CTA pulmonary artery enlargement in patients with severe aortic stenosis: Prognostic impact after TAVR

Valery L. Turner, Ayman Jubran, Juyong Brian Kim, Eva Maret, Kegan J. Moneghetti, Francois Haddad, Myriam Amsallem, Marina Codari, Virginia Hinostroza, Domenico Mastrodicasa, Anna M. Sailer, Yukari Kobayashi, Takeshi Nishi, Alan C. Yeung, Amelia C. Watkins, Anson M. Lee, D. Craig Miller, Michael P. Fischbein, William F. Fearon, Martin J. Willemink, Dominik Fleischmann

Summary: Enlargement of the main pulmonary artery on pre-procedural CTA is independently associated with 1-year mortality after TAVR, potentially improving prognostic accuracy in these patients.

Christopher Malaisrie, Seo Young Park, Sung-Cheol Yun, Euihong Ko, Hanbit Park, Seung-Ah Lee, Dae-Hee Kim, Ho Jin Kim, Joon Bum Kim, Suk Jung Choo, Duk-Woo Park, Seung-Jung Park

Summary: There were significant baseline differences between Asian and non-Asian patients undergoing TAVI, but the observed clinical outcome differences were largely explained by baseline variations in clinical, anatomical, and procedural factors.

Left Ventricular Hypertrophy and Biomarkers of Cardiac Damage and Stress in Aortic Stenosis

Elliot J. Stein, William F. Fearon, Sammy Elmariah, Juyong B. Kim, Samir Kapadia, Dharam J. Kumbhani, Linda Gillam, Brian Whisenant, Nishath Quader, Alan Zajarias, Frederick G. Welt, Anthony A. Bavry, Megan Coylewright, Robert N. Piana, Ravinder R. Mallugari, Daniel E. Clark, Jay N. Patel, Holly Gonzales, Deepak K. Gupta, Anna Vatterott, Natalie Jackson, Shi Huang, Brian R. Lindman

Summary: This study found that elevated levels of cardiac troponin (cTnT) and N-terminal pro-B-type natriuretic peptide (NT-proBNP) are associated with left ventricular hypertrophy (LVH) and cardiac injury in patients with severe aortic stenosis. These biomarkers can predict post-transcatheter aortic valve replacement mortality better than left ventricular mass index (LVMI), which has important implications for risk stratification and treatment of patients with aortic stenosis.

Global Longitudinal Strain and Biomarkers of Cardiac Damage and Stress as Predictors of Outcomes After Transcatheter Aortic Valve Implantation

Gupta, Anna Vatterott, Natalie Jackson, Shi Huang, Brian R. Lindman

Summary: Elevations in circulating cardiac troponin and NT-proBNP are associated with worsening GLS in patients with severe aortic stenosis. Biomarkers of cardiac damage and stress are independently associated with mortality after transcatheter aortic valve implantation, whereas GLS is not.

Modulation of mouse laryngeal inflammatory and immune cell responses by low and high doses of mainstream cigarette smoke

Meena Easwaran, Joshua D. Martinez, Juyong Brian Kim, Elizabeth Erickson-DiRenzo

Summary: This study evaluated the inflammatory and immune cell responses in the larynx after exposure to cigarette smoke. Using gene expression analysis and immunohistochemistry, it was found that high dose exposure to cigarette smoke had anti-inflammatory and immunosuppressive effects.

Reduced Pulmonary Artery Distensibility Predicts Persistent Pulmonary Hypertension and 2-Year Mortality in Patients with Severe Aortic Stenosis Undergoing TAVR

Valery Turner, Eva Maret, Juyong B. Kim, Marina Codari, Virginia Hinostroza, Domenico Mastrodicasa, Claire Watkins, William F. Fearon, Michael P. Fischbein, Francois Haddad, Martin J. Willemink, Dominik Fleischmann

Summary: This study found that pulmonary artery distensibility measured on preprocedural CTA is independently associated with persistent PH and two-year mortality in patients undergoing TAVR.

Air Pollution, Built Environment, and Early Cardiovascular Disease

Kai Zhang, Robert D. Brook, Yuanfei Li, Sanjay Rajagopalan, Juyong Brian Kim

Summary: As the world becomes more urbanized, there is growing concern about how urban environments impact cardiovascular health. Urban residents are exposed to various adverse environmental factors such as air pollution, built environment, and lack of green space, which may contribute to the development of cardiovascular disease and related risk factors. This article provides an overview of studies examining the impact of the environment, discusses current challenges in the field, and suggests potential directions for future research. The clinical implications of these findings are highlighted, alongside proposed interventions to promote cardiovascular health among children and young adults.

Discovery of Transacting Long Noncoding RNAs That Regulate Smooth Muscle Cell Phenotype

Huitong Shi, Trieu Nguyen, Quanyi Zhao, Paul Cheng, Disha Sharma, Hyun-Jung Kim, Juyong Brian Kim, Robert Wirka, Chad S. Weldy, Joao P. Monteiro, Thomas Quertermous

Summary: In this study, deep transcriptome sequencing of human coronary artery smooth muscle cells (SMCs) revealed 4579 known and 13655 de novo long noncoding RNAs (lncRNAs). These lncRNAs were found to be enriched near CAD-related transcription factors, genetic loci, and gene regulators of SMC identity. Two novel lncRNAs, ZIPPOR and TNS1-AS2, were identified to exert their function in SMC phenotypic transition through the CAD gene ZEB2.

Summary: This study demonstrates a correlation between cardiac structure and metabolism in patients with aortic stenosis, suggesting that metabolic dysfunction may be involved in the progression and mortality risk of this condition. These findings provide insights for improving the rehabilitation and survival of patients with aortic stenosis.

Whisenant, Nishath Quader, Alan Zajarias, Frederick G. Welt, Anthony A. Bavry, Megan Coylewright, Anna Vatterott, Natalie Jackson, Shi Huang, Brian R. Lindman

Summary: The study aimed to determine the prevalence of depression and cognitive dysfunction (CD) in patients undergoing transcatheter aortic valve replacement (TAVR) and assess their association with mortality and quality of life. The findings revealed that depression and CD are common in TAVR patients and are associated with increased mortality and worse quality of life.

Distance between valvular leaflet and coronary ostium predicting risk of coronary obstruction during TAVR

Jun-Hyok Oh, Yuhei Kobayashi, Guson Kang, Takeshi Nishi, Martin J. Willemink, William F. Fearon, Michael Fischbein, Dominik Fleishmann, Alan C. Yeung, Juyong Brian Kim

Summary: The study suggests that a combination of ELOD < 2 mm and a longer leaflet length than mid-ostial height may be associated with an increased risk for coronary obstruction during TAVR, aiding in predicting the risk of coronary obstruction in patients.
