The extent of resection in high grade glioma is increasingly been shown to positively effect survival. Nevertheless, heterogeneity and migratory behavior of glioma cells make gross total resection very challenging. Several techniques were used in order to improve the detection of residual tumor. Aim of this study was to analyze advantages and limitations of fluorescence and image guided resection. A multicentric prospective study was designed to evaluate the accuracy of each method. Furthermore, the role of 5-aminolevulinc acid and neuronavigation were reviewed. Twenty-three patients harboring suspected high grade glioma, amenable to complete resection, were enrolled. Fluorescence and image guides were used to perform surgery. Multiple samples were obtained from the resection cavity of each lesion according to 5-ALA staining positivity and boundaries as delineated by neuronavigation. All samples were analyzed by a pathologist blinded to the intra-operative labeling. Decision-making based on fluorescence showed a sensitivity of 91.1% and a specificity of 89.4% (p < 0.001). On the other hand, the image-guided resection accuracy was low (sensitivity: 57.8%; specificity: 57.4%; p = 0.346). We observed that the sensitivity of 5-ALA can be improved by the combined use of neuronavigation, but this leads to a significant reduction in specificity. Thus, the use of auxiliary techniques should always be subject to critical skills of the surgeon. We advocate a large-scale study to further improve the assessment of multimodal approaches. Delayed onset of fatal encephalitis in a COVID-19 positive patient

Luca Zanin, Giorgio Saraceno, Giulia Renisi, Liana Signorini, Luigi Battaglia, Marco Ferrara, Francesco Antonio Rasulo, Pier Paolo Panciani, Marco Maria Fontanella

Summary: This case report describes the delayed onset cell-mediated encephalitis caused by COVID-19 virus, which is the first recorded case occurring more than 40 days after the diagnosis. The patient experienced intense headache, fever, and subsequent generalized epileptic seizures before being diagnosed with brain death.

Treatment strategy for vertebral metastases from anal squamous cell carcinoma: a comprehensive literature review and case report

Edoardo Agosti, Simona Serioli, Kanwaljeet Garg, Alberto Pietrantoni, Pietro Luigi Poliani, Marco Maria Fontanella

Summary: This study reported a rare case of multiple vertebral metastases from anal squamous cell carcinoma (SCC) and aimed to define a treatment strategy. Through case presentation and literature review, it was found that there is currently no universally accepted treatment strategy for vertebral metastases from anal SCC. Prompt and aggressive, local and systemic, and multimodal treatment of the vertebral lesions may be crucial for improving patient outcomes.

The Role of Diskectomy in Reducing Infectious Complications after Surgery for Lumbar Spondylodiscitis

Summary: Surgery for pyogenic Spondylodiscitis as an adjunct to antibiotic therapy is an established treatment, but the technique and extent of surgical debridement remain a matter of debate. Some advocate diskectomy in all cases, while others argue that stand-alone instrumentation is sufficient.

Compartmental Cerebrospinal Fluid Events Occurring after Subarachnoid Hemorrhage: An Heparin Oriented Systematic Review

Fulvio Tartara, Andrea Montalbetti, Emanuela Crobeddu, Daniele Armocida, Eleonora Tavazzi, Andrea Cardia, Marco Cenzato, Davide Boeris, Diego Garbossa, Fabio Cofano

Summary: This paper provides a practical review of the pathophysiological events following subarachnoid hemorrhage (SAH). The complex cascade of pathophysiological events involves various interconnected but distinct pathways. The use of heparin, with its anticoagulant effect and ability to interfere with secondary events, may be an interesting therapeutic strategy for SAH management.

Can Post-Operative Posterior Reversible Encephalopathy Syndrome (PRES) Be Considered an Insidious Rare Surgical Complication?

Alessandro Frati, Daniele Armocida, Fulvio Tartara, Fabio Cofano, Sergio Corvino, Sergio Paolini, Antonio Santoro, Diego Garbossa

Summary: This study reviewed 47 cases of posterior reversible encephalopathy syndrome (PRES) that occurred after surgery to identify the clinical features, timing of symptom onset, and treatment of this rare surgical complication. The results showed that postoperative PRES can occur in both pediatric and adult patients, with cardiovascular disease being the most common comorbidity. PRES typically occurs within the first 3 weeks after surgery, and prompt recognition and treatment can lead to complete reversal of symptoms in most cases.

Radiofrequency Ablation in Vertebral Body Metastasis with and without Percutaneous Cement Augmentation: A Systematic Review Addressing the Need for SPINE Stability Evaluation

Stefano Colonna, Andrea Bianconi, Fabio Cofano, Alessandro Prior, Giuseppe Di Perna, Giuseppe Palmieri, Gianluigi Zona, Diego Garbossa, Pietro Fiaschi

Summary: Vertebral body metastases (VBM) are a common site of bone metastasis, and the treatment management presents challenges. Radiofrequency ablation (RFA) is a potential alternative, but it may affect the biomechanics and microanatomy of the vertebral body, increasing the risk of fractures and instability. This study aims to investigate the impact of RFA on spine stability.

Updates in Glioblastoma Immunotherapy: An Overview of the Current Clinical and Translational Scenario

Summary: Glioblastoma (GBM), the most common and aggressive central nervous system tumor, is difficult to treat due to its malignant behavior and infiltrative growth pattern. Although immunotherapeutic strategies hold promise for the fight against GBM, immune resistance and the immunosuppressive environment pose challenges. New therapeutic strategies targeting the immune system, such as immune checkpoint inhibitors and chimeric antigen receptor T cell therapy, have been explored, but significant results have not been achieved. The future direction of GBM therapy may involve a combined approach that includes multifaceted immunotherapy to directly kill tumor cells and activate the immune response.

Summary: Liquid biopsy research on Low-Grade gliomas (LGG) has remained less conspicuous than that on other malignant brain tumors. Reliable serum markers would be precious for diagnosis, follow-up and treatment. Micro-RNA (miRs) appears to have the highest clinical utility.

Summary: This study investigated the associations between specific single-nucleotide polymorphisms (SNPs) in the CYP2A6 gene and the presence and rupture of intracranial aneurysms (IAs) in Italian patients. The results showed that the frequency of aSAH was significantly higher in current smokers, regardless of the pattern of CYP2A6 SNPs. There was a correlation between IA rupture and cigarette smoking in patients with the heterozygous CYP2A6 *1B2 allele.

Summary: This study compared the surgical volumes and exposure areas of different surgical approaches to Meckel's cave and found that endoscopic approaches can provide adequate exposure, especially for the medial portions, while microsurgical transcranial approaches still seem to be the gold standard for optimal exposure and surgical volumes in the most lateral portion of Meckel's cave.

Neuropsychological Evaluation and Functional Magnetic Resonance Imaging Tasks in the Preoperative Assessment of Patients with Brain Tumors: A Systematic Review

Summary: This study systematically reviewed the scientific literature on the topic of preoperative neuropsychological tests and fMRI tasks. The results showed a strong correlation between neuropsychological and fMRI results, with language being the most commonly evaluated cognitive function. The study also suggested that selecting well-adjusted and individualized neuropsychological tasks may enable the development of personalized and more efficient protocols. Furthermore, fMRI findings may help identify plasticity phenomena to avoid unintentional damage during neurosurgery.
