Summary: This study investigated the muscle activation of patients with lumbar disc herniation (LDH) during walking using surface electromyography (SEMG) and established a diagnostic model based on SEMG parameters using the random forest (RF) algorithm for localizing the compressed nerve root in LDH patients. By collecting SEMG data from the tibialis anterior (TA) and lateral gastrocnemius (LG) of 58 LDH patients and 30 healthy subjects during walking, the researchers analyzed several SEMG parameters and established the RF model for diagnosis. The results showed that the RF model had an average accuracy of 84% and an area under the ROC curve of 0.93, with the RMS peak time of TA identified as the most important SEMG parameter. These findings indicate that the RF model can assist in the localization diagnosis of compressed nerve roots in LDH patients and provide references for optimizing the diagnosis model in the future.

Summary: This study visualized the morphology of lumbar herniated discs and Kambin's triangle using 3D CT/MRI fusion images. The measurements of the safe usage of transforaminal endoscopic approach and 3D lumbar nerve root sleeve angulation were evaluated. The results showed that the 3DNRA became smaller from L2 to S1, and the herniated discs were clearly visualized in the fusion image. The study concludes that this new imaging technique is important for the safety of endoscopic lumbar disc surgery.
