Summary: Dexmedetomidine administration had no significant effect on 28/30-day mortality and ventilator-free days in septic patients, but it could shorten the length of ICU stay.

Summary: Recent Bayesian reanalyses of prominent trials in critical illness have generated controversy by contradicting the initial conclusions based on conventional frequentist analyses. In most cases, Bayesian and frequentist analyses agreed, but Bayesian analysis identified interventions where benefit was probable despite the absence of statistical significance. Bayesian analysis in critical care medicine can help to distinguish harm from uncertainty and establish the probability of clinically important benefit for clinicians, policy makers, and patients.

Summary: This study reveals that the absence of basophils in ICU patients is strongly associated with poor prognosis and mortality. The findings suggest that basophil-mediated immunity may serve as a potential predictor for ICU patients' prognosis, and help identify patients who would benefit from intensified treatment and immunoenhancement therapy.

Summary: In this systematic review and meta-analysis, we found that melatonin and ramelteon do not seem to reduce delirium incidence in ICU patients, although there was a potential trend towards reducing the duration of mechanical ventilation. More studies are needed to confirm these findings.

Summary: The ACCCOS study aimed to determine factors associated with mortality in critically ill African COVID-19 patients. The study found that mortality was higher in African countries compared to other regions, with factors such as age, comorbidities, organ dysfunction, and resource availability playing a role in patient outcomes.

Summary: Guillain-Barre syndrome (GBS) is an acute polyneuropathy characterized by acute flaccid paralysis, with current treatment options including immunotherapies and mechanical ventilation in the ICU setting which are largely empirical. This review emphasizes the importance of ICU care and treatment for severe GBS patients, summarizing the latest knowledge on pharmacological therapies and ICU management.

Summary: This study evaluated the diagnostic yield of FDG-PET/CT in intensive care patients with BSI, showing a high sensitivity and specificity for identifying infection foci. Poor image quality was significantly associated with decreased likelihood of finding infection foci.

Summary: Randomized controlled trials are considered rigorous research designs, but face challenges in real-world settings. Pragmatic trials are viewed as a design that overcomes these challenges. Long-term care researchers and providers need to consider stakeholder engagement, diversity and inclusion, organizational strain and readiness, and learning from adaptations in the implementation of pragmatic trial interventions.

Summary: This study investigates the ventilation in multiple-bed hospital wards in developing countries, using computational fluid dynamics simulation. Three ventilation systems, VLAF, HLAF, and TcAF, were compared in terms of airflow distribution, CO2 concentration, and temperature distribution. The results showed that TcAF had the most effective airflow distribution, but did not meet the ASHRAE standard for temperature distribution. HLAF with two exhausts was found to be the most suitable option, with low CO2 concentration and temperature compliance with the ASHRAE standard.

Summary: Clinical trials for chronic thromboembolic pulmonary hypertension (CTEPH) are increasingly focused on procedural interventions, with Japan playing a significant role. Patient registries have a higher rate of published results compared to pharmaceutical/procedural interventions. Balloon pulmonary angioplasty trials are expected to provide important results in the near future.

Summary: Randomized controlled trials are considered rigorous, but pragmatic trials are seen as a more applicable research design for real-world settings. Summary: The study evaluated the diagnostic accuracy and prognostication of monocyte distribution width (MDW) in sepsis for patients admitted to ICU. MDW values were significantly higher in patients with sepsis or septic shock compared to those without sepsis. The increase in MDW was not affected by the etiology of sepsis, including in patients with COVID-19, and it was found to decrease in sepsis survivors.
