Acute lung injury (ALI) and acute respiratory distress syndrome (ARDS) are life-threating conditions still lacking a definite therapy and carrying a high mortality and morbidity, especially in children and infants. Albeit respiratory assistance and supportive therapies are crucial for ALI/ARDS, many drugs have been proposed to treat such syndromes through various mechanisms of action. On the whole the pharmacological therapy might play an important role in such a complex clinical situation but few evidence based data are available in pediatric and neonatal critical care. This review will focus on drugs directly available on the bedside, that is, medicines already administered in the practice or investigated in at least one clinical study. We will value the differences due to patient's age and the various causes of the syndrome, that may affect the response to the pharmacological therapy. A special attention will be given to the drugs directly deliverable into the lungs, as this strategy allows a total availability to the lung tissue. The experimental background behind each drug will be discussed and then clinical data in neonates and infants will be presented, if available. Data coming from adult critical care and thought to be somehow pertinent for the pediatric setting will otherwise be reviewed. Quality and evidence for or against each therapy will be evaluated according to the Scottish Intercollegiate Guidelines Network statement and practical reminders for clinicians will accordingly be provided.

Evolution of Ultrasound-Assessed Lung Aeration and Gas Exchange in Respiratory Distress Syndrome and Transient Tachypnea of the Neonate

Lucilla Pezza, Victor Sartorius, Barbara Loi, Giulia Regiroli, Roberta Centorrino, Lucia Lanciotti, Virgilio P. Carnielli, Daniele De Luca

Summary: This study aims to clarify the evolution of lung aeration and function in neonates with RDS and TTN. The results showed that the RDS cohort had worse lung aeration and oxygenation compared with the TTN cohort during the first 72 hours of life.

Needleless inhaled anesthesia with sevoflurane: Advantages of a simplified approach for children with spinal muscular atrophy undergoing intrathecal administration of nusinersen

Summary: This study evaluated the feasibility of needleless general anesthesia exclusively with sevoflurane for children with spinal muscular atrophy undergoing nusinersen administration. The results showed that only two cases of prolonged desaturation occurred during anesthesia, but no major complications were observed. There were no failures in the drug administration process.

Can lung ultrasound score accurately predict surfactant replacement? A systematic review and meta-analysis of diagnostic test studies

Letizia Capasso, Daniela Pacella, Fiorella Migliaro, Serena Salome, Fiorentino Grasso, Iuri Corsini, Daniele De Luca, Peter G. Davis, Francesco Raimondi

Summary: The study aimed to evaluate the accuracy of lung ultrasound score (LUS) in identifying preterm infants who require surfactant therapy. The results showed that LUS had high sensitivity, specificity, and diagnostic accuracy in predicting the need for the first surfactant dose. However, there was high heterogeneity in predicting multiple surfactant doses.

Ultrasound-assessed lung aeration, oxygenation and respiratory care in neonatal bile acid pneumonia: A nested case-control study

Giulia Regiroli, Giulia La Malfa, Barbara Loi, Alexandre Vivanti, Roberta Centorrino, Daniele De Luca

Summary: The aim of this study was to investigate the lung aeration and respiratory support of neonatal bile acid pneumonia (NBAP). A case/control study was conducted on neonates with NBAP, respiratory distress syndrome (RDS) or transient tachypnoea (TTN). Lung ultrasound score, oxygenation index and SpO(2)/FiO(2) were used to assess lung aeration and oxygenation. The results showed that NBAP started as a mild disorder in the first hours of life but later worsened and became similar to RDS.

Early life surfactant protein-D levels in bronchoalveolar lavage fluids of extremely preterm neonates

Daniele De Luca, Raquel Arroyo, Silvia Foligno, Chiara Autilio, Lhousseine Touqui, Paul S. Kingma

Summary: This study aimed to investigate the association between surfactant protein-D (SP-D) expression and clinical outcomes in extremely preterm neonates. The results showed that SP-D levels increased with gestational age and were inversely associated with the development of bronchopulmonary dysplasia (BPD). SP-D was also correlated with the duration of hospital stay, invasive ventilation, and total sPLA2 activity.

Effect of preterm chorioamnionitis on lung ultrasound score used to guide surfactant replacement

Laura Vivalda, Barbara Loi, Valeria Bisceglie, Rafik Ben-Ammar, Daniele De Luca

Summary: This study aimed to investigate the influence of clinical chorioamnionitis (CC) on lung ultrasound score (LUS) and ultrasound-guided surfactant treatment. The results showed that CC does not affect LUS in preterm neonates unless it triggers severe neonatal acute respiratory distress syndrome (NARDS). The occurrence of NARDS is the key factor influencing LUS.

Community Versus Vertically Acquired Neonatal SARS-CoV-2 Infection: The EPICENTRE Cohort Study

Daniele De Luca, Elizabeth Perkins, David G. Tingay

Summary: Neonatal Severe Acute Respiratory Syndrome-CoronaVirus-2 infections can be acquired from the community or vertically. An analysis of hospitalized neonatal patients in the EPICENTRE worldwide registry shows that community-acquired cases have more frequent clinical features (fever, respiratory signs, feeding difficulties, P < 0.0001) and receive antibiotics more often (P = 0.014) compared to vertically-acquired cases. Severe Acute Respiratory Syndrome-CoronaVirus-2 infections should be considered during the clinical examination of neonatal infections.

Fighting autoinflammation in FIRES: The role of interleukins and early immunomodulation

Marco Perulli, Gianpaolo Cicala, Ida Turrini, Elisa Musto, Michela Quintiliani, Maria Luigia Gambardella, Silvia Maria Pulitano, Sarah Bompard, Susanna Staccioli, Laura Carmillo, Gabriele Di Sante, Francesco Ria, Chiara Veredice, Ilaria Contaldo, Domenica Battaglia

Summary: Febrile infection-related epilepsy syndrome (FIRES) is a challenging condition with unfavorable outcome in most cases. Preliminary evidence suggests that elevated levels of certain interleukins may be related to a functional deficiency in anti-inflammatory pathways. Therefore, targeting innate immunity in treatment strategies may be effective. This case study describes a successful treatment approach using anakinra and a ketogenic diet for a patient with super-refractory status epilepticus (SE). High levels of IL-1RA in the patient's serum supported the escalation of therapy. After more than one year follow-up, the patient is seizure-free with moderate intellectual disability but good language skills. This case suggests that serum IL-1RA levels could be helpful in guiding treatment escalation and that anakinra and ketogenic diet show promise as immunomodulatory strategies.
