Summary: This study found a higher 1-year mortality rate in very elderly patients with bacteraemia, as well as an increased risk of death in patients with quick Sepsis Related Organ Failure Assessment (qSOFA) score of 2 or more and age of 90 years or older. This suggests that bacteraemia is associated with a poor prognosis and higher long-term mortality in elderly patients suspected of sepsis.

Summary: Early identification of kidney transplant recipients at risk of progressive allograft dysfunction is crucial for providing closer monitoring and more aggressive risk factor modification. The use of latent class analyses presents advantages in classifying recipients into risk categories, but also poses challenges in terms of clinical applicability and model derivation.

Summary: The study aimed to measure mitochondrial ROS levels in peripheral blood lymphocytes within 24 hours of admission in critical care patients and correlate it with survival at one year post-ICU hospitalization. Oxidative stress levels, measured as DHE fluorescence, showed a positive correlation with increased long-term mortality. In ICU patients, oxidative stress was found to be the major determinant of survival, outlining the cellular response to inflammatory stimuli.

Summary: The Oncology Grand Rounds series aims to help doctors better apply clinical research to actual patient treatment. This particular round focused on the management and complications of hematopoietic stem-cell transplantation (HSCT) in acute myelogenous leukemia (AML). The study found that while transplantation therapy provides long-term survival opportunities, it also increases the risk of late complications.

Summary: Millions of sepsis survivors suffer from late pulmonary complications, possibly due to persistent immune reprogramming. This study used a murine sepsis model to show enhanced/primed immune responses in the lungs and identified S100A8/A9 as a potential biomarker and therapeutic target for long-term pulmonary complications. This provides insight into the mechanisms mediating enhanced immune responses in sepsis survivors and paves the way for further investigation.

Summary: This study examines patient outcomes after recurrence of esthesioneuroblastoma and describes patterns of salvage therapeutic interventions. The 5-year overall survival after recurrence was found to be 63% and the 5-year progression-free survival was 56%. The time to develop a secondary recurrence was shorter than the time to develop the first recurrence, and patients with secondary recurrences were older at the time of their primary tumor diagnosis.

Summary: Coronary artery bypass grafting (CABG) is a common cardiac surgical procedure. This study aimed to assess the impact of perioperative infection on 5-year and 10-year survival in patients undergoing elective on-pump CABG surgery. The study found a strong relationship between post-operative infections and 5-year and 10-year mortality in CABG patients. Pneumonia and bloodstream infections were the only types of infection that significantly increased long-term mortality after CABG surgery.

Summary: This study evaluated long-term survival and prognostic factors for patients with autoimmune hepatitis (AIH) after liver transplantation (LT), particularly the impact of recurrent AIH (rAIH). The results showed that AIH patients had excellent survival rates after LT, but disease recurrence and chronic rejection reduced graft survival.

Summary: The study analyzed the long-term outcomes in 170 patients who underwent surgery for pheochromocytoma and sympathetic paraganglioma (PHEO/sPGL). It was found that even apparently benign and sporadic variants had a risk of long-term recurrence, highlighting the importance of lifelong follow-up.

Summary: The study investigated the effects of bisphosphonates on long-term graft outcomes after kidney transplantation and found that bisphosphonate therapy may improve long-term graft survival in kidney transplant recipients, particularly for those with osteopenia or osteoporosis. The use of bisphosphonates was associated with higher death-censored graft survival and significantly reduced risk of graft failure in the study cohort.

Summary: The study found that the use of TVH for early-stage endometrial cancer patients in the U.S. has decreased, with age being the primary factor influencing the performance of TVH. TVH does not negatively impact cancer-specific survival compared to TLH.

Summary: The study shows that a 121 metastasis-associated gene signature identified using differential expression analysis is significantly associated with progression-free and overall survival in melanoma patients. Machine learning models trained with these signature genes outperformed models using clinical covariates or published prognostic signatures in predicting metastases. The signature negatively correlated with immune cell infiltration and could potentially help stratify early-stage melanoma patients for further evaluation in clinical trials.
