Rapid identification and differentiation of Gram-negative and Gram-positive bacterial bloodstream infections by quantitative polymerase chain reaction in preterm infants

Objective. To evaluate the usefulness of the Gram-specific probe-based quantitative polymerase chain reaction test for rapid detection and differentiation of Gram-negative and Gram-positive bacterial bloodstream infection in preterm infants. Design: Cross-sectional study. Setting. University-affiliated Level III neonatal intensive care unit. Patients. Preterm infants with clinical features suggestive of late-onset infection. Interventions: None. Measurements and Main Results. In addition to the full sepsis screen, 0.5 mL of EDTA blood was collected aseptically for Gram-specific quantitative polymerase chain reaction evaluation. The results were analyzed with respect to outcomes of bacterial culture in blood and other body fluids, including peritoneal and cerebrospinal fluids. The diagnostic utilities of the quantitative polymerase chain reaction were determined. A total of 218 suspected infection episodes were investigated, of which 42 episodes were culture positive and 176 were culture negative. For Gram-negative infection, the quantitative polymerase chain reaction test correctly identified 19 of 22 episodes, and the sensitivity and specificity were 86.4% and 99.0%, respectively. For Gram-positive infection, the test correctly identified 14/19 episodes, and the sensitivity and specificity were 73.7% and 98.5%. The remaining one episode Was Candida albicans septicemia. None of the episodes with positive quantitative polymerase chain reaction test were classified into the wrong Gram stain category. More importantly, despite negative blood culture in five infants suffering from intra-abdominal sepsis (peritonitis [n = 4] and hepatosplenic abscess [n = 1]), the quantitative polymerase chain reaction test could detect the Gram-specific category of causative organisms in blood. Conclusions. The Gram-specific quantitative polymerase chain reaction test is reliable and highly specific for rapid identification and differentiation of Gram-negative and Gram-positive bloodstream and intra-abdominal infections. The result could be made available within 5 hrs after the specimen reaches the laboratory. A positive test-is able to rule in bacterial bloodstream infection before blood culture results become available, and serves as a guide to predict the virulence of the causative organism according to its Gram-specific category so that critical patients can be targeted for intensive treatment. Summary: Recent studies have highlighted the presence of a substantial amount of analyzable long cell-free DNA (cfDNA) in plasma through single molecule, real-time (SMRT) sequencing. In this study, the size biases of SMRT sequencing by PacBio and nanopore sequencing by ONT were evaluated. The results showed that both platforms had biases towards sequencing longer DNA fragments, with PacBio showing a stronger bias. Tissue-of-origin analysis based on single-molecule methylation patterns performed comparably on both platforms. It is important to be aware of the analytical characteristics and biases of the sequencing platforms used when analyzing the size and end motif of cfDNA.

Summary: A meeting of experts in November 2021 discussed the performance and cost-effectiveness of EBV-based approaches for screening early stage nasopharyngeal carcinoma. Both serum EBV antibody and plasma EBV DNA testing methods were found to be favorable. Additionally, the use of MRI was recommended to increase the sensitivity of NPC detection without greatly impacting the cost-effectiveness of the screening program.

Gestational age-specific neonatal mortality in Hong Kong: a population-based retrospective study

Summary: This study provides a detailed analysis of neonatal deaths in Hong Kong, showing that overall neonatal mortality remains stable but varies among different gestational weeks. The proportion of extremely preterm infants who died significantly increased, with hemorrhagic conditions being the leading cause of death. Congenital anomalies were the leading cause of death in neonates born after 27 weeks' gestation, but their cause-specific mortality has decreased, mainly attributed to trisomy 13/18 and multiple anomalies.

Comparison of the mucosal and systemic antibody responses in Covid-19 recovered patients with one dose of mRNA vaccine and unexposed subjects with three doses of mRNA vaccines

Summary: This study compared the effect of a booster vaccine in inducing mucosal and serological antibody between Covid-19 recovered patients and vaccine-only subjects. The results showed that the booster expanded the nasal antibody levels in recovered patients and improved the inhibition against the BA.1 variant and ancestral SARS-CoV-2. The nasal antibodies induced by natural infection lasted longer than those induced by vaccines, while both groups maintained high levels of plasma antibodies for at least 21 weeks after booster.

Summary: Using the WHO head circumference standards would lead to the overdiagnosis of macrocephaly and the underdiagnosis of microcephaly, except for Indians and some Asian neonates. Population-specific cut-offs or references are more appropriate for many populations. Healthcare professionals need to be educated about the limitations of the WHO head circumference standards.

Fitness of INTERGROWTH-21st birth weight standards for Chinese-ethnicity babies

Summary: This study compared the fitness of the INTERGROWTH-21st birth weight standards (INTERGROWTH21) with a local reference (FOK2003) for ethnic Chinese babies. The results showed that INTERGROWTH21 performed less well, especially in infants born <33 weeks' gestation, compared to FOK2003 in predicting complications related to small-for-gestational-age (SGA) infants. A more robust chart based on a larger sample of appropriately selected infants is needed.
