Summary: Activation of a volunteer responder system in cases of out-of-hospital cardiac arrest (OHCA) was associated with higher rates of bystander CPR, bystander defibrillation, and 30-day survival compared to no system activation. A randomized controlled trial is necessary to determine the causal effect of volunteer responder systems.

Summary: In the study by Suverein et al., the use of extracorporeal cardiopulmonary resuscitation (CPR) in patients with refractory out-of-hospital cardiac arrest was examined. The results showed similar survival rates with a favorable neurologic outcome between extracorporeal CPR and conventional CPR. This raises questions about the role of extracorporeal CPR as an adjunct to conventional CPR.

Summary: This study found that replacing the no-flow time component in OHCA and CAHP models with new objective variables, such as CT, MRI, and biomarkers, can improve the accuracy and sensitivity in predicting the neurological prognosis of cardiac arrest patients. Among 106 patients, approximately 57.5% had poor neurological outcomes. By adding new variables to replace the no-flow time, the prediction of poor neurological outcomes was improved.

Summary: The study found significant regional differences in the rationale and interval until termination of resuscitation in out-of-hospital cardiac arrest cases. Cases terminated due to obvious death or do-not-resuscitate orders/verbal directives often receive similar durations of resuscitation as survivors.

Summary: The effect of gender on survival and neurological outcome varies in patients with out of hospital cardiac arrest. Women admitted for OHCA had lower survival rate and good neurological outcome compared to men. However, after adjusting for baseline characteristics, gender was not associated with survival or neurological outcome.

Summary: This study found that care at cardiac arrest centers (CACs) was associated with improved survival and neurological outcomes for patients with nontraumatic out-of-hospital cardiac arrest. The benefits were consistent regardless of varying CAC definitions. Patients with shockable rhythms and those without prehospital return of spontaneous circulation benefited more from CACs. The evidence for bypassing hospitals or interhospital transfer remains inconclusive.

Summary: The study found that volunteer citizen responders dispatched to out-of-hospital cardiac arrest reported low risk of physical injury. Monitoring and considering the risk of injury should continue to be an essential safety measure in citizen responder programs.

Summary: A randomized trial comparing hypothermia and normothermia treatment after out-of-hospital cardiac arrest showed no significant differences in death or functional outcomes at 6 months. The hypothermia group had a higher incidence of arrhythmia resulting in hemodynamic compromise compared to the normothermia group.

Summary: This study assessed interventions before ambulance arrival and survival for public out-of-hospital cardiac arrest (OHCA) patients in residential neighborhoods. The results showed that there were fewer resuscitative efforts before ambulance arrival and lower survival rates for OHCA patients in residential neighborhoods compared to nonresidential areas. Therefore, targeted efforts to improve early cardiopulmonary resuscitation (CPR) and defibrillation for public OHCA patients in residential neighborhoods are needed.

Summary: A clinical prediction rule of termination of resuscitation for out-of-hospital cardiac arrest patients with pre-hospital defibrillation was derived from a retrospective multicenter cohort study. The rule includes EMS call to ED time and ETCO2 level as independent predictors for predicting death before ED arrival. When both criteria are met, there is a high specificity and positive predictive value for predicting death before ED arrival in OHCA patients with pre-hospital defibrillation.

Summary: A randomized, double-blind, placebo-controlled trial conducted on patients with non-traumatic out-of-hospital cardiac arrest (OHCA) showed that the use of vasopressin did not significantly increase the likelihood of sustained return of spontaneous circulation in patients with diastolic blood pressure (DBP) <20 mm Hg during the early cardiac compression period.

Summary: This study compared the effectiveness of mechanical and manual CPR in Out-of-Hospital Cardiac Arrest (OHCA) patients. Mechanical chest compressors could increase the probability of return of spontaneous circulation (ROSC) and 30-day survival, with different devices having different performances. The load-distributing-band device was the only one that positively affected 30-day survival.

Summary: The study found that patients with plaque psoriasis have higher levels of anti-CA IgA and IgG in their plasma, and the cellular response to Candida albicans primarily occurs in CLA(+) T cells, particularly Th17 cells. In plaque psoriasis, the levels of anti-CA IgA are directly associated with the CLA(+) Th17 response. Proteomic analysis revealed distinct profiles in psoriasis patients with high anti-CA IgA levels.

Summary: Privacy protection is crucial in health research, and federated learning techniques allow for building machine learning models in a distributed manner to ensure data security. Studies have shown that federated models can achieve similar accuracy and generalizability as standard centralized statistical models, while providing stronger privacy protections.

Summary: This study described the safety and efficacy results of the JAK1 inhibitor INCB054707 in patients with moderate-to-severe HS. The results showed that the drug was well tolerated and demonstrated therapeutic effects in patients with moderate-to-severe HS. These findings provide proof of concept for the use of JAK1 inhibitors in the treatment of HS.

Summary: This study aimed to understand the ideal inpatient consultation and identify the reasons for common failures in consultation. The study found that successful inpatient consultation requires a series of complex and time-sensitive information exchanges, which are vulnerable to failure. Improving consultation quality will require not only reducing low-value consultations but also actively preventing common defects such as information inaccuracies and misinterpretation.
