Cyclooxygenase as a Target for Colorectal Cancer Chemoprevention

Colorectal cancer (CRC) is one of the most common neoplasia in Western countries and the second leading cause of cancer-related death. The vast majority of cases belong to sporadic forms, whereas a small but relevant proportion of them corresponds to inherited disorders, i.e. familial adenomatous polyposis and Lynch syndrome. These individuals with germline mutations in cancer-promoting genes, along with those who had already developed a colorectal neoplasm, either adenoma or carcinoma, stand to benefit from chemopreventive interventions. A large body of evidence indicates that the use of aspirin and other non-steroidal anti-inflammatory drugs (NSAID) can reduce the risk of CRC. Experimental studies have demonstrated that these drugs decrease the incidence of carcinogen-induced colon tumors in rodents, and several epidemiological investigations and therapeutic trials have also shown a 40-50% reduction in the risk of colorectal adenoma and cancer in individuals taking NSAIDs. Moreover, patients with familial adenomatous polyposis taking sulindac or celecoxib experience a reduction in adenoma size and number. The chemopreventive effects of NSAID are largely related to inhibition of cyclooxygenase-2 (COX-2), the inducible isoform of cyclooxygenase that catalyzes the conversion of arachidonic acid to prostaglandins. COX-2 overexpression is a frequent, but not universal event in colorectal neoplasms. Indeed, approximately 50% of adenomas and 80% of CRC express high levels of COX-2 mRNA and protein in neoplastic tissue. In this article, we will review the role of cyclooxygenase as a target for CRC chemoprevention, with special attention to the use of selective and non-selective COX-2 inhibitors in both individuals genetically predisposed and those who have already developed a colorectal neoplasm.

Summary: This study is the first investigation analyzing polygenic risk score (PRS) in a two-step colorectal cancer screening program. The results showed that PRS could improve the accuracy of current colorectal cancer screening, especially for higher at-risk subgroups. However, the capacity of PRS is limited and should be complemented by additional biomarkers.

Summary: Serrated polyposis syndrome (SPS) is a common polyposis syndrome that increases the risk of colorectal cancer (CRC). A germline frameshift variant in the POLD1 gene was identified in an SPS patient with CRC. The variant also appeared in three other SPS-affected family members. Organoid models were generated to study the variant's impact, but no significant differences were observed compared to the control group. Further research is needed to understand the functional consequences of POLD1 alterations. This study provides the first organoid model for germline POLD1 variants, establishing a foundation for studying SPS, CRC, and other malignancies.

Summary: Gastric cancer is a global public health problem with high mortality rates. Biomarker research is crucial for early detection of gastric cancer. Technological advances have identified several potential biomarkers, but their low specificity in biofluids limits their clinical use. Recent research focuses on exploring gastric juice as an alternative source for biomarker identification, as it contains disease-specific biomarkers generated at the site of damage. This narrative review describes potential biomarkers for gastric cancer screening in gastric juice.

Summary: Surveillance methods for pancreatic cancer in high-risk individuals are unclear and have recently changed. A study found that the risk of pancreatic ductal adenocarcinoma is higher in carriers of a germline pathogenic variant in CDKN2A than previously reported. The findings support the latest surveillance recommendations for these individuals.

Summary: This study used the latest PLSD cohort to investigate the mortality rates and age of cancer diagnoses in carriers of pathogenic MMR variants. The results showed that non-colorectal Lynch syndrome cancers caused more deaths than colorectal cancers in these carriers. Reducing deaths from non-colorectal cancers is a crucial challenge for the medical care of Lynch syndrome.

Summary: The segmentation of patients into homogeneous groups can improve the effectiveness of eradication therapy. The study aims to determine the most important treatment strategies used in Europe and evaluate the effectiveness of first-line treatments based on year and country. The analysis found that the overall treatment effectiveness increased from 87% in 2013 to 93% in 2022, with the highest effectiveness achieved in 2022 using bismuth-quadruple therapy in Spain. Cluster analysis identified homogeneous treatment groups and assessed the effectiveness of different first-line treatments.

Summary: This study analyzed the effectiveness and safety of triple-dose administration of single capsule bismuth quadruple therapy (scBQT) in patients included in the European Registry on Helicobacter pylori Management. The results showed that the three times a day treatment schedule was more effective than the traditional four times a day schedule in terms of overall cure rates. There were no differences observed in treatment adherence or safety between the two regimens.
