Purpose of review This article aims to make a review of the up-to-date knowledge on anaphylaxis and outline the recent advances on pathophysiology, diagnosis, and management of anaphylaxis. Recent findings New data confirm the increase in prevalence of anaphylaxis and emphasize immunopathologic mechanisms. However, anaphylaxis is often underdiagnosed and guidelines are poorly applied, particularly in emergency departments. Summary An improvement of rapid diagnosis and treatment combined with education of population will decrease mortality and morbidity of anaphylaxis.

Summary: This systematic review assessed the effect of inhibiting nitric oxide pathways on blood pressure and survival in animal models of anaphylactic shock (AS). The results showed that inhibition of nitric oxide pathways improved survival and/or blood pressure in some experimental conditions. However, there is insufficient evidence to support the use of this approach as a definitive treatment for AS in humans.

Summary: The mechanism of anaphylactic shock and potential treatments were investigated using experiments on ovalbumin rats. The potassium channel blocker 4-aminopyridine (4-AP) was found to be effective in restoring blood pressure and improving survival rates, especially when used in combination with other drugs.

Summary: This systematic review and meta-analysis aimed to determine the association between emergency department length of stay (EDLOS) and in-hospital mortality (IHM). The analysis of multiple studies showed a significant association between EDLOS over 24 hours and IHM in ICU-admitted patients, as well as a correlation between low EDLOS and IHM in non-ICU admitted patients. This highlights the importance of minimizing prolonged stays in the emergency department and providing special attention to patients admitted after a short ED stay.

Identification of Distinct Clinical Phenotypes of Heterogeneous Mechanically Ventilated ICU Patients Using Cluster Analysis

Summary: This retrospective study aimed to predict the outcomes of ventilated ICU patients on the first day of ventilation by deriving their clinical phenotypes. Cluster analysis was performed on the eICU and MIMIC-IV cohorts to identify and validate four clinical phenotypes. These phenotypes showed heterogeneity among ICU patients and were associated with different mortality and extubation success rates. They also responded differently to ventilation strategies in terms of treatment duration, but did not have a difference in mortality.

Summary: This retrospective study aims to investigate the relationship between emergency department (ED) length of stay (EDLOS) and inhospital mortality (IHM) in elderly patients. The study found a significant correlation between EDLOS and IHM, with a higher risk of IHM in patients with shorter EDLOS and longer boarding time (BT). Specific areas and teams dedicated to older patients care could be proposed to policymakers.

This review aims to summarize the current evidence on screening tools for delirium in the context of the ED.
