A Double-Blind Randomized Controlled Trial To Study the Efficacy of Topiramate in a Civilian Sample of PTSD

Objective: To evaluate the efficacy and tolerability of topiramate in patients with post-traumatic stress disorder (PTSD). Method: We conducted a 12-week double-blind, randomized, placebo-controlled study comparing topiramate to placebo. Men and women aged 18-62 years with diagnosis of PTSD according to DSM-IV were recruited from the outpatient clinic of the violence program of Federal University of Sao Paulo Hospital (Prove-UNIFESP), Sao Paulo City, between April 2006 and December 2009. Subjects were assessed for the Clinician-Administered Posttraumatic Stress Scale (CAPS), Clinical Global Impression, and Beck Depression Inventory (BDI). After 1-week period of washout, 35 patients were randomized to either group. The primary outcome measure was the CAPS total score changes from baseline to the endpoint. Results: 82.35% of patients in the topiramate group exhibited improvements in PTSD symptoms. The efficacy analysis demonstrated that patients in the topiramate group exhibited significant improvements in reexperiencing symptoms: flashbacks, intrusive memories, and nightmares of the trauma (CAPS-B; P = 0.04) and in avoidance/numbing symptoms associated with the trauma, social isolation, and emotional numbing (CAPS-C; P = 0.0001). Furthermore, the experimental group demonstrated a significant difference in decrease in CAPS total score (topiramate -57.78; placebo -32.41; P = 0.0076). Mean topiramate dose was 102.94 mg/d. Topiramate was generally well tolerated. Conclusion: Topiramate was effective in improving reexperiencing and avoidance/numbing symptom clusters in patients with PTSD. This study supports the use of anti-convulsants for the improvement of symptoms of PTSD.

Bullying as a Risk Factor for Eating Disorder Behaviors Among Students: Secondary Analysis for a Cluster Randomized Controlled Trial

Summary: This study investigates the association between being a victim of bullying and body dissatisfaction as well as eating disorder behavior (EDB) among Brazilian middle school students. The results suggest that being a victim of bullying predicts a higher likelihood of engaging in EDB, independent of exposure to intervention programs. However, bullying itself is not directly associated with body dissatisfaction. The study highlights the importance of addressing bullying in school environments.

Replication of a predictive model for youth ADHD in an independent sample from a developing country

Peri-Traumatic Dissociation and Tonic Immobility as Severity Predictors of Posttraumatic Stress Disorder After Rape

Summary: This study investigated the association between tonic immobility (TI) and peri-traumatic dissociation (PTD) with posttraumatic stress disorder (PTSD) in women who experienced sexual violence. The results showed a significant positive correlation between PTD and PTSD severity, as well as anxiety and depressive symptoms. Risk factors for PTD occurrence included history of physical and sexual abuse during childhood, lower education, and lower income. TI was not associated with PTSD severity.

This study highlights the potential of typical developing models and emphasizes the important role of the cerebellum in mental health, cognition, and education.

Impact of adverse childhood experiences on educational achievements in young people at clinical high risk of developing psychosis

Summary: Research shows that adverse childhood experiences (ACE) can impact educational achievements in individuals at clinical high risk of psychosis (CHR). Specifically, CHR individuals with ACE had lower education levels, employment rates, and estimated IQ compared to healthy controls. Early intervention programs for psychosis should integrate interventions to support the educational and vocational recovery of young CHR individuals, and public health and social interventions to prevent or reduce the impact of ACE are recommended.

Speech Illusions in People at Clinical High Risk for Psychosis Linked to Clinical Outcome

Summary: The study found that individuals at clinical high risk for psychosis have a lower sensitivity to hearing speech in noise and uncertainty about the affective valence of this speech, which is associated with adverse clinical outcomes. The findings suggest that the ability to hear speech illusions in noise could be used as a cognitive marker to predict the likelihood of remission or transition to psychosis in individuals at risk.

Digital mental health interventions for school teachers - A narrative review

Summary: This article aims to synthesize the evidence for digital mental health interventions for school teachers. A literature search retrieved 11 studies that fulfilled the inclusion criteria. Although these interventions had some impact on the mental health of school teachers, there were limitations in study designs and data quality, as well as a need for effective evidence-based interventions.

Polygenic risk score for attention-deficit/hyperactivity disorder and brain functional networks segregation in a community-based sample

Summary: Neuroimaging studies suggest that brain development mechanisms may explain some ADHD symptoms. However, the mechanisms by which genetic factors influence brain development in relation to ADHD remain largely unknown. This study aimed to investigate the associations between an ADHD polygenic risk score and brain network functioning. The findings suggest correlations between the ADHD polygenic risk score and the segregation of cingulo-opercular networks and the default mode network, supporting the role of attentional networks and DMN in attention processes. However, these associations were not found at follow-up.

The convergence between the Pacific Alliance and Mercosur: Advances, stagnation, and contemporary challenges

Summary: This article analyzes the relationship dynamics between the Pacific Alliance and Mercosur from 2011 to 2022, focusing on their moments of advancement and retraction. Based on data analysis and literature review, it suggests that the initial competition between the two regional processes has been overcome by efforts to manage their differences, leading to ongoing dialogue. However, the challenges of Latin American integration and ideological differences among national governments hinder the establishment of common trade policies, requiring cooperation beyond tariff elimination. Restoring the convergence agenda will depend on political willingness and engagement from economic elites.

Summary: Methods to prospectively identify individuals at high risk of developing psychosis have been proposed, and this study translated and adapted two screening tools (PQ-16 and PRIME-Screen) into Brazilian Portuguese. After translation and cross-cultural adaptation, these new instruments can be used to screen Brazilian Portuguese-speaking patients who are at risk of psychosis in primary care.
