Morphological and Immunohistochemical Changes After Corneal Cross-Linking

Purpose: To present light and electron microscopic as well as immunohistochemical findings after corneal cross-linking (CXL). Methods: Six keratoconus corneas after CXL, 12 keratoconus corneas without CXL, and 7 normal corneas were examined by light microscopy, indirect immunohistochemistry using antibodies against proapoptotic BAX, antiapoptotic survivin, and BCL-2, and anti-smooth muscle actin and, in part, by transmission electron microscopy. Direct immunofluorescence with 4'6-diamidino-2-phenylindole was performed to analyze keratocytes/area in the anterior, middle, posterior, peripheral, and central corneal stroma. Results: The period between CXL and keratoplasty ranged from 5 to 30 months. All keratoconus corneas showed the typical histological changes. Increased proapoptotic BAX expression and/or antiapoptotic survivin expression were noticed in keratocytes and endothelium in 2 keratoconus specimens after CXL. Smooth muscle actin was only observed in subepithelial scar tissue of 2 keratoconus corneas without CXL. Keratoconus corneas after CXL revealed a significant reduction in keratocyte counts in the entire cornea (P = 0.003) compared with keratoconus corneas without CXL and normal corneas. This difference was because of a loss of keratocytes in the anterior (P = 0.014) and middle (P = 0.024) corneal stroma. Keratocytes in CXL corneas were reduced in the center (P = 0.028) and the periphery (P = 0.047). Conclusions: CXL in human keratoconus can cause considerable morphologic corneal changes up to 30 months postoperatively. Especially noteworthy is a long-lasting, maybe permanent, keratocyte loss in the anterior and middle corneal stroma involving the central and peripheral cornea. As long-term corneal damage after CXL is of genuine concern, particular care should be taken to perform this procedure only in accordance with investigational protocols.

Risk Factors for Keratoconus Progression in Children Compared with Young and Middle-aged Adults

Summary: The study investigated the risk factors for keratoconus progression in different age groups. The factors associated with progression were corneal radius, eye rubbing, corneal thinning, contact lens use, and corneal thickness. Further analysis revealed that anterior chamber volume and keratoconus match index were associated with progression in children, while other factors like distance from apex to the thinnest point and Vogt striae were associated with progression in young and middle-aged adults.

Accelerated Corneal Crosslinking for Treatment of Keratoconus in Children and Adolescents under 18 Years of Age

Summary: The efficacy of accelerated epithelium-off corneal crosslinking was evaluated in keratoconus patients < 18 years of age. The study showed that this treatment can stabilize KC progression even > 2 years postoperatively and leads to a deep demarcation line.

Non-invasive endothelial cell density measurement of in toto pre-stripped DMEK-roll - impact of pre- and intraoperative endothelial cell loss on postoperative midterm clinical outcome

Summary: The aim of this study was to measure the endothelial cell density (ECD) of the in toto pre-stripped endothelial Descemet membrane lamellae (EDML) and to describe the impact of pre- and intraoperative endothelial cell loss (ECL) on postoperative midterm clinical outcome. The results showed that despite a significant decrease in ECD up to 6 months postoperatively, visual acuity further improved and thickness further decreased up to 1 year postoperatively, indicating the feasibility of non-invasive ECD measurement technique.

Corneal Perforation as a Rare and Late Manifestation of Choroidal Melanoma

Summary: The purpose of this study is to report a rare and late manifestation of choroidal melanoma in the form of corneal perforation and to highlight the major histopathological findings of this unusual combined clinical presentation. The patient, a 74-year-old male, presented with corneal perforation of the right eye and absence of light perception for 6 months. Primary enucleation was performed due to the prolonged finding and reduced visual prognosis.

Culturing Limbal Epithelial Cells of Long-term Stored Corneal Donors (Organ Culture) In Vitro - A Stepwise Linear Regression Algorithm

Summary: The study assessed the impact of various factors, including donor age, storage time, and corneal ring size, on the outgrowth of human limbal epithelial cells. The results showed that lower donor age, fewer medium changes during storage, shorter medium time, and smaller corneoscleral ring size were associated with higher success rates of LEC outgrowth. A stepwise linear regression algorithm was employed to analyze the influencing parameters.

In Vitro Expression Analysis of Cytokines and ROS-Related Genes in Human Corneal Fibroblasts and Keratocytes of Healthy and Keratoconus Corneas

Summary: The purpose of this research is to investigate the expression of cytokines and ROS-related genes in stromal cells of healthy and keratoconus (KC) corneas. Expression analysis was performed for cytokines and ROS-related genes in corneal fibroblasts (HCFs/KC-HCFs) or keratocytes (Keratocytes/KC-Keratocytes) using qPCR and ELISA. The study found that the expression of most inflammatory markers was decreased in keratocytes compared to fibroblasts, and there were no differences in cytokine expression between healthy and keratoconus cells. TNF-alpha expression was increased in KC keratocytes compared to corneal fibroblasts. No differential expression of ROS-related genes was detected between healthy and diseased cells in both fibroblasts and keratocytes.

Vision-Related Quality of Life among Patients with Different Types of Age-Related Macular Degeneration

Summary: This study aimed to assess the vision-related quality of life of patients with age-related macular degeneration (AMD) and evaluate the differences according to different types of AMD. The results showed that all types of AMD had a negative effect on vision-related quality of life, but the effect was more pronounced in patients with exudative AMD.

A Comparison of Optical Biometers Used in Children for Myopia Control

Visual Impairment and Low Vision Aids-A Comparison between Children and Adults

Summary: This study aims to compare the etiology and fitting of low vision aids in visually impaired children and adolescents with that of adults. The study found that children had better visual acuity compared to adults and the most common cause of visual impairment in children was albinism while it was age-related macular degeneration in adults. Fitting magnifying visual aids significantly improved near and distance visual acuity, with optical aids being the most commonly prescribed aids.

Contrast Adaptation in Pseudophakic Patients with Macular Disorders
