Intestinal failure in adults: Recommendations from the ESPEN expert groups

Background & aims: Intestinal failure (IF) is defined as the reduction of gut function below the minimum necessary for the absorption of macronutrients and/or water and electrolytes, such that intravenous supplementation is required to maintain health and/or growth. Functionally, it may be classified as type I acute intestinal failure (AIF), type II prolonged AIF and type III chronic intestinal failure (CIF) The ESPEN Workshop on IF was held in Bologna, Italy, on 15-16 October 2017 and the aims of this document were to highlight the current state of the art and future directions for research in IF. Methods: This paper represents the opinion of experts in the field, based on current evidence. It is not a formal review, but encompasses the current evidence, with emphasis on epidemiology, classification, diagnosis and management. Results: IF is the rarest form of organ failure and can result from a variety of conditions that affect gastrointestinal anatomy and function adversely. Assessment, diagnosis, and short and long-term management involves a multidisciplinary team with diverse expertise in the field that aims to reduce complications, increase life expectancy and improve quality of life in patients. Conclusions: Both AIF and CIF are relatively rare conditions and most of the published work presents evidence from small, single-centre studies. Much remains to be investigated to improve the diagnosis and management of IF and future studies should rely on multidisciplinary, multicentre and multinational collaborations that gather data from large cohorts of patients. Emphasis should also be placed on partnership with patients, carers and government agencies in order to improve the quality of research that focuses on patient-centred outcomes that will help to improve both outcomes and quality of life in patients with this devastating condition. The need for biomarkers to determine response to enteral nutrition during and after critical illness: an update

Summary: This review summarizes the experimental and clinical studies assessing biomarkers proposed for prognosis or determining response to enteral nutrition. Several pathophysiological mechanisms have been identified that could provide biomarkers for determining response to enteral nutrition. Although several biomarkers have been studied, most studies are insufficient and none have been implemented in clinical practice. Studies mainly focus on simple associations between biomarkers and outcomes, lacking emphasis on dynamic changes in biomarker levels. Furthermore, research into the pathophysiology and clinical features of gastrointestinal dysfunction, including enteral feeding intolerance, is necessary to explore potential mechanisms for specific biomarkers.

Nascent to novel methods to evaluate malnutrition and frailty in the surgical patient

Summary: Preoperative nutrition assessment and intervention are crucial for improving postoperative outcomes. The Global Leadership Initiative for Malnutrition (GLIM) criteria are increasingly used in surgical settings to diagnose malnutrition, and novel approaches to assessing muscle mass can identify at-risk patients. Screening and assessing frailty can determine the cost/benefit of surgery and the value of interventions. Evaluation of nutrition risk and status is essential for predicting and mitigating surgical outcomes.

Association between prealbumin, all-cause mortality, and response to nutrition treatment in patients at nutrition risk. Secondary analysis of a randomized controlled trial

Summary: In a post hoc analysis of a nutrition trial, low prealbumin levels were found to be associated with higher all-cause 180-day mortality rates. Individualized nutrition support was more effective than usual-care nutrition in improving survival at 30 days in patients with low prealbumin levels. These findings highlight the importance of prealbumin levels as a biomarker for assessing nutrition risk.

Digital Informed Consent: Modernising Information Sharing in Surgery to Empower Patients

Summary: This study aimed to assess the feasibility of a digital information-sharing platform to support a digital informed consent process. The platform provided patients with multimedia information on gallstones and treatment options, and allowed them to record their medical history and ask questions. Despite some drop-outs due to difficulties in accessing the platform, patients welcomed the opportunity to receive information digitally before their consultation and felt empowered to be involved in decision-making.

Summary: This study investigated the incidence and severity of COVID-19 infection in patients receiving home parenteral nutrition (HPN) for chronic intestinal failure (CIF) from a European database. The results showed a significant variation in infection rates among countries, with the majority of cases being asymptomatic or mild but a significant proportion of infected patients experiencing severe or fatal outcomes. Lack of vaccination was associated with a higher risk of death.
