidence of persistent atrial fibrillation compared to anti-arrhythmic drugs.

Glycemic Metrics Derived From Intermittently Scanned Continuous Glucose Monitoring

Summary: This study analyzed isCGM and HbA1c data and found that there is an optimal regression relationship between the average glucose over 60 days and HbA1c. Time in range is significantly correlated with glucose management indicator. High glycemic variability is positively associated with time below range. The combination of time in range and time below range is suggested as a new composite quality indicator.

Variation of glucose time in range in type 1 diabetes

Summary: This study assessed the variation of glucose time in range (TIR) for individuals with type 1 diabetes who use intermittently scanned continuous glucose monitoring (isCGM). The results showed that the within-individual standard deviation (SD) for TIR was small, but there were large differences in TIR between different time periods. However, these findings may not be applicable to individuals using closed-loop insulin delivery with insulin pumps.
