The rationale for deep brain stimulation in Alzheimer's disease

Alzheimer's disease is a major worldwide health problem with no effective therapy. Deep brain stimulation (DBS) has emerged as a useful therapy for certain movement disorders and is increasingly being investigated for treatment of other neural circuit disorders. Here we review the rationale for investigating DBS as a therapy for Alzheimer's disease. Phase I clinical trials of DBS targeting memory circuits in Alzheimer's disease patients have shown promising results in clinical assessments of cognitive function, neurophysiological tests of cortical glucose metabolism, and neuroanatomical volumetric measurements showing reduced rates of atrophy. These findings have been supported by animal studies, where electrical stimulation of multiple nodes within the memory circuit have shown neuroplasticity through stimulation-enhanced hippocampal neurogenesis and improved performance in memory tasks. The precise mechanisms by which DBS may enhance memory and cognitive functions in Alzheimer's disease patients and the degree of its clinical efficacy continue to be examined in ongoing clinical trials.

Summary: This systematic review summarizes the clinical indications, outcomes, and adverse effects of insular laser interstitial thermal therapy (LITT) through a review of the literature. The results show that LITT is effective in treating both insular seizures and insular tumors. However, the lack of prospective studies highlights the need for randomized and controlled trials to evaluate the efficacy of LITT.

Beta band oscillations in the motor thalamus are modulated by visuomotor coordination in essential tremor patients

Summary: This study investigates the role of beta oscillations in the basal ganglia and cerebellar regions in visuomotor coordination. The recordings during the visuomotor task showed a decrease in beta oscillations and an increase in firing rates. These findings suggest that beta oscillations are modulated by novel visuomotor tasks and facilitate information transmission to the thalamocortical circuit.

Hemiatrophy-hemiparkinsonism and Poland syndrome: A causative or coincidental association?

Summary: Despite no previous reports on parkinsonism in Poland syndrome (PS) and the lack of studies on treatment response, we present a case of ipsilateral parkinsonism in PS, resembling hemiatrophy-hemiparkinsonism, with a positive response to levodopa and subthalamic deep brain stimulation.

Predictors of Future Deep Brain Stimulation Surgery in de novo Parkinson's Disease

Summary: We report a case of a cervical dystonia patient who lost the ability to swim and dance during active stimulation of the globus pallidus internus. Using unique functional and structural imaging analysis, we investigated the anatomical underpinning of this phenomenon. The results showed significant stimulation-evoked responses during fMRI.

Investigation of neurophysiologic and functional connectivity changes following glioma resection using magnetoencephalography

Summary: This study used magnetoencephalography to investigate the neurophysiologic and functional connectivity changes following glioma surgery. The findings showed distinct alterations in spectral power following tumor resection, with frequency band changes unique to each patient. Recovery of regional functional connectivity was observed in the post-operative setting, with comparable changes in low to mid gamma-associated functional connectivity noted in four patients. These findings provide a framework for future studies to examine other network changes in glioma patients and correlate neurophysiologic changes with individual patients' clinical outcomes.
