Summary: The Saudi Critical Care Society (SCCS) formed a panel of experts to issue evidence-based recommendations for the management of COVID-19 in the intensive care unit. They used the GRADE approach and evidence-to-decision framework to assess the quality of evidence and generate recommendations. The panel issued 12 recommendations on pharmacotherapeutic interventions for severe and critical COVID-19.

Summary: Most Medicare ACOs use care plans to manage care of complex patients. The care plans include patient history, current medical needs, and future care plans. Care plans are mostly developed and maintained by care management staff.

Summary: Resilience Engineering principles are gaining popularity in healthcare and FRAM is the most well-known method used. However, there is a lack of guidance on its application, which may hinder its adoption and lead to misuse. This article provides self-reflective analysis of FRAM use cases and identifies lessons for successful application in healthcare. Four themes are developed, including core characteristics, flexibility in epistemological paradigm, diversity in interventions, and model complexity.

Summary: The study aims to estimate the joint impact of social care, public health, and healthcare expenditure on mortality in England, and to assess the effect of spending constraints post-2010 on total mortality. The findings suggest that all forms of public healthcare-related expenditure save lives, with additional social care expenditure being more productive than additional healthcare expenditure. The results indicate that the slowdown in life expectancy improvement in England and Wales since 2010 may be attributed to spending constraints in the healthcare and social care sectors.

Summary: This study retrospectively analyzed the trends in intraoperative fluid and vasopressor administration in patients undergoing elective abdominal surgery. The results showed an association between these factors and acute kidney injury. The findings support previous randomized trials and highlight the challenge of translating evidence into clinical practice.

Summary: The VICTORIA trial showed that Vericiguat reduced the risk of cardiovascular death or heart failure hospitalization in high-risk heart failure patients. However, the treatment effect varied depending on baseline NT-proBNP levels. The economic analysis revealed that the cost-effectiveness of Vericiguat was influenced by the observed NT-proBNP treatment effect heterogeneity.

Summary: This review focuses on the best practices in malnutrition screening and assessment, estimation of energy needs, risks and management of sarcopenic obesity, tailored nutrition recommendations, and role of immunonutrition in critically ill patients with obesity. Knowledge gaps persist in managing the complex nutritional needs of these patients. Education efforts and research on current guidelines and evidence are needed to optimize nutrition strategies.

Summary: This study examines care coordination for healthcare referrals in accountable care organizations (ACOs). Traditional fee-for-service and capitation agreements are found to be insufficient in incentivizing care coordination, while a risk- and cost-sharing mechanism can induce integrated care coordination efforts and improve profit variability.

Summary: This study examines the practice styles and skills of emergency department physicians and their impact on healthcare operations. The research finds significant variation across physicians in their practice styles and skills, and that physicians with costly practice styles often have worse outcomes in terms of more ED revisits and more hospitalizations.

Summary: Healthcare institutions aim to provide quality service and safety, and can undergo accreditations and comparisons with other institutions to achieve recognition. The role of managers is to communicate results and propose strategies for improvement. However, the focus should be on improving conditions for patients and staff rather than pursuing prestige.

Summary: Critical care telemedicine (CCT) enables access to expertise, reduces variability in care, and prepares health systems for sudden increases in demand. Successful implementation of CCT requires strong teamwork between bedside and hub teams, training for ICU clinicians, and ensuring staff have the necessary knowledge and support for optimal utilization of CCT.

Summary: This survey of CCM-trained anesthesiologists identified a high rate of board certification, practice in academic settings, and involvement in resident education. Areas of dissatisfaction included burnout, work-life balance, and lack of respect, suggesting potential strategies to improve satisfaction with anesthesia critical care practice and fellowship training.

Summary: Using a regression discontinuity design, this study investigated the impact of no cost sharing on paediatric care usage and health outcomes in Japan. The results showed that no cost sharing was associated with increased outpatient care usage and spending, but did not affect inpatient care usage. Notably, the effect of no cost-sharing policy was greater among children from high-income households, suggesting that it disproportionately benefits higher-income families and may contribute to larger disparities.

Summary: Management of invasive fungal infections (IFI) and subsequent treatment choices remain challenging for physicians in the ICU. Continuous education of healthcare providers (HCPs) represents the cornerstone requirement for starting an antifungal stewardship program (AFS). This study aimed at evaluating knowledge gaps in systemic antifungal prescribing among physicians and clinical pharmacists in a critical care setting.

Summary: This study explores the factors contributing to low contract renewal rates in insurance programs in poor countries, based on the experience of a micro-insurance health program in India. The findings suggest that poor understanding of insurance concepts and supply-side information failure are major reasons for low contract renewal. Additionally, cognitive bias under short-term framing and classical expected utility maximizers have differing impacts on contract renewal decisions.

Summary: Antitrust guidance allows participation in Medicare ACOs to meet clinical integration standards, potentially leading to price increases for independent primary care practices joining health system-led ACOs. However, these price jumps were rare and modest, with minimal impact on health systems' pricing power or market share. Closer monitoring and evaluation of ACO arrangements are recommended due to the potential for growth.

Summary: In 2017, the Medicare Shared Savings Program (MSSP) started incorporating regional spending into ACO benchmarks, favoring ACOs with lower baseline spending. Providers' responses were characterized by changes in the mix of participating ACOs and practices. The study found a shift towards providers with lower preexisting levels of spending, suggesting a selection response.
