Summary: This study investigated the prevalence, incidence, and predictors of familial dilated cardiomyopathy (FDC) in relatives participating in family screening. The results showed that 22% of relatives were diagnosed with FDC at baseline, and genetic sequencing identified the etiology in 37% of families screened. During follow-up, 2.0% developed FDC per person-year. Baseline screening helped identify relatives at low risk of developing FDC, reducing the need for follow-up.

Summary: The study aimed to assess the prognostic value of plasma catestatin concentration in patients with heart failure with reduced ejection fraction (HFrEF) in predicting unplanned hospitalization and all-cause death at a 2-year follow-up. The study included 122 hospitalized Caucasian patients in NYHA classes II to IV. Patients who died during the follow-up period had higher levels of CST, NT-proBNP, troponin T, creatinine, and glucose, and lower TAPSE, hemoglobin concentration, hematocrit, and platelet count compared to survivors. Plasma catestatin concentration was positively associated with NYHA class and blood NT-proBNP concentration. Higher plasma catestatin concentration increased the risk of all-cause death by more than five times. Plasma CST concentration is a valuable prognostic parameter in predicting death from all causes and unplanned hospitalization in patients with HFrEF.

A study analyzed the maximum lactate concentration on day 1 of 10,724 septic patients and categorized them into three groups based on lactate levels. Findings showed that severe hyperlactatemia was linked to high mortality rates, especially if it persists beyond 24 hours. The results suggest that severe hyperlactatemia, along with clinical parameters, could be used to determine treatment limits.

Summary: Early after ST-segment elevation myocardial infarction (STEMI), LV function analysis may be affected by initial LV reshaping and hypokinesia, as well as concomitant microvascular dysfunction. This study compared left ventricular ejection fraction (LVEF) and stroke volume (SV) using different imaging modalities in the early stages of STEMI. The results showed that 2D analysis methods of LVEF yielded consistent results using cineventriculography (CVG), 2-dimensional echocardiography (2DE), and 2D cardiovascular magnetic resonance (CMR) within 24 hours and 5 days after STEMI. SV measurements differed significantly between imaging techniques due to variations in volumetric measurements.

Summary: This study evaluated the impact of ACE inhibitors and angiotensin receptor blockers (ACE-I/ARB) on elderly patients with moderate-to-severe mitral regurgitation (MR) and mildly reduced to preserved ejection fraction. The results showed that patients treated with ACEI/ARB had a lower risk for the combined endpoint of death and heart failure-related readmission, even after adjustment for risk scores and frailty. Therefore, ACE-I/ARB might be a valuable therapeutic option for conservatively treated patients.
