Genetics of susceptibility to infection with enteric pathogens

Purpose of review This review examines recent developments in human genetic susceptibility to enteropathogens that cause infectious diarrhea. Recent findings The affinity of specific norovirus genogroups to different histoblood group antigens (HBGAs) on secretor cells has been studied in different epidemiologic studies. HBGAs are also used as receptors by Vibrio cholerae with different degrees of affinity between biotypes. Polymorphisms in the CD14, lactoferrin and osteoprotegerin promoter genes were associated to diarrhea in travelers. Single nucleotide polymorphisms in the IL-8 genes are also associated to increased risk for enteroaggregative Escherichia coli and Clostridium difficile infection. IL-10 haplotypes were associated to enterotoxigenic E coli associated diarrhea in exposed individuals. A family-based study showed a significant association of the LPLUNC1 gene and cholera. The major histocompatibility complex class II antigens are associated to different degrees of susceptibility and resistance to Salmonella, Cryptosporidium and Entamoeba infection. Summary Variants in genes that encode molecules that mediate attachment, pathogen recognition, inflammatory cytokine response, innate and acquired immunity are being identified as determinants of host genetic susceptibility to infectious diarrhea.

Summary: This study investigated Clostridioides difficile infection in cancer patients, finding that patients with EIA+ were older, more likely to fail therapy and experience recurrence, with the presence of ribotypes associated with poor outcomes increasing the risk of treatment failure.

Human herpesvirus 6 and central nervous system disease in oncology patients: A retrospective case series and literature review

Summary: This study reviewed HHV-6 CNS infection in patients admitted to MD Anderson Cancer Center between March 2016 and December 2018. Among 725 patients tested for HHV-6, 19 cases (2.6%) of HHV-6 mediated CNS disease were identified, with most patients being HSCT recipients. Survival rate at 240 days post-transplant was 62%. The study also found that significant diagnostic discordance exists between testing platforms, and HHV-6 reactivation leading to CNS disease can also occur in patients receiving CAR T-cell therapy or biologic immunotherapy.

Norovirus in Cancer Patients: A Review

Summary: Norovirus is the leading cause of viral-related diarrhea in cancer patients, with immunosuppressed patients shedding the virus for longer periods. Challenges in diagnosis and treatment of Norovirus infection in cancer patients exist, with potential treatments including screening for inhibitors and developing specific T cells.

Summary: This study provides a highly accurate and complete reference genome of Cryptosporidium parvum, based on Oxford Nanopore reads and improved using Illumina reads. The assembly includes 8 chromosomes and resolves 13 telomeres, with a high completion rate of 98.4% single-copy BUSCO genes. This high-quality reference genome will facilitate future comparative genomic studies across the Cryptosporidium clade.
