Summary: Chronic intestinal failure, a rare complication of Crohn's disease, can greatly impact quality of life and treatment options include home-based parenteral nutrition and intravenous infusion. However, 60% of patients may need lifelong reliance on parenteral nutrition, and promising new drugs have brought new dimensions to therapy.

Summary: This real-life study aimed to evaluate the efficacy and outcomes of Teduglutide in the treatment of short-bowel-syndrome (SBS)-associated chronic intestinal failure (CIF). Among the 331 SBS-CIF patients included in the study, 56 patients (16.9% of the cohort) were treated with Teduglutide, resulting in a 60% reduction in parenteral support (PS) volume, and a treatment retention rate of over 80% at 2 years.

Summary: This study compares outcomes and surgical decision-making in patients with short bowel syndrome (SBS) and fistulas. The authors found that patients who underwent reconstructive surgery had higher rates of nutritional autonomy, while older patients with higher comorbidity index were less likely to achieve cessation of home parenteral nutrition (HPN). There were no differences in long-term nutritional and survival outcomes or surgical decisions between patients with SBS and fistulas, or between enterocutaneous and enteroatmospheric fistulas.

Summary: This paper discusses the nutritional strategies available for clinicians caring for pediatric patients with short bowel syndrome. The research found that there is currently no consensus on the best way to feed these patients, and there are significant variations in practice between different centers. Therefore, more research is needed to establish common treatment protocols.

Summary: Short bowel syndrome requires different management strategies, and long-term use of parenteral nutrition can help regain bowel function. However, some patients may still require more aggressive treatment.

Summary: Short gut syndrome and parenteral nutrition have negative effects on kidney function, particularly in terms of secondary hyperoxaluria and glomerular filtration. Individualized dietary intervention and a balanced multidisciplinary team are crucial for maintaining nutrition, preventing renal stone disease, and improving overall outcomes.

Summary: This study investigated biomarkers for the regulation mechanisms in the intestinal barrier during adaptive response or treatment with the glucagon-like peptide-2 analog in short bowel syndrome patients. Gene expression analyses and intestinal permeability testing were conducted. The results showed altered gene expression in nutrient transport genes and the usefulness of mannitol recovery in describing gut absorptive capacity.

Summary: Short bowel syndrome is a rare disorder characterized by severe intestinal dysfunction, leading to malabsorption and the need for parenteral nutrition support. Treatment is complex and involves various approaches such as nutrition support, fluid and electrolyte management, and pharmacologic therapies.

Summary: De-adaptation of SBS in elderly patients is a rare but important phenomenon that may be related to age-related declining intestinal function. Understanding and emphasizing the concept of intestinal failure in clinical practice can lead to better management of malnutrition and diarrhoea in elderly patients.

Summary: With the increasing survival rate of children with short bowel syndrome (SBS) and intestinal failure (IF), there is a growing need to focus on healthcare transition (HCT) from pediatric to adult care. Adolescents and young adults with SBS/IF should receive disease education, empowerment, and support during this transition. This article reviews the literature on HCT in children with chronic illnesses, discusses barriers and strategies for successful transition, and advocates for structured and multicomponent HCT programs for uninterrupted high-quality care.

Summary: This review summarizes the experimental and clinical studies assessing biomarkers proposed for prognosis or determining response to enteral nutrition. Several pathophysiological mechanisms have been identified that could provide biomarkers for determining response to enteral nutrition. Although several biomarkers have been studied, most studies are insufficient and none have been implemented in clinical practice. Studies mainly focus on simple associations between biomarkers and outcomes, lacking emphasis on dynamic changes in biomarker levels. Furthermore, research into the pathophysiology and clinical features of gastrointestinal dysfunction, including enteral feeding intolerance, is necessary to explore potential mechanisms for specific biomarkers.

Summary: Preoperative nutrition assessment and intervention are crucial for improving postoperative outcomes. The Global Leadership Initiative for Malnutrition (GLIM) criteria are increasingly used in surgical settings to diagnose malnutrition, and novel approaches to assessing muscle mass can identify at-risk patients. Screening and assessing frailty can determine the cost/benefit of surgery and the value of interventions. Evaluation of nutrition risk and status is essential for predicting and mitigating surgical outcomes.

Summary: In a post hoc analysis of a nutrition trial, low prealbumin levels were found to be associated with higher all-cause 180-day mortality rates. Individualized nutrition support was more effective than usual-care nutrition in improving survival at 30 days in patients with low prealbumin levels. These findings highlight the importance of prealbumin levels as a biomarker for assessing nutrition risk.

Summary: This study aimed to assess the feasibility of a digital information-sharing platform to support a digital informed consent process. The platform provided patients with multimedia information on gallstones and treatment options, and allowed them to record their medical history and ask questions. Despite some drop-outs due to difficulties in accessing the platform, patients welcomed the opportunity to receive information digitally before their consultation and felt empowered to be involved in decision-making.
