Current Treatment and Drug Discovery Against Leishmania spp. and Plasmodium spp.: A Review

Malaria and leishmaniasis are the most prevalent tropical diseases caused by protozoan parasites. Half of world's population is at risk of malaria and more than 2 million of new cases of leishmaniasis occur annually. There are no vaccines available for these diseases and current treatments suffer from several limitations. Therefore, novel drugs for malaria and leishmaniasis are much-needed. This article reviews the agents currently in use for treatment of these diseases, their known mechanisms of action and weaknesses. We present an overview of the main strategies for drug discovery and the relevance of these parasites genomics/proteomics data for a rational search of molecular targets and matching leads. In this direction, we emphasize the importance of the highly integrated partnerships and networks between scientists in academic institutions and industry involving several countries that promise to increase the chances of success and enhance cost-effectiveness in drug discovery against these parasitic diseases. In addition, we approach the available assays for testing lead compounds in large scale and their limitations for they represent one of the bottlenecks in the pipeline for novel drug discovery. We conclude the article presenting a recent coordinated initiative (TDR Transfection Network) established to overcome some of these limitations by the generation of Plasmodium and Leishmania transgenic parasites better suited for HTS platforms.

Summary: The prodrug approach of converting lead compounds into amino AOCOM ether-substituted analogues improves aqueous solubility and oral bioavailability, meeting key requirements typical for drug candidate profiles. The prodrug is activated through a pH-triggered intramolecular cyclization-elimination reaction, independent of biotransformations and animal-assisted processes. This novel approach was successfully demonstrated on antimalarial compounds, achieving single-dose cures in a rodent malaria model without advanced formulation techniques.

Summary: This study utilizes high-resolution microscopy to characterize temporal changes of the TVN in Plasmodium vivax liver stage, revealing its presence in dormant hypnozoites and association with host nuclei. The unexpected association of the host water channel protein AQP3 with TVN-derived vesicles is demonstrated, suggesting a potential function of AQP3 during this stage. These findings enhance understanding of host-parasite interactions in both dormant and replicating P. vivax liver stage forms.

Summary: In trypanosomatids, RNA-binding proteins (RBPs) play a crucial role in gene expression regulation through the posttranscriptional modification by protein arginine methyltransferases (PRMTs). This study functionally characterized the predicted PRMTs in Leishmania braziliensis and revealed the variation of R-methylation profiles among Leishmania species and different lifecycle stages. The results suggest the existence of a PRMT-interacting protein network and the importance of arginine methylation in parasite-host cell interactions.

Differential Growth Rates and In Vitro Drug Susceptibility to Currently Used Drugs for Multiple Isolates of Naegleria fowleri

Summary: The study investigated the drug susceptibility and growth rates of 11 different clinical isolates of the amoeba Naegleria fowleri, which causes a deadly brain infection. Significant differences in drug susceptibility and growth rates were found among the isolates, highlighting the need for additional clinical isolates in drug assays and the development of more targeted therapeutics. The findings provide a baseline for drug susceptibility among clinical isolates and suggest the potential for future combination therapy and research on mechanisms of action.
