Summary: Cigarette smoke exposure increases the incidence and severity of acute respiratory distress syndrome (ARDS), but the mechanisms are unclear. This study found that cigarette smoke impairs mitophagy flux and exacerbates mitochondrial damage and NLRP3 inflammasome activation, leading to increased acute lung injury. Inhibition of inflammasome or downstream component IL-1 beta attenuates the cigarette smoke-induced lung injury.

Summary: Acute lung injury (ALI)/acute respiratory distress syndrome (ARDS) is a life-threatening inflammatory lung disease, mainly caused by sepsis, infections, trauma, toxic reagents, and COVID-19. Current treatment options for ALI/ARDS are limited, but gene therapy using therapeutic nucleic acids has shown potential. Two classes of therapeutic nucleic acids, knock-in genes and oligonucleotides, have been developed for the treatment of ALI/ARDS. Delivery systems for efficient delivery of therapeutic nucleic acids into the lungs have also been explored. This review discusses ALI/ARDS gene therapy mainly in terms of delivery systems and provides insights for the development of new treatment strategies.

Summary: Acute respiratory distress syndrome (ARDS) is a life-threatening condition in critically ill patients with unclear mechanisms. Neutrophil extracellular traps (NETs) released by activated neutrophils have a critical role in inflammatory injury. This study investigated the role of NETs and the underlying mechanism in acute lung injury (ALI). The findings suggest that targeting cGAS-STING may be a potential therapeutic approach for ARDS/ALI.

Summary: The aim of this study was to investigate the lung aeration and respiratory support of neonatal bile acid pneumonia (NBAP). A case/control study was conducted on neonates with NBAP, respiratory distress syndrome (RDS) or transient tachypnoea (TTN). Lung ultrasound score, oxygenation index and SpO(2)/FiO(2) were used to assess lung aeration and oxygenation. The results showed that NBAP started as a mild disorder in the first hours of life but later worsened and became similar to RDS.

Summary: This study aimed to investigate the association between surfactant protein-D (SP-D) expression and clinical outcomes in extremely preterm neonates. The results showed that SP-D levels increased with gestational age and were inversely associated with the development of bronchopulmonary dysplasia (BPD). SP-D was also correlated with the duration of hospital stay, invasive ventilation, and total sPLA2 activity.

Summary: This study aimed to investigate the influence of clinical chorioamnionitis (CC) on lung ultrasound score (LUS) and ultrasound-guided surfactant treatment. The results showed that CC does not affect LUS in preterm neonates unless it triggers severe neonatal acute respiratory distress syndrome (NARDS). The occurrence of NARDS is the key factor influencing LUS.

Summary: Neonatal Severe Acute Respiratory Syndrome-CoronaVirus-2 infections can be acquired from the community or vertically. An analysis of hospitalized neonatal patients in the EPICENTRE worldwide registry shows that community-acquired cases have more frequent clinical features (fever, respiratory signs, feeding difficulties, P < 0.0001) and receive antibiotics more often (P = 0.014) compared to vertically-acquired cases. Severe Acute Respiratory Syndrome-CoronaVirus-2 infections should be considered during the clinical examination of neonatal infections.

Summary: The clinical features and management of post-traumatic fat embolism syndrome (FES) in a group of teenagers were described, and a systematic review of pediatric case reports was provided. FES was found to be common in adolescent trauma patients, with neurological and respiratory impairments, as well as hemodynamic instability. Prognosis was generally favorable in patients receiving prompt intensive care support.

Summary: Febrile infection-related epilepsy syndrome (FIRES) is a challenging condition with unfavorable outcome in most cases. Preliminary evidence suggests that elevated levels of certain interleukins may be related to a functional deficiency in anti-inflammatory pathways. Therefore, targeting innate immunity in treatment strategies may be effective. This case study describes a successful treatment approach using anakinra and a ketogenic diet for a patient with super-refractory status epilepticus (SE). High levels of IL-1RA in the patient's serum supported the escalation of therapy. After more than one year follow-up, the patient is seizure-free with moderate intellectual disability but good language skills. This case suggests that serum IL-1RA levels could be helpful in guiding treatment escalation and that anakinra and ketogenic diet show promise as immunomodulatory strategies.
