Syndesmotic Stabilization in Pronation External Rotation Ankle Fractures

Boden et al. suggested syndesmosis fixation was not necessary in distal pronation external rotation (PER) ankle fractures if rigid bimalleolar fracture fixation is achieved and was not necessary with deltoid ligament injury if the fibular fracture is no higher than 4.5 cm of the tibiotalar joint. We asked whether height of the fibular fracture with or without medial stability predicted syndesmotic instability as compared with intraoperative hook testing in these fractures. We reviewed 62 patients (35 male, 27 female) with a mean age of 45.6 years (range, 19 80 years). Using a bone hook applied to the distal fibula with lateral force to the distal fibula in the coronal plane, we fluoroscopically assessed the degree of syndesmosis diastasis in all patients. The mean height of the fibular fracture in patients with a positive hook test was higher than in patients with a negative hook test (54.2 mm; standard deviation [SD], 29.3 versus 34.8 mm; SD, 21.4, respectively). The height of the fibular fracture showed a positive predictive value of 0.93 and a negative predictive value of 0.53 in predicting syndesmotic instability; specificity of the criteria of Boden et al. was high (0.96). However, sensitivity was low (0.39) using the hook test as the gold standard. The criteria of Boden et al. may be helpful in planning, but may have some limitations as a predictor of syndesmotic instability in distal PER ankle fractures.

Summary: The ten-year outcomes of seleXys TH+ cup with Ceramys CoC bearing showed an unacceptably high rate of aseptic loosening, but stabilized over time. The stabilization of early revision rates may be attributed to a problem with osseointegration during the transition from primary to definitive stability.

The influence of hydroxyapatite coating on continuous migration of a Zweymuller-type hip stem: a double-blinded randomised RSA trial with 5-year follow-up

Summary: This study compared the migration characteristics of hydroxyapatite coated (HA+) and uncoated (HA-) Zweymuller-type hip stems through a double-blinded randomised RSA trial with a 5-year follow-up. The results showed that the addition of a hydroxyapatite coating did not influence the migration of the hip stem after 5 years, and there were no significant differences in clinical outcomes between the two groups.

Summary: This study analyzed data from the Dutch Arthroplasty Register and found that ceramic-on-ceramic (CoC) bearings had a higher 2-year cup revision rate compared to ceramic-on-polyethylene (CoPE) bearings in press-fit total hip arthroplasty. The main reason for revision in CoC bearings was cup loosening.

Limited knee extension during gait after total knee arthroplasty is related to a low Oxford Knee Score

Summary: There was a significant relationship between the difference in knee extension angle during gait between patients and controls with the Oxford Knee Score (OKS), where an increase of 1 degree in difference led to a reduction of 1.0 to 1.6 points in OKS. Patients overall exhibited less knee extension compared to controls, while ankle and hip gait parameters and joint moments did not show any correlation with OKS.

Summary: This study aimed to investigate whether consulting an occupational medicine specialist (OMS) within 3 months after total knee arthroplasty (TKA) surgery can lead to an earlier return to work (RTW) for patients. The results showed that consulting an OMS did not result in a decrease in time to RTW for TKA patients. However, TKA patients with high expectations returned to work earlier without consulting an OMS.

Summary: The purpose of this study was to assess the failure rate, revision rate, and functional outcomes of treating acute ACL rupture with the Ligamys device. It was found that the failure rate of the device was 30.2%, which did not meet the non-inferiority criterion compared to ACL reconstruction. However, good functional outcomes were observed in patients without the need for secondary reconstructive surgery, even in the presence of anteroposterior knee laxity.
