Purpose of review Lung transplant continues to be hampered by the number of available donors. Ex-vivo lung perfusion (EVLP) has emerged as an essential tool for the reassessment, under a controlled scenario, of lungs that initially did not meet transplantation criteria. The purpose of the current study is to review the most recent clinical and research reports and summarize their findings. Recent findings Several centers have presented positive data when using ex-vivo perfusion to identify better grafts from the higher risk donor pool. The posttransplant results, when using these organs, are not significantly different from those obtained when using standard criteria donors. In addition, a great amount of research reports have emphasized the potential of ex-vivo perfusion as a platform in reducing the injurious effects of ischemia-reperfusion. Summary EVLP has already proved its value as a tool to identify 'good' lungs from the previously rejected pool. But as new therapeutics arise, EVLP will also prove its value as a reproducible platform for their evaluation.

Waiting list activations decreased overall, while waiting list deaths remained similar regardless of local COVID-19 prevalence.

Summary: Gastroparesis is common after lung transplantation and is associated with worse outcomes, particularly the development of CLAD. Black patients are more likely to be diagnosed with gastroparesis post-transplant. While gastroparesis is linked to CLAD, it does not impact overall mortality.

Summary: The study found that high levels of bile acids and predominance of conjugated bile acids are independent predictors of chronic lung allograft dysfunction, mortality, and bacterial infections. Summary: This study investigated the outcomes of recipients bridged to a first-time retransplantation (ReTx) by extracorporeal life support (ECLS). The results showed that the overall survival of the ECLS-ReTx group was significantly worse compared to the regular-ReTx and index-Tx groups. Risk factors such as recipient age, intertransplant interval, primary graft dysfunction as transplant indication, and type of ECLS were identified as significant factors affecting mortality in ECLS-ReTx recipients.

Malakoplakia Presenting as a Lung Mass in a Lung Transplant Recipient: Case Report

Summary: Lung nodules or masses can occur after lung transplant and can be caused by various malignant or benign conditions, including opportunistic infections. In this study, we report the clinical course and management of a lung transplant recipient with pulmonary malakoplakia and provide a literature review. This is the first reported case of pulmonary malakoplakia in a lung allograft caused by Escherichia coli infection.

Summary: This study analyzed the clinical presentation, management, and outcomes of lung transplant recipients during the first and second surge of the COVID-19 pandemic. The results showed that the second surge cohort had a higher proportion of patients with mild disease, fewer patients with a history of obesity, and a decreased mortality rate compared to the first cohort.

Summary: This study describes the characteristics, management, and outcomes of lung transplantation in 11 candidates with HPS-related pulmonary fibrosis. Summary: This study aimed to identify factors associated with achieving out-of-bed physical therapy in ECMO-supported patients and assess the safety and feasibility of early mobilization with femoral cannulation. The results showed that pre-transplantation, venovenous ECMO, later cannulation year, and higher Charlson comorbidity index were associated with increased odds of achieving out-of-bed physical therapy, while invasive mechanical ventilation and femoral cannulation were associated with decreased odds. Physical therapy with femoral cannulation was safe, feasible, and had low complication rates.
