Summary: This meta-analysis evaluated the efficacy and safety of laparoscopic surgery (LS) versus open surgery (OS) for colonoscopic perforation. The results showed that LS group had shorter hospital stay and postoperative fasting time, lower postoperative complication morbidity, and faster recovery. Therefore, LS is a safe and efficacious technique for colonoscopic perforation.

Summary: This study evaluated colonoscopists' second victim experience and perception discordance between colonoscopists and patients regarding colonoscopic perforation. Survey results showed that colonoscopists were more satisfied with intervention strategies that included sufficient explanation, empathy, courteous listening, and monetary compensation. Significant patient-colonoscopist discordance was found in areas such as judgment of medical error, health professionals' apology, monetary compensation, and criminal penalties for colonoscopists in the context of colonoscopic perforation.

Summary: This study investigated the prognostic factors associated with percutaneous endoscopic gastrostomy (PEG). It found that sarcopenia is negatively correlated with the prognosis after PEG. Among the 127 patients studied, approximately half (56%) had sarcopenia, and the study showed that patients with sarcopenia had shorter median survival time after PEG compared to those without. Therefore, sarcopenia is a predictive indicator of poor prognosis after PEG.

Summary: This study evaluated the clinical impact of quality improvements in endoscopic vacuum therapy (EVT) management on upper gastrointestinal (UGI) tract leakage patients. The results showed that the quality improvements in EVT management led to decreased composite score MTL30, increased efficacy, reduced need for additional procedures and reoperations, shortened duration of leakage therapy and length-of-stay, decreased morbidity, and improved nutrition status at discharge.

Factors such as diagnostic colonoscopy, abdominal rebound tenderness, sigmoid colon location, delayed recognition, and abdominal tenderness were associated with poor clinical outcomes.

The overall healing rates for simple and complex fistulas were 81.1% and 69.0% respectively, with significant differences observed between different surgical techniques. Technology-assisted procedures showed a trend towards increased adoption for both simple and complex fistula cases.

Key findings include the recommendation of at least 3 to 5 years of practice in proctology before starting teleconsultations, and the identification of specific conditions where teleconsultation is deemed appropriate, such as pilonidal disease. Further standardization of infrastructures, logistics, and legality in telemedicine remains to be elucidated.
