Objectives: To investigate the behavioural and neurophysiological pattern of cataplexy. Methods: Seven narcolepsy with cataplexy patients underwent daytime videopolygraphy using humorous movies or/and jokes to trigger cataplectic attacks. Results: During segmental cataplectic attacks, EMG showed brief and irregular periods of silencing focally involving facial, neck, axial or limb muscles, sometimes coinciding with bursts of rapid eye movements. All patients enacted intentional movements in response to these segmental postural lapses. During global cataplectic attacks, EMG showed suppression of activity alternated with patterned enhancement, enhanced EMG activity in neck muscles preceding that of other cranial, axial and lower limb muscles. This waxing and waning EMG pattern ended with a complete body collapse and persistent muscle atonia. Breathing irregularities, heart rate (HR) instability and EEG desynchronization were observed during global cataplectic attacks without any appreciable blood pressure changes, but with HR deceleration and silencing of sympathetic skin response while in complete atonia. Patients subjectively perceived the involuntary postural lapses as startling and alarming. Conclusions: Cataplexy in our patients showed many of the features of tonic REM sleep. Significance: Cataplexy can be construed as a freezing-like perturbation of the orienting response with transient impairment of posture and movements resulting in a patchwork-compromise-behaviour. Being creative during lockdown: The relationship between creative potential and COVID-19-related psychological distress in narcolepsy type 1

Anita D'Anselmo, Sergio Agnoli, Marco Filardi, Fabio Pizza, Serena Mastria, Giovanni Emanuele Corazza, Giuseppe Plazzi

Summary: The study explored psychological distress associated with COVID-19 restrictions and its relationship with depressive symptoms and creativity in patients with narcolepsy type 1 (NT1). Results showed that patients with NT1 experienced improvements in subjective sleepiness while controls reported worsening sleep quality during the lockdown. Depression and NT1 symptom severity were significant predictors of COVID-19-related distress. Creative performance, particularly generative fluency, was found to reduce the detrimental effect of depression on the patients' wellbeing. In contrast, creative originality was a disadvantageous moderator in the relationship between NT1 symptom severity and distress associated with the traumatic event.

Summary: This study aims to explore the associations between problematic online pornographic use (POPU), psychological stress, emotion dysregulation and insomnia symptoms, and understand the mediating role of psychological stress and emotion dysregulation in the relationship between POPU and insomnia symptoms. The findings showed that higher psychological stress and emotion dysregulation fully mediated the link between POPU and insomnia symptoms.

Portrayals of narcolepsy from 1980 to 2020: a descriptive analysis of stigmatizing content in newspaper articles

Giorgia Varallo, Luca Pingani, Alessandro Musetti, Gian Maria Galeazzi, Fabio Pizza, Gianluca Castelnuovo, Giuseppe Plazzi, Christian Franceschini

Summary: This study aimed to describe how narcolepsy is portrayed in newspapers. The findings showed that there is a certain degree of stigmatization and stereotyping in the media portrayal of narcolepsy. The results provide guidance for improving how narcolepsy is depicted in the media and can inform the development of targeted anti-stigma campaigns.

Summary: This systematic review and meta-analysis found that sleep disturbances and disorders are significantly associated with chronic postsurgical pain, but with a small effect size. The association varies in different patient groups and outcomes, possibly due to various factors. Therefore, preoperative evaluation of sleep issues is important in identifying patients at risk for chronic postsurgical pain.

Summary: This study aimed to evaluate the effects of adding WBC intervention to a multidisciplinary rehabilitation program for FM patients with obesity. The results showed that the group that underwent WBC intervention had greater improvements in pain severity, depressive symptoms, disease impact, and sleep quality. However, no significant differences were found in performance-based physical functioning between the two groups.

Summary: The study aimed to explore gender and sociodemographic differences in psychological resilience in older adults during the COVID-19 lockdown in Italy. Results showed that older women had lower resilience scores compared to men, and older adults in relationships or with children had higher resilience scores. Additionally, lower resilience scores were associated with higher levels of stress, anxiety, and depression.
