Severe autonomic failure (SAF) refers to combined abnormalities in reflex and tonic autonomic function. SAF indicates increased risk of death in post-infarction and heart failure patients, but has not been studied in aortic stenosis (AS). Here, we investigated SAF in patients with AS and tested its correlation with hemodynamic and biochemical markers. We prospectively enrolled 174 patients with moderate to severe AS in sinus rhythm (age 76 +/- A 9 years; mean aortic valve area 0.9 +/- A 0.3 cm(2)). Heart rate turbulence (as marker of autonomic reflex activity) and deceleration capacity (as marker of autonomic tonic activity) were calculated from 24-h Holter recordings. According to the previously published technology, SAF was considered present if both factors were abnormal. 44 (25.3%) of the 174 patients had signs of SAF. Patients with SAF had lower left ventricular ejection fraction (LVEF: 48.1 vs. 54.8%; p = 0.002), lower mean aortic gradients (28 vs. 34 mmHg, p = 0.019), higher systolic pulmonary artery pressures (46.8 vs. 40.9 mmHg, p = 0.028), higher levels of brain natriuretic peptide (905 vs. 407 ng/l; p = 0.003) and higher levels of high sensitive troponin I (0.65 vs. 0.24 mu g/l; p = 0.013). Impaired LVEF (a parts per thousand currency sign50%) was the only independent factor associated with SAF, but only explained autonomic abnormalities in less than half of the patients. In patients with moderate to severe AS prevalence of SAF is high. SAF correlates with hemodynamic and biochemical markers indicating increased risk. Future studies should evaluate the prognostic value of SAF in patients with AS.

Summary: Through a post hoc analysis of the DANISH study, researchers found that periodic repolarization dynamics can help identify patients with nonischemic cardiomyopathy who will benefit from prophylactic ICD implantation. PRD was significantly associated with mortality, with patients having higher PRD values experiencing greater mortality reduction after ICD implantation.

Analysis of Fibrotic Plaques in Angiographic Manifest Cardiac Allograft Vasculopathy in Long-term Heart Transplanted Patients Using Optical Coherence Tomography

Summary: This study investigated the role of intracoronary imaging in heart transplant patients with cardiac allograft vasculopathy (CAV). The findings suggest that the presence of fibrotic, lipid, and calcified plaques, as well as the severity of stenosis, are associated with CAV(anglo). High-resolution intracoronary optical coherence tomography (OCT) could potentially improve secondary prevention and outcomes in these patients.

Comparing efficacy and safety in catheter ablation strategies for atrial fibrillation: a network meta-analysis

Summary: This network meta-analysis compared the efficacy and safety of different catheter ablation strategies for atrial fibrillation. The results showed that combining additional ablation strategies with pulmonary vein isolation (PVI) significantly reduced the risk of recurrence compared to PVI alone, with no significant differences in safety.

TAVI in patients with low-flow low-gradient aortic stenosis-short-term and long-term outcomes

Summary: This study aimed to characterize different groups of low-flow low-gradient aortic stenosis and determine their outcomes after transcatheter aortic valve implantation (TAVI). The study found that while symptoms improved in all subsets of low-flow low-gradient patients after TAVI, these patients still had a higher 3-year mortality rate compared to high-gradient patients.

Adult T-cells impair neonatal cardiac regeneration

Theresa Dolejsi, Murilo Delgobo, Thomas Schuetz, Luigi Tortola, Katrin G. Heinze, Ulrich Hofmann, Stefan Frantz, Axel Bauer, Frank Ruschitzka, Josef M. Penninger, Gustavo Campos Ramos, Bernhard J. Haubner

Summary: Newborn mice and humans have temporary cardiac regenerative potential that declines rapidly as they age. Transfer of adult T-cells into neonatal mice after myocardial infarction leads to impaired cardiac regeneration, increased fibrosis, and dysfunction, demonstrating a trade-off between regenerative potential and T-cell development.

TAVI for patients with normal-flow low-gradient compared to high-gradient aortic stenosis*

Julius Steffen, David Andreae, Michael Nabauer, Nikolas Reissig, Philipp M. Doldi, Magda Haum, Martin Orban, Hans Theiss, Konstantinos Rizas, Daniel Braun, Sven Peterss, Joerg Hausleiter, Steffen Massberg, Simon Deseive

Summary: In this study, the researchers found that patients with normal-flow low-gradient aortic stenosis (NFLG-AS) are heterogeneous, with a subgroup resembling patients with high-gradient aortic stenosis (HG-AS) in characteristics and outcomes. Lower gradient NFLG-AS patients have increased long-term mortality, indicating that the use of transcatheter aortic valve replacement (TAVI) for these patients requires careful consideration.

CTA-determined tricuspid annular dilatation is associated with persistence of tricuspid regurgitation after transcatheter aortic valve replacement

Summary: The aim of this study was to analyze the predictive value of CTA-determined tricuspid annular dilatation (TAD) on the persistence of tricuspid regurgitation (TR) in patients undergoing transcatheter aortic valve replacement (TAVR) for severe aortic stenosis (AS) and concomitant at least moderate TR. The study found that TAD is an independent predictor of TR persistence and is associated with increased 2-year all-cause mortality.

Summary: This study compared the de-escalation strategy with standard dual-antiplatelet therapy in ACS patients using individual patient data meta-analysis. The de-escalation strategy was associated with decreased ischaemic and bleeding endpoints, with unguided de-escalation showing a greater reduction in bleeding risk.
