The prognostic factors of systemic sclerosis for survival among Koreans

Systemic sclerosis (SSc) is a systemic autoimmune disease characterized by fibrosis of the skin and internal organs, which can cause significant morbidity and mortality. The prognostic factors for survival were not fully evaluated in Asian population. We investigated the prognostic factors for survival of SSc among Korean patients. A total of 243 SSc patients were enrolled from Seoul National University Hospital between 1972 and 2007. Age at onset, gender, cutaneous subset, autoantibody status, major organ involvement, and occurrence of malignancy were evaluated with all-cause mortality as the end point. A multivariate Cox proportional hazard model was used to retrieve the prognostic factors for survival. During the follow-up of 1,967 person-years, 33 patients died. Old age at onset (hazard ratio [HR] 7.4, 95% confidence interval [95% CI] 1.9-28.1), diffuse cutaneous subset (HR 2.5, 95% CI 1.1-5.9), presence of anti-Scl-70 antibody (HR 3.0, 95% CI 1.2-7.1), forced vital capacity less than 70% (HR 2.8, 95% CI 1.3-6.2), and heart involvement (HR 4.2, 95% CI 1.7-10.2) were found to be significant risk factors for mortality in multivariate analysis. In Korean SSc patients, old age, diffuse cutaneous involvement, anti-Scl-70 antibody, and internal organ involvement are risk factors for mortality.

Utility of the breath-holding test in patients with systemic sclerosis

Summary: This study evaluated the clinical utility and reliability of the breath-holding test (BHT) in assessing cardiopulmonary function in patients with SSc. The results showed that BHT had significant correlations with the Borg dyspnea scale, SHAQ, and pulmonary parameters, suggesting that it may be a useful surrogate marker of pulmonary capacity in SSc patients.

Angiographic characteristics of vasculopathy in patients with idiopathic inflammatory myopathies and systemic sclerosis

Risk of Bloodstream Infection in Patients with Systemic Lupus Erythematosus Exposed to Prolonged Medium-to-High-Dose Glucocorticoids

Summary: This study aimed to investigate the incidence rate and risk factors of bloodstream infection (BSI) in patients with systemic lupus erythematosus (SLE) exposed to medium to high doses of glucocorticoids. The results showed that the 1-year incidence rate of BSI was significantly higher in SLE patients receiving prolonged medium-to-high-dose glucocorticoids compared to the general SLE population. Severe disease activity and high-dose glucocorticoid treatment were identified as risk factors for BSI.

Quantitative interstitial lung disease scores in idiopathic inflammatory myopathies: longitudinal changes and clinical implications

Summary: This study investigated the longitudinal changes and clinical significance of computer-aided quantitative scores from high-resolution CT (HRCT) images in patients with idiopathic inflammatory myopathies-related interstitial lung disease (IIMs-ILD). The results showed that QILD scores were related to lung function, ILD progression, and prognosis, with UIP patterns being associated with a higher risk of QILD progression. These findings suggest that QILD scores may serve as potential imaging biomarkers in IIMs-ILD.

Performance of cut-offs adjusted with positive control band intensity in line-blot assays for myositis-specific antibodies

Summary: The study investigated the diagnostic performance of band intensity (BI) cut-offs adjusted by a positive control band (PCB) in a line-blot assay (LBA) for myositis-related autoantibodies (MRAs). Sera from 153 idiopathic inflammatory myositis (IIM) patients and 79 healthy controls were tested. The study found that adjusting BI using PCB improved the diagnostic performance of the LBA, and an aBI cut-off value of > 20 was optimal for IIM diagnosis using the EUROLINE LBA panel.

Summary: The choice of first-line biologic or targeted synthetic drug treatment in seropositive rheumatoid arthritis (RA) patients can affect the risk of herpes zoster (HZ). Tofacitinib carries a higher risk of HZ compared to other drugs, while infliximab and adalimumab also increase the overall HZ risk. A history of HZ is also an independent risk factor for HZ in RA patients receiving DMARD treatment.
