Is an Impacted Morselized Graft in a Cage an Alternative for Reconstructing Segmental Diaphyseal Defects?

Large diaphyseal bone defects often are reconstructed with large structural allografts but these are prone to major complications. We therefore asked whether impacted morselized bone graft could be an alternative for a massive structural graft in reconstructing large diaphyseal bone defects. Defects in the femora of goats were reconstructed using a cage filled with firmly impacted morselized allograft or with a structural cortical autograft (n = 6 in both groups). All reconstructions were stabilized with an intramedullary nail. The goats were allowed full weightbearing. In all reconstructions, the grafts united radiographically. Mechanical torsion strength of the femur with the cage and structural cortical graft reconstructions were 66.6% and 60.3%, respectively, as compared with the contralateral femurs after 6 months. Histologically, the impacted morselized graft was replaced completely by new viable bone. In the structural graft group, a mixture of new and necrotic bone was present. Incorporation of the impacted graft into new viable bone suggests this type of reconstruction may be safer than reconstruction with a structural graft in which creeping substitution results in a mixture of viable and necrotic bone that can fracture. The data suggest that a cage filled with a loaded morselized graft could be an alternative for the massive cortical graft in reconstruction of large diaphyseal defects in an animal model.

Computational tibial bone remodeling over a population after total knee arthroplasty: A comparative study

Summary: This study validated postoperative bone response using finite element analysis and strain-adaptive remodeling theory in total knee arthroplasty. A FE study on the effect of interference fit and coefficient of friction on the micromotions and interface gaps of a cementless PEEK femoral component

Summary: This computational study investigates the sensitivity of micromotions and interface gaps of a cementless PEEK femoral component to the interference fit and coefficient of friction. The study found that, besides the implant material, the micromotions and interface gaps of the implant were most sensitive to the interference fit. Compared to the cobalt-chrome femoral component, the PEEK femoral component generated higher micromotions and interface gaps when equal interference fit and friction values were applied.

Knee Bone and Cartilage Segmentation Based on a 3D Deep Neural Network Using Adversarial Loss for Prior Shape Constraint

Have Surgery and Implant Modifications Been Associated With Reduction in Soft Tissue Complications in Transfemoral Bone-anchored Prostheses?

Summary: This study compared two groups of patients with transfemoral amputation treated with different surgical techniques and implant designs, and found that changes to treatment can reduce the incidence of soft tissue infections and stoma-related complications. However, the use of a shallower stoma and titanium implant was associated with an increased risk of surgical site infections before stoma creation. Serious complications such as deep implant infection were rare in both groups during the 2-year follow-up period.

Oxygen consumption and gait dynamics in transfemoral bone-anchored prosthesis users compared to socket-prosthesis users: A cross-sectional study

Summary: Comparative analysis between satisfied transfemoral socket-suspended prosthesis (SSP) users, satisfied transfemoral bone-anchored prosthesis (BAP) users, and able-bodied individuals (AB) revealed that SSP users had higher oxygen consumption and trunk dynamics compared to AB during walking. BAP users had intermediate results between SSP users and AB, but no significant differences were found. Greater trunk excursions were associated with higher oxygen consumption, while a higher degree of pistoning in SSP users was related to larger trunk dynamics. These findings suggest that evaluating CoM and trunk dynamics, as well as pistoning, could help determine the suitability of BAP for SSP users and potentially benefit a larger population.

Reproducibility in modeling and simulation of the knee: Academic, industry, and regulatory perspectives

The effect of periprosthetic bone loss on the failure risk of tibial total knee arthroplasty

Summary: This study used finite element analysis (FEA) to simulate the effects of postoperative bone density changes on the failure of tibial total knee arthroplasty (TKA) due to bone resorption. The results showed that postoperative bone loss significantly increased the risk of TKA loosening, especially in patients with poor bone quality. Additionally, the study examined the probability of medial or lateral subsidence of the implant and aimed to improve clinical implications.

Pre-Planning the Surgical Target for Optimal Implant Positioning in Robotic-Assisted Total Knee Arthroplasty

Summary: Robotic-assisted total knee arthroplasty can achieve accurate implantation. This study demonstrates the feasibility of reproducing pre-diseased kinematics and ligament strains to optimize the position of knee components. By using a patient-specific musculoskeletal model and an optimization algorithm, the deviations between pre-diseased and post-operative kinematics and ligament strains were significantly reduced, providing valuable information for the pre-planning of robotic-assisted surgery.

An anatomically shaped medial meniscus prosthesis is able to partially restore the contact mechanics of the meniscectomized knee joint

Summary: The study aimed to investigate whether a flexible medial meniscus prosthesis is capable of sharing loads with the direct tibiofemoral cartilage contact and its effect on tibial pressure distribution after total meniscectomy. The second-generation prosthesis was found to share loads with the direct tibiofemoral contact and significantly reduce pressure on the medial tibial cartilage. These findings suggest that the prosthesis has the potential to relieve post-meniscectomy pain symptoms.
