Additional oligofructose/inulin does not increase faecal bifidobacteria in critically ill patients receiving enteral nutrition: A randomised controlled trial

Background & aims: Patients with diarrhoea during enteral nutrition (EN) have been shown to have low faecal bifidobacteria concentrations. Oligofructose/inulin selectively stimulate the growth of bifidobacteria in healthy humans. This study investigates the effect of additional oligofructose/inulin on the gastrointestinal microbiota, short-chain fatty acids (SCFA) and faecal output in patients receiving EN. Methods: Adult patients in the intensive care unit (ICU) who were starting EN with a formula containing fibre were randomised to receive 7 g/d of additional oligofructose/inulin or an identically packaged placebo (maltodextrin). A fresh faecal sample was collected at baseline and following at least 7 days of supplementation. Faecal microbiota were analysed using fluorescent in-situ hybridisation and faecal output was monitored daily. Results: Twenty-two patients (mean age 71 years) completed at least 7 days of intervention (mean 12 days). At the end of the intervention, there were no significant differences in the concentrations of bifidobacteria between the groups, after adjusting for baseline values (oligofructose/inulin 6.9 + 1.4, placebo 7.8 + 1.3 log(10) cells/g dry faeces, P > 0.05), but there were significantly lower concentrations of Faecalibacterium prausnitzii (7.0 + 1.0 vs. 8.4 + 1.3 log(10) cells/g, P = 0.01) and Bacteroides-Prevotella (9.1 + 1.0 vs. 9.9 + 0.9 logo cells/g, P = 0.05) in patients receiving additional oligofructose/inulin. There were no differences in faecal concentrations of any SCFA, secretory IgA, daily faecal score or incidence of diarrhoea between the two groups. Conclusions: Additional oligofructose/inulin did not increase faecal bifidobacteria in critically ill patients receiving EN, although it did result in lower concentrations of E prausnitzii and Bacteroides-Prevotella. A qualitative exploration: Dietary behaviour of Malaysian breast cancer survivors

Summary: The study found that participants had varying dietary practices, with some following dietary recommendations and others not. The social environment was crucial, as many women relied on family and friends for food choices, and individuals in charge of food preparation had to cater to family preferences. Additionally, individuals found it difficult to maintain healthy dietary changes during the acute survivorship phase due to a lack of health consciousness and limited access to healthy food. Importantly, there was a lack of dietary guidance from healthcare professionals, especially dietitians, in long-term survivorship care.

Changes in Dietary Intake of Breast Cancer Survivors: Early Findings of a Malaysian Breast Cancer Prospective Cohort Study

The study suggests the need for more proactive dietary interventions and continuous monitoring among breast cancer survivors for their survivorship care.

Summary: This study analyzed the gut microbiota composition of multi-ethnic IBS patients and explored the impact of a low FODMAP diet on their gastrointestinal symptoms and gut microbiota. The results showed that IBS patients had a greater abundance of Parabacteroides species and lower levels of bacterial fermenters and short-chain fatty acids producers compared to healthy controls. Following a low FODMAP diet, about 70.6% of IBS patients experienced symptom and quality of life improvement. Rating: 8/10.

A systematic review of the relationship between household food insecurity and childhood undernutrition

Summary: This systematic review explores the association between household food insecurity (HFIS) and undernutrition in children under 5 years old, highlighting the need for policymakers to consider key factors in tackling childhood undernutrition and HFIS. The review found a significant association between HFIS and undernutrition, particularly stunting and underweight, across all national income levels. Therefore, sustainable and inclusive economic growth should be a key policy goal in addressing food insecurity and childhood undernutrition, and multisectoral interventions are needed.

Barriers and facilitators to education access for marginalised non-citizen children in Malaysia: A qualitative study

Summary: In Malaysia, marginalised non-citizen children are excluded from formal education at public schools. This study explores the barriers and facilitators to educational access among various marginalized children, and highlights the importance of legislative/policy changes, financial constraints, cultural norms, proximity to learning centers, and institutional discrimination. It suggests the gradual inclusion of all children in public schools, the recognition and support of learning centers, and the realization of the right to work for refugees and stateless peoples in order to achieve universal education access.

Standard vs. Nutrient-Enriched Cow's Milk and Its Impacts on Child Growth: A Systematic Review and Meta-Analysis

Summary: Stunting in children is caused by malnutrition or undernutrition, which hinders their growth and has negative effects on their overall health. This study reviewed the effects of different types of cow's milk on children's growth. The results showed that standard cow's milk may be more effective in promoting children's growth compared to nutrient-enriched cow's milk, but more research is needed on the effects of standard milk on child growth. Inconsistent findings were found between nutrient-enriched cow's milk and children's growth. It is important to ensure that children's diets include recommended nutrient intake, including milk.

Association between stress and eating behaviour among Malaysian adolescents prior to examination

Summary: Studies have found that mental health issues and obesity among adolescents are increasing due to urbanization and changes in lifestyle. This study aimed to investigate the level of stress and its impact on eating behavior among Malaysian adolescents. A total of 797 multi-ethnic Malaysian secondary school students participated in this cross-sectional study. The findings showed that 29.1% of adolescents experienced high stress, with a mean saliva cortisol level of 3.8 nmol/L. There was a positive correlation between perceived stress and emotional overeating, which was stronger among urban, female, underweight, and moderately stressed adolescents. Additionally, a positive correlation was found between perceived stress and food responsiveness, which was strongest among Malay, male, underweight, and highly stressed adolescents. The study suggests that the perceived stress level prior to exams can influence the emotional and external eating patterns of adolescents.

Strategies such as awareness campaigns, sodium content labeling, and product reformulation have been implemented, but face barriers such as poor consumer acceptance and lack of guidelines. Enablers such as prioritizing salt reduction, creating awareness, collaboration, research and technology, and incentives and salt tax have been proposed.

A systematic review on factors associated with non-alcoholic fatty liver disease (NAFLD) among adolescents

Summary: This systematic review aimed to determine the prevalence and factors contributing to non-alcoholic fatty liver disease (NAFLD) among Asian adolescents. The studies included in the review showed a prevalence of NAFLD ranging from 8.0% to 16.0% among adolescents, with unhealthy diet and lack of physical activity being potential factors. Although most studies found no association between lifestyle factors and NAFLD, there is still evidence showing a link.
